Fall was found on new haven mains, 3 breaks inby spad #1602 Fall was 18'W x 20' Long and 4' Thick. Roof support was 4' glue bolts.

Fall happened in intersection & fell from 6 to edge of #5 fall about 50' long 20' wide 4' thick.

Roof fall found by fire boss it fell at #10 bk on slope side 4' thick from middle of bk. & feathered out to left rail about 10' inside fall about 20' long total

A roof fall occurred at Block#137 Track entry/#6 entry in a intersection plus approx. 15' each way past the rib line of cross-cut. Roof was slate-fell approx. 8' H. X 18" W. approx. 45' to 50' in L. Area had been bolted with 6' torque tension bolts. Area was developed approx. 9/1/2008.

Roof fall was found on a normal weekly exam of the Box Cut Intake airways. Block 12 @ spad #3677 "old #1 section track entry". Fall is in an intersection 18' wide, 25' long, 4 1/2' thick. Bolted with 4' resin bolts - Bolts pulled out approx. 6" sandstone main roof - Slate top fell.

Fall was found at 47 break track entry spad # 851 #5 entry. Roof fell at 47 break to 48 break 60'L, 6'H, 18'W. Roof support was 6'TT bolts.

Fall was found on #2 belt spad #1082 #4 entry, 1 row inby corner, tight on rib line. Fall was 6' to 6.5' H roof control was 6'TT bolts.

EE was traveling with MSHA inspector, walking Alloy's return airways to MP's #22 and 23 when they encountered a roof fall at spad # 1753, an intersection in a adjacent entry to the entry the airway examiner normally travels. These air ways are all 'common' in a 9 entry panel. We dangered the area off.

EE was traveling with MSHA Inspector, walking the alley main return towards the return air shaft - when they encountered a roof fall @ block #5 - spud #258 - a intersection in a adjacent entry to the entry the air way examiner normally travels - These air ways are all common in a 5 entry panel - we dangered area off.

#1 section was idle on the midnite shift-The section was being pre-shifted/ firebossed prior to day shift entering the mine, when the fireboss found the fall. It was located at the last open break # 4 entry intersection spad # 4339 approximatly 4'1/2 high.

Employee has over 30 years of underground mining experience, most of which has been at the face. His last two years of employment have been as a Continuous Miner Operator at Big River Mining

Offside bolter operator was putting up the inside bolt of an entry in the face on West Mains when a piece of drawrock fell from inby. The drawrock fell onto the T-bar over his canopy onto a rib and on his lower legs. The rock measured 4'x5'x4" and took several workers to remove from the operators legs. He was taken to the ER and has been hospitatlized due

While tramming a bolter from one entry to another from the deck. EE held his hand up outside the canopy to remove the flypad obstructing his view and trammed the roof bolter with his left hand exposed and crushed his hand between the canopy of the deck into the rib.

While the individual was rolling up piece of curtain to scoop the entry, he stepped on a piece of coal and twisted his ankle.

The intersection of the intake course at brk 19 spad 318 fell in approximately 20' x 20' x 4' due to a coal rider seam in the sandstone.

While starting to drill in the roof a piece of rock fell and struck the edge of the canopy which EE was standing under, the rock flipped back into the area where he was standing and struck him on the back.

While standing beside the roof bolter in entry no. 4 on section no. 4, employee claims a piece of rock fell and struck him on the back.

Fall was discovered in the roadway on allor mains at break #61. Fall is approx 25'long, 18'wide and 3 1/2 to 4' high.

While walking along the operators side of the roof bolter, EE claims to have stepped on a small rock laying along the rib, as he stepped on the rock EE lost his footing and fell to the mine floor.

While removing bolts on a cable take-up reel cover, a piece of draw rock fell and struck ee.

While walking along the number 1 belt line, at break number 19, EE stepped into some mud and claims to have twisted his right knee. He continued to work until 4/11/08 when he stopped working due to knee pain.

A fall measuring approximately 18' W x 40' Long x 6' High occurred on section no. 4, entry 2.

A roof fall measuring 20'long x 18'wide and 4' in height. Occurred on the new haven section at or near spad #1795.

Inby air-lock door exiting mine was not operating properly. Crew tried four times raising and lowering door, after failing to get door to raise higher than 3', EE tried to manually assist door. EE was struck by garage door when man trip began tramming forward with no warning. EE had to be treated for a laceration on his chin, approx 3"L X 1/4"deep.

EE exited a small 4 wheel mantrip to check and date up a pump. Upon exiting the mantrip he fell backwards and struck his head on a slate bar.

EE was walking under a piece of high line when a rock near the location where he was walking fell and struck the high line and knocked the high line loose. The line fell and struck ee causing him to fall. He tried to catch himself with right hand during the fall.

EE was walking in number 4 entry of number 4 section, when he stepped up onto a cable cross-over. While standing on the cross-over he lost his balance and slipped, causing him to fall.

EE started to drill a hole in the roof to install a roof bolt. In doing so a small piece of rock fell and struck him on the back of his hand.

Employee was tramming scoop down entry. Fly board caught him in canopy of scoop and pinned his back against the scoop canopy, scraping his shoulder blades & pulling back muscles.

EE was bent over looking for an oil leak on the miner, when a small piece of draw rock fell, striking EE on the back of the head and neck.

While making repairs to the miner, He lifted a rub rail to replace it and in doing so he strained his back.

While backing the mine out of number 6 entry, ee whom was facing in the opposite direction from which he was walking, stepped on a rock and twisted his right knee. He was removed from work on 11-21-2007 by physician orders.

While walking in Number 3 entry, employee claims to have slipped and fallen, twisting his right knee. He was removed from work by Doctor on 11-20-07.

While rearranging roof bolt plates on the bolter, hishand slipped and he struck the edge of an adjacent stack of bolt plates, causing a laceration on his left ring finger.

While loading belt into a utility car, EE claims that he raised the forks of the skid steer to adjust the belt dog and while doing so he fell from the machine and the machine lurched approx. 3' forward and ran over his foot and ankle.

EE had been riding on the step area of the Getman Tractor, from #4 belthead to the bottom of the slope. The tractor was stopped for EE to exit the tractor. After his exit, EE fell and placed , by catching himself with his right hand in a pinch point area. The tractor turned and in doing so, pinched EE's fingers.

While riding in the mantrip, a piece of draw rock fell and struck the top edge of the canopy and flipped into the area where the employees were riding. In doing so the rock struck the victim on the foot.

While inserting a glue stick into a drill hole, employee slipped and fell striking his back on the drill pot. He was removed from work by a specialist on 10-15-07.

While loading a piece of 60" betl structure into a scoop, EE felt pain in his lower back.

Employee was walking under a belt cross under when he stepped on a small piece of belt that had been discarded and was covered with mud. EE slipped with his right foot causing him to fall back and sit on his left foot and ankle.

EE was retreaving a chain from a parked scoop bucket. After he stepped out of the bucket, the scoop operator thought he was clear. The operator started the pump motor and started to back up the scoop. The scoop slid sideways and struck EE, pinning his right leg against the rib with the scoop bucket.

EE was assisting a certified electrician repair a de-energized cable. While holding the cable, a piece of draw rock fell and struck EE on the back of the right hand.

While loading belt structure into a parked scoop bucket, employee stepped on a top roller. The roller rotated under the weight of his foot, causing a twisted left ankle.

EE was utilizing the top of a battery box on a DBT scoop as a stable platform to support his notebook while he was writing notes. The roof bolt operator was unaware of EE's location when he started tramming the roof bolter forward. The roof bolting machine was trammed approximately 10 feet forward when the bolter struck EE from behind.

On a general inspection of the emergency escape capsule, the capsule failed to operate properly. The mine was evacuated until repairs were made and a test was conducted. Mine reopened 4:00 pm.

While unloading a mandoor from the supply, employee lost his grip. The door slid through his hands causing a laceration through his leather gloves on his right index finger.

Employee was descending a set of 3 steps at the bath house to retrieve a set of keys from his vehicle. He slipped on the bottom step and fell.

While making a belt splice, the knife that was being utilized slipped and cut the palm of the right hand.

Employee was walking through a cross-cut to reset a water pumping new location, when a section of draw rock fell from between permanent roof support and struck employee, knocking him to the ground.

While moving the miner into the face of no. 2 entry, employee was standing in no. 3 left entry. He started to walk across the entry and stepped incorrectly. This caused him to twist his knee. He was treated and referred to an orthopedic specialist for review.

After repairs to miner had been completed, EE was assisting in the replacement of the rubrail. During the procedure, the rubrail slipped & smashed his right middle finger above the last knuckle against the frame of the machine.

Employee was helping the scoopman rock dust & a piece of rock fell & hit him on the head.

While repairing to start a new drill hole a small piece of rock fell & struck the drill pot & bounced back & struck EE on the shoulder. EE visited his Dr. 5/18/07 and was instructed to stay off work until 5/23/07 for rest.

EE was riding on the off side of mantrip with his left foot protruding from the mantrip. EE stated he struck his toe against the rib and that caused him to twist his knee. EE went to Dr. 5/17/07 and Dr. informed EE that he had a twisted knee & needed to take 5 days off work the let knee heal.

During a weekly inspection 4 falls were detected. The location of the falls are at or near spad 270, 175, 212 & 281. No injuries, no airways were blocked & escapeway are clear to travel. The falls were detected on sliding hill main area.

On the sliding hill mains section, an unplanned fall above anchorage occured on the above stated date and time. The fall is approximately 60' long, 18' wide and 6' high. The fall is located in a cross cut between #5 and #6 entry. Both sides of the cross cut area have been cribbed off.

EE was starting to make repairs in a section of cable when his utility knife slipped from the cable jacket material of the cable. The knife blade came in contact with his left thumb causing a small laceration between his thumb nail and first knuckle.

The Safety Department informed me, on 3/21/07, that EE was walking in the #4 entry on Sliding Hill Mains, when he stepped into a section of uneven groung and twisted his left knee.

While making the "C" cut in the cut sequence, a rock located in front of the last row of bolts between the bolt and right rib, fell from the front to back in a swinging motion and came back into the area where the operator was standing and struck him on the shoulder.

While installing a personal identification tag on a miners utility belt, the instrument being used to puncture the belt material penetrated the belt material at a much quicker rate than anticipated. The cutting device being utilized came in contact with the left middle finger causing a small laceration.

While erecting 8'0" elevation floor steel on R.O.M. Belt Drive Building an unsupported, at one end, 10" channel struck employee on the head. He instantly fell to the concrete floor. Rescue squad showed up 20 minutes later and took over.

Employee was cutting q decking. Metal flew up under safety glasses in to right eye.

An unintentional roof fall occurred in the slope entry being developed at the mine. The roof fall measured approximately 22' wide X 0 to 10' thick X 70' long. There was no injury to any employees, however, the fall occurred on the continuous miner parked in the slope entry.

Employee was unloading a truck (Pipes) and the load was not properly blocked. Employee stepped on pipe causing it to tip up. loosing balance.
