Employee stated was moving a 55 gallon barrel of oil by hand when EE felt something pop in right shoulder. Employee stated did not want medical attention, just wanted to document the incident.

Employee was driving a dump truck moving rock material from the pit area to the load point for the crusher when he stated that he hit a dip in the travel way and jarred his neck. Employee works at the quarry site as a truck driver only, he is not a full time quarry/miner.
Employee was using an air-wrench to loosen nuts on the stripper bar; when the nut broke loose the air-wrench released and smashed the employees finger between the air-wrench and the side of the crusher.
Employee/Miner was centering the new belt that had just been installed when he pulled a muscle in his lower back. He is scheduled to return to work on Tuesday, May 26th, 2015 after his follow-up doctor visit.
Employee was removing rocks that had gotten jammed in the crusher; as he was removing the rocks, one of them shifted and smashed his left pinky finger between the rock and the crusher plate.

Operator was getting onto the dozer when he missed the hand grip and fell backwards landing against a pickup truck parked next to the dozer. The operator incurred minor injuries to his back and left leg. Operator returned work on light duty on 7/20/2011.

At about 745 a.m. employeee, a contractor for explosive energies, went to open a side door of his work truck. When the door lock opened the latch hit him in the mouth and cut his lip. He was taken to a clinic for treatment. This person got six stitches in his lip.

EE WAS REMOVING HYDRAULIC LINE WHEN PRESSURE RELEASED CAUSING HYDRAULIC FLUID AND DEBRIS TO COMEIN CONTACT WITH FACE AND RIGHT EYE. EE STATED HEWAS WEARING SAFETY GLASSES AT THE TIME.
EE WAS PROPING UP AN IRON PLATE TO MEASURE WHEN THE PLATE FELL ON HIS LEFT FOREARM CAUSING A HAIRLINE FRACTURE. NOTE: THE MACHINARY WAS COMPLETELY LOCKED OUT AND SHUTDOWN WHEN INJURY OCCURRED.

EE WAS SECURING A SIGN POST WITH HIS HANDS WHILE ANOTHER EE WAS OPERATING THE LOADER BUCKET TO DRIVE THE POST INTO THE GROUND WHEN THE POST SLIPPED AWAY FROM UNDERNEATH THE BUCKET FORCIBLY STR IKING HIS THUMB.
