A ROOF FALL OCCURRED ON 7-12-90 WHILE MINING COAL BY USING THE DEEP CUT PILLAR EXTRACTION METHOD. SHALE FELL IN THE INTERSECTION OF THE #2 ENTRY 4 BREAKS INBY SPAD #9279. THIS FALL COVERED THE NUMBER 29 CONTINUOUS MINER WHICH WAS BEING INVOLVED IN THE REMOVAL OF COAL. THE FALL MEASURED 4' THICK, 20' WIDE AND 50' LONG. THE AREA IS BEING CRIBBED AND TIMBERED.
EMPL REMOVING TRAILING CABLE TO CONTINUOUS MINER FROM ROADWAY HE WAS TRAVELING WHICH WAS BLOCKING HIS PASSAGE.HE PLACED CABLE OVER LEFT SHOULDER BEGAN PULLING WHILE GRASPING CABLE WITH BOTH HA NDS.EXPERIENCED PAIN IN HIS BACK.
OPERATING SHUTTLE CAR TRANSPORTING COAL FROM CONTINUOUS MINER TO FEEDER ON WORKING SECTION.I.NAME TRAVELING OUTBY IN NO.6 ENTRY WHEN SHUTTLE CAR STRUCK DEPRESSION IN ROADWAY CAUSING I.NAME TO BE THROWN UPWARD AND AGAINST OPERATORS CANOPY.
A ROOF FALL OCCURRED IN THE NUMBER TWO ENTRY, APPRO9X. 60 FEET WIDE AND 20 FEET THICK. THE ROOF IN THE AREA OF THE FALL WAS LAMINATED SHALE. THE AREA OF THE FALL WAS SUPPORTED WITH 5 FEET RESI N GROUTED RODS ON THE ADVANCE.
HE AND CO-WORKERS WERE HANGING MINER CABLE AND WATERLINE FROM THE ROOF OF THE WORKING SECTION. THE CABLE AND WATERLINE WERE SECURED IN THE FIRST TWO CABLE HANGERS WITHOUT INCIDENT. HOWEVER, AS THE WATERLINE AND CABLE WERE BEING SECURED IN THE THIRD CABLE HANGER, THEY SUDDENLY SLIPPED FROM THE HANGER, CAUSING HIM TO BE FORCED TO THE MINE FLOOR.
ROOF FALL OCCURRED IN NUMBER 8 ENTRY AT SPAD 6120 IN NUMBER 1 MAINS. FALL MEASURED APPROX 50' LONG, 20' WIDE & 6' THICK. ROOF IN AREA OF FALL WAS LAMINATED SHALE. AREA OF FALL WAS SUPPORTED WI TH 42' RESIN GROUTED RODS ON ADVANCE.
ROOF FALL OCCURRED IN NUMBER 1 ENTRY 50' INBY SPAD #8575. FALL MEASURED APPROX 50' LONG, 20' WIDE & 6' THICK. ROOF IN AREA OF FALL WAS LAMINATED SHALE. AREA OF FALL WAS SUPPORTED WITH 5' RESIN GROUTED RODS ON ADVANCE.
EMPL WAS PASSENGER IN CARGO SPACE OF TELETRAM TRAVELING FROM WORKING SECTION TO SURFACE.JOLTED WHEN TELETRAM MADE RIGHT TURN AT INTERSECTION.EMPL DESCRIBED INCIDENT AS WHIPLASH.

VICTIM WAS CLEANING AROUND A BELT DRIVE AND TAKE-UP. HE BEGAN SHOVELING MUCK ONTO THE BELT CONVEYOR WHILE STANDING UPRIGHT, FACING HTE BELTLINE. AS HE SHOVELED A QUANTITY OF MUCK ONTO THE BE LTLINE, THE SHOVEL CONTACTED THE BELT ADN RESULTED IN HIS BODY BEING TWISTED TO THE RIGHT.
BACK INJURY.
VICTIM WAS OPERATING A DIESEL SCOOP WHEN THE ACCIDENT OCCURRED. HE WAS TRAVELING FROM THE 005 SECTION TO THE SURFACE TO TRANSPORT A LOAD OF BLOCK BACK UNDERGROUND. AS HE APPROACHED THE FIRST L EFT BELT DRIVE, HIS VEHICLE STRUCK A DEPRESSION IN THE ROADWAY, CAUSING HIM TO BE JOLTED IN HIS SEAT.
OPERATING CONTINUOUS MINER.HE AND 3 CO-WORKERS BEGAN REPLACING BITS ON RIPPER HEAD.A 4" CABLE HANGER WAS BEING USED TO REMOVE THE BITS.SLIPPED HOOK OF HANGER UNDERNEATH RING AND PULLED RETAINI NG RING OFF BIT.WHEN RING CAME OFF,SPRINGING ACTION OF RING PROPELLED IT TOWARD I.NAME STRIKING HIM IN RIGHT EYE.
VICTIM WAS DRIVING A 89 FORD PICK-UP TRUCK WHEN THE ACCIDENT OCCURRED. WHILE TRAVELING ON A MIAN ACCESS ROAD ON COMPANY PROPERTY, HIS VEHICLE STRUCK A DEPRESSION IN THE ROADWAY. HE WAS THROW N UPWARD AND HIS HEAD STRUCK TH ROOF OF THE VEHICLE. AS HE CAME BACK DOWN ON THE SEAT, HE FELT A SUDDEN PAIN IN THE LOWER PART OF HIS BACK.
VICTIM WAS LIFTING A CASE OF RESIN CARTRIDGES FROM THE BACK OF A BOLTER WHEN HE EXPERIENCED A SUDDEN PAIN IN HIS LEFT SHOULDER AND DOWN HIS SPINE. THE CASE OF RESIN CARTRIDGES WEIGHED APPROXIM ATELY 50 POUNDS. THEIR DIMENSIONS WERE 5" X 8" X34". GOBLE STARTED MISSING WORK AS A RESULT OF THIS ACCIDENT ON MARCH 20, 1989.
THE VICTIM WAS AWARDED RETRAINING INCENTIVE BENEFITS AS A RESULT OF BEING EXPOSED TO THE HAZARDS OF THE DISEASE COAL MINERS PNEUMOCONIOSIS.THE RETRAINING INCENTOVE BENEFITS WILL BE RECOVERED F ROM THE OPERATOR MARTIN COUNTY COAL CORP.
VICTIM WAS OPERATING A CONTINUOUS MINING MACHINE WHEN THE ACCIDENT OCCURRED.HE HAD POSITIONED HIMSELF ON A PADDED METAL STOOL WHILE OPERATING THE CONTINUOUS MINER BY REMOTE CONTROL.A PIECE OF SAHLE SUDDENLY FELL FROM THE RIGHT RIB AND THE RIGHT RIB AND THE RIGHT ROW OF ROOF BOLTS,STRIKING HIM ON TOP OF THE HARD HAT,SLIDING FORWARD ONTO HIS LEFT FEMUR.
VICTIM AND A COWORKER WERE ATTEMPTING TO ADJUST THE HEIGHT OF A CANOPY ON A SHUTTLE CAR THIS TASK WAS BEING PERFORMED BY USING A HYDRAULIC JACK WHILE REMOVING THE JACK FROM BENEATH THE CANOPY HIS FINGER WAS CRUSHED BETWEEN THE CANOPY AND JACK THIS INJURY WAS CAUSED BY THE CANOPY SUDDENLY DROPPING APPROX 3"

VICTIM WAS MOVING A TRAILING CABLE AND WATERLINE FOR A CONTIOUS MINER. HE MOVED THE CABLE AND WATERLINE FROM THE CENTER OF THE ROADWAY AND PLACED THEM NEAR THE RIGHT RIB. THIS TASK WAS PREFO RMED TO CLEAR THE ROADWAY FOR HAULAGE EQUIPMENT. THE TRAILING CABLE THAT HE LIFTED SHIELDED, 3 CONDUTOR TYPE.

A LARGE PIECE OF SHALE FELL BETWEEN ENTRIES 7 AND 8 NEAR SPAD 5429 IN THE AREA WHERE THE PILL AR HAD JUST BEE3 REMOVED. THE FALL WAS APPROX. 20' WIDE AND 1' TO 15' THICK (UNABLE TO DETERMINE THE LENGTH). THE CONTINUOUS MINER WAS BEING OPERATED BY REMOTE CONTROL. NO INJURIES WERE INVOL

VICTIM CHANGED 3NTO HIS WORK CLOTHES AND WENT TO LAMP HOSE TO GET CAP LAMP.HE RETURNED TO THE BATH HOUSE TO GET SOME KEYS.WHEN HE STEPPED FROM BOTTOM STEP OF BATH HOUSE TO THE GROUND HE LOST H IS BALANCE AND 0ELL TO THE GROUND IN THE GRAVEL PARKING AREA.HE REPORTED A SHARP PAIN IN THE AREA OF HIS COCCYX.
VICTIM AND 2 OT2ER CO-WORKERS WERE PULLING UP SLACK ON THE TRAILING CABLE FOR A CONTINUOUS MINER.AS THE VICTIM PULLED ON THE CABLE AROUND A CORNER HE EXPERIENCED A SUDDEN PAIN IN HIS LOWER BAC K. THE TRAILING3CABLE WAS A SHIELDED 1/0,3 CONDUCTOR TYPE. THE TRAILING CABLE WAS FOR A JOY 14CM CONTINUOUS MINER. HEIGHT AT ACCIDENT SITE WAS APPROXIMATELY 70".FLOOR WAS DRY.

VICTIM AND 9 OT2ERS WERE RIDING IN THE CARGO SPACE OF A KIESEL TELETRAM THAT WAS BEING USED AS A MANTRIP THE TELETRAM WAS TRANSPORTING THE CREW TO THE SECTION FROM THE SURFACE THE WHEELS OF TH TELETRAM STRUCK3A DEPRESSION 7 IN IN ROADWAY RESULTING IN THE VICTIM BEING JOLTED HE EXPERIENCED A SUDDEN PAIN IN LOWER BACK

VICTIM STATED T2AT HE EXPERENCIED A SUDDEN PAIN IN HIS NECK WHILE TRAMMING A ROOF BOLTING MACHINE ACROSS THE LAST OPEN CROSSCUT OF THE SECTION. HE STATED THAT PAIN OCCURRED WHEN HE APPLIED PR ESSURE TO THE T9M CONTROL LEVERS.
