In the process of changing the clean coal tubes the employee flopped the gate from tube 2 to tube 1. EE hand and glove on right hand got caught on the flop gate handle and the tube. This resulted in a cut to hand. EE was taken to the hospital where ee received 8 stitches and returned to work. EE has missed no work.

Employee was pulling a wash down water hose up to the next floor of the prep plant by hand. The water hose slipped through right hand and a metal hose clamp pulled through their hand and cut the inside of the hand. Employee took themselves to the hospital and received 8 stitches. Employee returned to work that same evening and has missed no work.

Employee sprained lower back and right shoulder lifting to help install clean coal screens in the prep plan. Employee continued to work and went to the doctor on 08/15/2017 and was taken off from work until 08/29/2017.

EE was cleaning a supply building and a box of miscellaneous parts fell and struck him on the left little finger.

EE was throwing the flop gate in the prep plant. It became fouled, and he was shaking it with his left hand. When the flop gate freed, it caught his finger against a piece of angle iron, cutting it.

EE was helping set a re-tracker for a de-railed railroad car and the CSX employee dropped his side of the re-tracker, resulting in the re-tracker falling on EE's left ring finger cutting it.

EE was working on the 3rd floor of the Screening Plant. He was at the sizing grid and stepped on a piece of floor grating. The piece of grating gave way and EE fell approximately 12 ft. to the floor below.
Employees were sliding steel beams across the floor. Injured employee's end of the steel beam hit a pipe and moved to the left catching his left hand (middle finger) between the beam and the pipe.

EE WAS HELPING CHANGE A 40 HP ELECTRIC MOTOR IN PREP PLANT. THE MOTOR WAS ON A TOW-WHEEL DOLLY. MOTOR STARTED TO FALL OFF DOLLY. EE GRABBED IT TO STABILIZE IT AND STRAINED LOWER BACK.
EE WAS USING A HIGH PRESSURE WATER HOSE TO WASH THE FLOOR IN THE BASEMENT OF PREP PLANT. WHEN THE WATER WAS TURNED ON, THE PRESSURE CAUSED HIM TO LOOSE HIS BALANCE. HE FELL ONTO A CEMENT BEAM CAUSING PAIN IN RIB AREA.
