Miner was refueling a haul truck, using a ladder to access the fuel receptacle. While descending the ladder, the minor lost footing and missed a rung and landed on the right ankle causing a sprain.

EE was attempting to cut a zip tie with a razor knife. Knife was in right hand and EE was holding zip tie with left. The knife slipped and lacerated the EE's left palm.

Mechanic was using a pry bar to straighten a bearing on a dog bone on a CAT 777 when the pry bar slipped and the dog bone fell cutting EE's finger.
Barge loader was moving a piece of cable. Pulled back while pulling to move cable.
Employee was ascending an access ladder to the right-hand side of a 992K Cat loader to check engine fluid levels. Their left hand slipped on the wet hand hold on the ladder, they used their right arm to correct the fall. In the process they aggravated a previous non work shoulder injury.

Employee was in process of removing a speed reducer from the head pulley of a conveyor due to it failing. After removing the bolts from the moment coupling, the employees were unbolting the reducer from the mounting frame when it suddenly fell over crushing an employee's left foot and breaking their right arm, right thumb, and right finger.

Employee was working routine work and it was a very windy day. Dust/dirt particles blew behind EE's safety glasses into EE's eye.

Employee was walking and tripped on frozen ground from tire travel tracks. Employee failed to have proper awareness of surroundings
Employee was attempting to climb up ladder to haul truck at start of shift.
Employee using grinder broke thumb. Should have used vice to hold metal prior to grinding.
Employee not following procedure climbed 6' ladder that was not level, employee was not wearing PPE gloves, the ladder tipped and employee cut finger.
Employee was lifting hammer lift assembly without help. Equipment too heavy for one person. Employee stated was angry with co-worker so didn't follow procedure. The hammer assembly slipped hit right big toe. Employee has language barrier. EE didn't inform us of the accident until investigation was launched on 8-13-19.
Employee got finger caught between screen and guard.
Employee was trying to dislodge a rock from the primary crusher with a 30lb jack hammer when a piece of rock struck EE in left wrist and arm knocking EE backwards.
EE noticed winch cable had slack and was hanging down the side of dock. EE grabbed a pole to try to grab cable and move it to proper side of gavel. The winch cable was activated causing the cable to jump the gavel striking EE on the outside of left knee.

Employee was walking near river bank feeling with foot to determine edge of bank. Once employee felt employee located it employee knelt down to feel with hand the ground gave way and employee fell about 10 feet.

It was stated that the accident happened on 12/29/2015 but not reported until 1/8/2016. Was hooking water hose to fill up the water truck and tripped over wooden blocks and lost his balance. Says he aggravated his left shoulder.
Employee was washing out from under the crusher platform using a water hose and spray nozzle to clean out build-up that had accumulated over the last week. While doing so the water hose burst causing the hose to whip and splashed mud and debris in his face and right eye.

EE WAS OVERSEEING CORE DRILLING OPERATION. HE ATTEMPTED TO ALERT DRILLER BY STEPPING ONTO PLATFORM, PLATFORM WAS MUDDY & HE SLIPPED. HE SLIPPED & STRUCK HIS RT. RING FINGER AGAINST A DRILL LIF TER, CUTTING IT WHICH REQUIRED STITCHES.
EE WAS LIFTING FLOOR GRATE IN RECLAIRE TUNNEL, STEPPED BACKWARD AND TWISTED BACK.

CUTTING METAL BIN STRAPS ON METAL WITH CUTTING TORCH. CUT ONE OF THE STRAPS THEN LOOKED AROUND TO SEE WHERE THE NEXT ONE WAS. HE WAS STEADYING THE TORCH AGAINST HIS RIGHT HIP AND AS HE TURNED THE TORCH TURNED TOO. HE FELT THE HEAT BUT DIDN'T REALIZE IT WAS BURNED UNTIL HE FINISHED.

THE LID OF THE BATTERY BOX HAS TO B E OPENED IN ORDER TO GAIN ACCESS TO THE LIGHT. THE DOOR TO THE CAB WAS OPEN, BUT WAS SHUT BY ANOTHER EE, CAUSING THE BATTERY BOX LID TO FALL. TEN SUTURES TO CLOSE. INJURED WAS WELDING ON LOADER BUCKET OUTSIDE IN WINDY CONDITIONS. HE GOT WELDING SLAG IN HIS LEFT EYE. INJURED WAS WEARING WELDING HELMET AND SAFETY GLASSES. SINCE THE WELDING SLAG WAS WEDGED IN LEFT EYE, IT HAD TO BE DRILLED OUT.

ON 9-29-94 TWO EE'S WERE WELDING A PATCH IN DISCHARGE CHUTE ON #4 BELT. EE WAS STANDING ON THE PLATE THAT OTHER EE WAS WELDING. ON 9-30-94, EE TOLD OTHER EE THAT HE THOUGHT HE HAD BURNED HIS F OOT. ON MONDAY, 10--3-94 AT ABOUT 1:00 PM EE TOLD TEAM LEADER THAT HE WAS GOING TO THE DR,TO SEE ABOUT HIS FOOT, AS HE THOUGHT HE HAD BURNED IT WELDING THE PATCH IN CHUTE LAST WEEK.
A LOADED BARGE WAS LEAKING. HE WAS ON THE BARGE AND I.NAME WAS ON DOCK BARGE HANDING HIM A 2" SUCTION HOSE WITH A 12" SQUARE PLATE FOR A SCREEN AND WEIGHT ON THE HOSE. HE HAD THE PLATE IN HAND AND THE HOSE TWISTED, CAUSING HIS HAND TO HIT THE BARGE WHILE HOLDING ONTO THE PLATE. SUTURES AND SPLINT WERE REQUIRED.
EE WAS PULLING BARGE BRAKE CABLE ALONG SIDE OF DOCK BARGE TO HOOK UP CARGO BARGE WHEN THECABLE FELL OFF DOCK BARGE INTO THE WATER. EE WAS HOLDING THE BRAKE CABLE IN ONE HAND AND HOLDING TO SAF ETY CABLE WITH OTHER HAND. WHEN CABLE FELL INTO WATER, IT PULLED EE. HIS BACK HURT BETWEEN SHOULDERS. EE WAS PUT ON LIGHT DUTY.
