the clamps on a 2" air line failed and the whip check did not properly arrest the hose and it struck a miner in the left ankle causing a broken leg.

While attaching an air line for a Drill the miner was tightening up the 2.5" line when they tapped the ears on the fitting with a small hammer the miner did not notice that the valve was cracked and the valve separated and struck the miner in the face neck and chest.

Using an iron bar to move a small horizontal DTH drill when the drill started to tip over the miner reached out to grab the drill to try to stop it from rolling over and dislocated shoulder

Reported to work complaining of pain in both hands. Suspect carpal tunnel

Employee was walking and tripped over some used drill bits that were lying on the ground.

While threading a bit on to a drill steel the drill steel fell pinning the employees right hand to the ground.

Employee was working outside during cold weather and got frost bite on his toes.

Employee was pulling on a diamond wire to free it after it had become frozen over night. During the process felt pain in lower abdomen. Visit to Doctor confirmed that it was a hernia.

Employee reported soreness in their right wrist/hand which was diagnosed as Carpel Tunnel Syndrome.

Employee was unplugging a cord and sprained his wrist.

Loading drill steel he felt pain in his right shoulder.

He was trying to whip a power cord off a pin installed in quarry wall.

While moving a quarry drill the crane swung to the right and caught the injured between that drill and another drill nearby.

Loose clothing caught with slot drill rod twisting arm.

Crack opened up unexpectantly and employee lost his balance and fell.

Employee said he twisted his knee in April but never told anyone.

Employee stepped in a hole as he was walking and twisted his knee.

As employee was removing plug wedges from stone, the stone opened up and the employee struck his forearm on the edge of the stone, causing a laceration which required sutures to close.

While putting oil in Uclid employee slipped on ice and fell to ground causing abdominal strain and pain in right leg.

While putting a drill rod in the shank - the shank was turning and the rod caught his hand and twisted it causing ligament strain to wrist, elbow and shoulder.

Injured was holding a wrench on a bolt, when another employee tried to loosen a nut with an impact wrench. The bolt spun causing the wrench to move striking his thumb against the drill, causing a crushing injury. Sutures needed to close wound.

WHILE LIFTING A PIECE OF DRILL STEEL EMPLOYEE FELT PAIN IN HIS BACK.

EMPLOYEE BENT OVER THEN STRAIGHTENED UP AND FELT PAIN IN HIS BACK.

AS EE WAS QUARRYING STONE, HE FELT PAIN IN GROIN AREA.

HE WAS PUSHING THE DETINATOR TO MAKE A BLAST. HE LOOKED UP AND WAS HIT BY A PIECE OF FLYING STONE.

WHILE SETTING UP THE DRILLK THE DRILL TIPPED OVER, WHEN THE D5RILL TIPPED, HE MOVED OUT OF THE WAY AND FELL DOWN.

WHILE MOVING THE SLOT DRILL WITH THE PRY BAR, HE FELT A PAIN IN GROIN AREA.

AS EE WAS LIFTING A JACKHAMMER HE FELT PAIN IN HIS LOWER BACK.

WHILE SPLITTING THE BLOCK OF GRANITE, HE BROKE A PIECE AND IT FELL AGAINST HIS ARM CAUSING A LACERATION.

SETTING UP STAGING,TURNED AND FELT PAIN IN BACK.

EMPL MOVING MAST OF BAR DRILL TO END OF BARREL,HAND ON LEVERS CAME IN CONTACT WITH LADDER LAYING AGAINST DRILL PINCHING HAND BETWEEN LEVERS AND LADDER.

AS EMPL WAS REMOVING HOSE FROM AIRTRAC HE SLIPPED ON OILY SURFACE.

AS EMPLOYEE PICKED UP A PIECE OF GROUT TO THROW INTO THE GROUT BOX, A PIECE OF GRANITE THAT WAS UNDER THE ONE HE PICKED UP FELL ON HIS INSTEP.

EMPL WAS PULLING DRILL ROD OUT OF LIFT HOLE WITH CHAIN PULLER WHEN HIS THUMB WAS CAUGHT BETWEEN CHAIN AND MACHINE CAUSING A FRACTURE TO HIS L THUMB

EMPLOYEE WAS WASHING OUT PUMP HOSE HE LEANED ONN STAGING WAS ON UNEVEN GROUND AND TIPPED OVER ALLOWING EMPLOYEE TO FALL TO THE GROUND FIVE FEET BLOW,HIS LEG STRUCK METAL CULVERT CAUSING ALACE RATION TO LEG REQURING THREE STITCHES TO CLOSE.

LOADING GROUT INTO BOX FELT PAIN IN BACK.

INJURED EMPL WAS LIFTING 20' BAR ROD & TURNED BODY BUT DID TURN FEET CAUSING POSSIBLE TORN LIGAMENTS IN KNEE DID NOT REPORT UNTIL HE WENT TO DRS ON 3/4/89/

EMPLOYEE WAS SQUATTING DOWN HELPING FOREMAN MEASURE A PIECE OF STONE. EMPLOYEE WENT TO STAND UP, BACK MADE SNAPPING SOUND, PAIN IN LOWER BACK.

EMPLOYEE CLEANING SUMP HOLE ATTEMPTED TO MOVE PUMP WITH LOADER. LOADER TIRES WENT INTO DEPRESSION, MACHINE MOVED FORWARD PINNING EMPLOYEE BETWEEN BUCKET AND GRANITE WALL. SCRAPES AND BRUISES TO PELVIS REGION.

EMPLOYEE WAS STRIKING FEATHER WEDGER WITH SLEDGE HAMMER WHEN ONE DISLODGED & STRUCK HIM IN THE FOREHEAD CUTTING WOUND REQUIRED FOUR STITCHES TO CLOSE

EMPLOYEE WAS A7TEMPTING TO LIGHT THE AIR TORCH WHEN IT FLASHED BACK INTO THE O2 LINE

EMPLOYEE BENT D2WN TO PICK UP END OF BROWNING AIR TORCH AND HURT HIS BACK.
