EE WAS MOVING CRIB BLOCKS OFF TOP OF FEEDER WHILE MAKING A BELT MOVE. A PIECE OF DRAW ROCK FELL (APPROX 1" X 1" X 1/2" THICK). IT STRUCK HIS FINGERS AGAINST A CRIB BLOC, FRACTURED INDEX AND MI DDLE FINGER OF LEFT HAND.

WHILE TRAMMING A 3 WHEELER, THE STEERING WHEEL BECAME OUT OF CONTROL AND THE STEERING WHEEL KNOB HIT HIM ACROSS THE TOP OF HIS RIGHT HAND WHICH CAUSED THE BREAK.

EE WAS PULLING ON MINER CABLE, WHEN HE HAD A PAIN IN HIS LOWER BACK. HE WORKED ALL SHIFT AND COMPLAINED OF PAIN IN LOWER BACK AND LEGS.
HE WAS SUPERVISING THE SETTING OF A TAILPIECE, WHEN THE TAILPIECE STARTED TOWARD HIM HE HAD TO QUICKLY MOVE OUT OF THE WAY AND TWISTED HIS KNEE.
HE FELT A SHARP PAIN IN HIS BACK WHILE HANGING THE CONTINUOUS MINER CABLE.

EE WAS MAKING A PRESHIFT EXAM FOR A MAINTENANCE SHIFT WHEN HE FOUND A ROOF FALL IN THE #1 ENTRY ABOUT 80' INBY SPAD #6778. THE FALL WAS APPROX. 18' X 40' X 15' THICK. NO INJURIES RESULTING FRO M FALL. VENTILATION WAS NOT AFFECTED.
FOREMAN WAS TRAVELING HIS INTAKE AIR COURSE WHEN HE DISCOVERED ROOF FALL IN THE #6 ENTRY 1 BREAK OUTBY SPAD AT 4808. THE FALL WAS APPROX 20X25X6FT. THE AREA WAS DRAPED AND CRIBS WERE SET. STO PED BRATICE LINE OVER. THERE WERE NO INJURIES.
ON 8-31-98 EE WAS PUTTING GLUE ON ROOF BOLTER WHEN HE TWISTED HIS BACK WHILE TURNING WITH GLUE. HE CONTINUED TO WORK AND WENT TO DR THAT NIGHT AND RETURNED TO WORK THE NEXT DAY. ON 10-1-98, EE TURNED IN A LIGHT DUTY SLIP TO FOREMAN AND REPORTED THE ACCIDENT AS A LOST TIME.
ROOF FALL OUTBY SEVCTION IN THE #6 ENTRY. FALL APPROX. 18' X 18' X 5' THICK. THE FALL WAS IN THE INTAKE AIR PASSAGE. FALL WAS CRIBBED OFF 7 DANGERED OFF. ROOF FALL WAS SLICER SIDE HORSE'S BACK . NO INJURIES. TEST HOLES WERE CHECKED AORUND AREAR. AREA OK.
EMPLOYEE WAS INSTALLING CHAIN HANGERS FOR A BELT MOVE. HE WAS INSTALLING THE LAST BOLT INTO THE HOLE. WHEN HE PUSHED THE ROOF BOLT INTO THE HOLE HE CAUGHT HIS LEFT FOREFINGER BETWEEN THE ROO F BOLT AND THE HANGER PLATE.
EE WAS ROOF BOLTING IN THE #4 ENTRY. EE WAS INSTALLING THE 2ND BOLT IN THE 2ND ROW WHEN A PIECE OF DRAW ROCK MEASURING 16" X 12" X 3" FELL FROM THE TOP STRIKING HIS LOWER RIGHT LEG. THE FOREMA N GAVE EE FIRST AID AND TRANSPORTED HIM TO THE SURFACE. EE WAS TAKEN TO WHITESBURG HOSP. FOR TREATMENT.

A APPROX. 10:20 PM, THE ROOF IN THE #4 INTERSECTION, 50' INBY SPAD #5517 BEGAN WORKING. THE AREA WAS DANGERED OFF AND THE INTERSECTION FELL APPROX 10:55 PM. THE FALL WAS 25" X 18' X 4 1/2' -6' . THE AREA WAS DANGERED OFF AND THERE WERE NO INJURIES. VENTILATION WAS NOT AFFECTED.
THE SECTION REPAIRMAN WAS PUTTING A LID BACK ON THE PANEL CONTROL BOX ON THE #SC31 SHUTTLE CAR WHEN HE HAD PAIN IN HIS LOWER BACK.
AT APPROX 1745 #2 BELT STOPPED, AFTER INVESTIGATION THE PROBLEM WAS DISCOVERED THAT A ROOF FALL HAD OCCURRED 30' NBY SPAD #2486 ON THE TRACK AND BELT ENTRY. THE FALL WAS APPROX 40'X20'X36"TO4 8"T. THE AREA WAS DANGERED OFF AND CRIBS SET. THERE WERE NO INJURIES AND VENTILATION WAS NOT AFFECTED.
AT APPROX 11:40 AM ON 7-25-96 THE SECTION FOREMAN HEARD A ROCK FALL SOMEWHERE OUTBY THE SECTION.AFTER INVESTIGATING THE NOISE, SECTION FOREMAN FOUND A FALL AT SPAD #5053. FALL MEASURED APPROX 18'X 25'X 5'. THE AREA WAS DANGERED OFF AND CRIBBED OFF AROUND THE FALL. VENTILATION WAS NOT AFFECTED AND NO INJURIES.
THE 3RD SHIFT CREW WAS CLEANING UP AND ROCK DUSTING AREA WHEN THE FALL WAS DISCOVERED. ONE BREAK INBY SPAD #5049 IN THE #6 ENTRY. THE FALL WAS TIMBERED UP AND DANGERED OFF. THE FALL DID NOT AF FECT VENTILATION AND THERE WERE NO INJURIES. FALL MEASURED APPROX 18'X 18'X 4'.

OUTSIDE MAN EE WAS LOADING BLOCKS FROMA STACK OF BLOCKS ONTO A PUSH OUT CAR WHEN PULLING A SINGLE BLOCK FROM THE TOP OF THE STACK A BLOCK DIRECTLY UNDERNEATH IT CAME OFF STRIKING HIM ON THE LE FT FOOT BREAKING THE BONE BEHIND THE LITTLE TOE

THE FALL WAS 1 1/2 BREAKS INBY SPAD 4023 TO THE RIGHT OF MAIN LINE IN A SET OF 40'S. THE SIZE OF FALL WAS APPROX. 18 X 20' X 4'. THE AREA WAS CLOSE TO OUTCROP. FALL WAS TIMBERED OFF AND AREA DANGERED. VENTILATION WAS NOT AFFECTED BY FALL AND NO INJURIES.
AT APPROX 6:05 AM ON 10-19-94 EE FOUND ROOF FALL IN THE RETURN AIR COURSE APPROX 1400 FT OUTBY WORKING SECTION. THE FALL WAS 1 BREAKS FROM SPAD #3732 TO LEFT IN 50'S. THE APPROX SIZE OF FALL WAS VENTILATION WAS NOT AFFECTED BY FALL.
ON 9-12-94 EE STATED THAT HE HURT HIS LIFT KNEE ON THE 21SC WHILE TURNING AROUND ON THE CAR HE BUMPED HIS KNEE ON THE DECK. HE WENT TO THE DODGE THAT EVENING AFTER WORK AND RETURNED TO WORK TH E NEXT DAY. EE WENT TO DR FOR HIS KNEE AGAIN ON OCT 6,12,20TH NOV 8 HE HAD OPERATION TO HIS KNEE BUT DUE TO OLD INJURY ON HIS KNEE IT WAS NOT DETERMINED TO BE A JOB RELATED INJURY TIL A LATER
EE WAS IN PROCESS OF INSTALLING WHEELS TO JBLONG RUBBER RAIL ROCKDUSTER THE ROCKDUSTER WAS INSIE SHOP BUILDING & THE TIRES WERE OUTSIDE THE SHOP DOOR EE PICK UP A TIRE & STARTED INSIDE THE SHO P WHEN HE STEPPED UP INTO THE DOOR HE FELT PAIN IN LOWER BACK
AT APPROX 3 AM EMPLOYEE WAS MAKING THE RETURN AREA OF THEMINES WHEN HE FOUND ROOF FALLS IN TWO LOCATIONS. #1-2 BREAKS INBY SPAD #2531 & #2 1 BREAK INBY SPAD #2536. BOTH FALL'S WERE APPROX 16 - 38". THICK BY 14'X20". THE AREA WAS TIMBERED OFF AND DANGERED. NO INJURIES AND VENTILATION WAS NOT AFFECTED.

THE INJURED EMPLOYEE WAS MAKING A POWER MOVE ON THE 001 SECTION.HE WAS GETTING THE SLACK CABLE OUT OF THE HAUL ROAD WHEN HE TRIED TO PULL THE 21030-CC CUT MACHINE CABLE.HE FELT A SHARP PAIN IN HIS BACK.THE FOREMAN WAS THEN NOTIFIED AND THE INJURED EMPLOYEE WAS TAKEN TO THE SURFACE.ON THE SURFACE HE REFUSED AN AMBULANCE AND SAID HE COULD GO HOME.
ATTEMPTED TO STEP ON A BELT DRIVE AND FELL APPROX. 5' LANDING ON HIS BACK. HE REPORTED THAT AFTER SEEKING MEDICAL TREATMENT AT OUT LADY OF THE WAY HOSPITAL MARTIN, KY. HE HAD A TWISTED MUSCLE IN HIS BACK AND A PULLED LIGAMENT. NO IMPAIRMENT RATING AT THIS TIME.
