Rocks lodged in loader belt. Employee was instructed to stay with loader while proper safety procedure was put into motion by other employees. Employee willfully climbed on loader bucket and attempted to remove rock, subsequently falling and striking face on belt cover. He broke a tooth and required stitches to his lip.

Employee was shoveling sand away from belt area of screen and injured lower back. Not sure how.
Employee was removing rock from sand screen. He lost his footing, slipped and fell on rocks next to him. Sustained bruising and soft tissue contusion. Awareness of surroundings, conscious of footing and balance would help to not repeat accident.

While moving loader on site, employee had bucket too low to ground. He hit bump in surface of ground and bucket dug into ground stopping loader abruptly. Employee was thrown into windshield, lacerating forehead. Employee received stitches. Loader required repair to joystick, not parts, mechanic time only.

EE was walking up conveyor belt to clear any large boulders that might be stuck on shaker chute when he tripped and lost his balance from his own two feet. He hit the shaker on his left side of his chest. He got up and told his supervisor.

Operator was cleaning spoils up around jaw. Right hand door on backhoe hit a protruding iron beam on upper platform on jaw, breaking the door glass and damaging the door frame, there were no injuries. He will receive a review of task training.

Was dislodging a rock on the grizzly rock crusher, rock came loose and hit EE on the lower right leg.
Employee was using prybar to dislodge rock and when rock moved he smashed his finger between prybar and metal chute.
