Employee was at sand & gravel plant and went to pull lever, slipped, and fell. EE put out hand to catch self and the left hand hit a few crushed #2 gravel on deck and cut hand.

He was stepping down off a tractor to refuel and he slipped off the step. He landed on his back on the concrete pad and hit a piece of steel on the pad.
Employee is a mechanic who was assembling a motor in a dozer. He was climbing up and down on stepladder carrying parts and thinks he strained his back and hips.
Employee was raking out gravel from underneath the crusher and one stone came out and hit him in the forehead. He was wearing a hard hat and safety glasses.
Employee was working on crusher deck which is 3 feet off ground; he went to grab a bar, slipped and landed on the ground on his feet. He then stumbled backwards and fell on a piece of steel on the ground.

Employee states he wears full hearing protection and equipment has tested ok for noise levels. He had to be retested due to ear wax build up, which he has had problems all year long with. He is part-time and on off time he farms and cuts wood. We cannot see any work related loss from our investigation.
