Using grinder in mill room. After lunch returned to area & replaced conveyors. 2PM discomfort in eye & washed it w/eye wash. At 2:30PM clocked out as norm. Next morning clocked in norm for shift. Told us eye was irritated. Sent EE for medical assessment. Dr. removed small metal splinter from right eye. Splinter was residual that got into eye from wiping sweat from face.

EE notified CED of a pulmonary fibrosis and emphysema diagnosis on 8/21/2024. EE also notified CED on the same date that EE has also been diagnosed to be a carrier for m/z phenotype Alpha-1-antitrysin deficiency which is a genetic mutation that affects the lungs.

EE was on foot. Pushed a stuck man lift they were following. EE initially strained left calf muscle but DID NOT seek medical attention. 8/17/18 EE sought medical attention due to pain shift/discoloration. Medical personnel diagnosed as left gastrocnemius strain and put EE on restricted duty. Follow up visit 8/21/2018 closed the case. Medical released the EE to Full/Regular duty.

EE had just completed swapping thrust roller on kiln. The EE removed employee's safety glasses to wipe employee's eyes. Employee believes a piece of debris came off EE's glove when EE rubbed EE's eyes. EE worked rest of day. Returning to work the following day, EE was sent for medical assistance due to redness and discomfort. Medical personnel swabbed EE's eye to removed object.

Employee had just finished cutting a piece of pipe with a hacksaw and rested arm on the work bench when something punctured left forearm. Upon self examination, the employee identified and removed a 1/8" metal splinter with fingertips. Employee sought medical assistance 3 days later due to redness and swelling. Medical personnel administered a shot of Doxycycline.

EE notified CED of silicosis diagnosis on 8/19/2015. He was on short term disability from January 2015 to July 2015 due to a diagnosis of lymphadenopathy mycobacterium avium, which was determined to be non-work related by his Dr. No definitive diagnosis of silicosis attributable to employment. Company reserves the right to withdraw this filing.

Employee was performing maintenance on a conveyor belt when sand particle got into employee's right eye. Sand particle caused irritation that subsequently turned into conjunctivitis.

EE was turned and looking away when several blocks fell from the side of mill shell onto employee's lower, back, right leg. Employee's exertion of forward, downward pressure on right foot in reaction to block hitting back of lower leg caused a closed fracture to one bone on top of right foot.
