They were putting belt on conveyor. Chain came off of bar holding belt. Belt swung over and hit employee who was on a ladder and tipped him and the ladder over. Broke his left arm
Went to step on a wet concrete block to get at a guard and slipped off and fell over.

We were putting in a tile, I dug the hole about 7'wide 7' deep. We were working on the 2nd half and a shovel fell in. EE said he would get it, I told him no the bank needed cut wider. As I was doing that he went in to get it I yelled for him to stop as the bank was caving. He tried to run but didn't make it.
EE was cutting a conveyor belt and the utility knife slipped and went into his leg just to the side and above the knee. EE was cutting belt in the rain. EE received 2 stitches.

Employee was taking the belly pan off 250 E. to wash. Was on his knee's with impact in right hand at arms length above his head. When bolt came loose pan fell and pulled his back trying to get impact out.

EE was climbing down off the dragline & when he stepped down the ground was uneven he fell. Broke his right leg & has a chip on his right ankle.

EMPLOYEE WAS HAULING MATERAIL FOR STOCKPILE WHEN FRONT LEFT TIRE ON THE TRUCK BLEW-OUT CAUSING THE STEERING WHEEL TO ABRUPTLY PULL TO THE LEFT AND JOLTING AND TWISTING THE DRIVER'S ARMS AND SH OULDERS AS HE TRIED TO KEEP IT UNDER CONTROL. THERE WAS NO NOTICEABLE INJURY AT THE TIME, BUT THE EMPLOYEE BEGAN TO HAVE BACK AND SHOULDER PAINS A FEW DAYS LATER.
