A passenger vehicle with a 54 old driver was traveling north on Williamsville Road and at the point of the haul road crossing the vehicle struck a loaded haul truck at the left front steer tire slightly deflecting toward the left side of the road: resulting in damage to the vehicle and minor injuries to the driver.

Employee, wearing appropriate PPE, received unidentified foreign debris in the right eye. Following treatment at a local medical center, a prescription medication was given to prevent infection.

Employee was washing boom off, lost track of where EE was standing and slipped and fell striking the right side of EE's head. Employee appeared to bleed from the nose and the onsite safety supervisor took precautionary measures and called ambulance.

Employee was using a step ladder to tighten the top lug nuts on a 3600RSI. As he was moving the ladder his left foot went thru the ladder, missed the bottom rung & he fell to the ground on his right side, catching his body weight on his right wrist.
Employee was dismounting from a Haul Truck (CAT 785), employee missed grabbing the handrail, lost balance and foot slipped from the access ladder and fell approximately 10 feet to the ground.

EE was working normally when he bent down to pick up a tool and felt pain in his lower back. Diagnosed as possible lumbago.
Employee was assisting in rolling rack segments into place using pipe. Employee was moving the rear pipe to the front when his right ring finger tip was caught under the pipe smashing the right ring finger. Employee required 7 stitches and has a fracture of the finger above knuckle.

LACERATED AND FRACTURED
EE WAS PERFORMING OVERHEAD WORK WHEN A BOLT FELL FROM OVERHEAD STRIKING HIM IN THE MOUTH CAUSING A LACERATION TO THE LIP AND A CHIPPED TOOTH.

EE WAS ASSISTING THE DRAGLINE CREW IN TRYING TO SET THE UPPER RAIL ON HTE WALKING CAM. HE PLACED HIS HAND UNDER A NUT AND MOTIONED THE CRANE OPERATOR TO RAISE THE LOAD. WHEN THE LOAD WAS RAISE D, THE EE'S FINGER WAS PINCHED BETWEEN THE NUT AND THE LOWER RAIL.
EE TRIED TO PICK UP A D10 ROLLER AFTER HE FINISHED REPLACING IT WITH A NEW ONE. THE EE PICKED THE 320 LB. ROLLER UP AND WHEN THE HEAT FROM THE ROLLER PENETRATED HIS GLOVES HE LET GO OF ONE SID E LETTING THE OTHER ONE FALL ON HIS RIGHT HAND.THE ROLLER WAS NOT SMOOTH BECAUSE OF PREVIOUS REPAIRS. THE ROLLER CUT THROUGH THE GLOVES BREAKINGTHE PINKY FINGER. THIS RESULTED IN REMOVAL/PINK

THE EE WAS IN THE PROCESS OF REMOVING SHIMS FROM A LATCH BAR ON THE P&H 2100. EE COULDN'T FIND THE PROPER TOOL USED TO HOLD THE TRIP LEVER UP, SO HE USED A SHIM. DURING THE REMOVAL OF THE SHIM , THE SHIM USED TO HOLD THE TRIP LEVER UP SLIPPED OUT OF POSITION ALLOWING THE BAR TO SMASH HIS THUMB. EE RECEIVED A LACERATION OF THE KNUCKLE AND BROKE HIS THUMB IN THREE PLACES.

EE WAS ATTEMPTING TO LIFT A 30' I-BEAM TO TURN IT OVER WITH OPEN HOOKS. THE HOOKS WERE PLACED ON THE END OF THE BEAM AND THE OVERHEAD CRANE WAS BEING USED TO LIFT IT UP. THE EE RAISED THE BEAM AND ANOTHER EE STUCK A BLOCK UNDER THE BEAM SO IT COULD BE RE-RIGGED. THE BEAM WAS BEING LOWERED WHEN THE HOOKS CAME OFF AND STRUCK THE EE IN THE RIGHT SIDE SAFETY GLASS, CAUSING LACERATION.
EE WAS CHANING OUT A HOIST CYLINDER ON AN R100 TRUCK. THE CYLINDER WAS SUSPENDED WITH A CHAIN FALL. WHEN ATTEMPTING TO REMOVE THE BOTTOM OF THE CLYLINDER FROM THE STUB SHAFT WITH A PRY BAR, TH E EE REACHED AROUND THE CYLINDER WITH HIS LEFT HAND TO POSITION THE PRY BAR. IT SLIPPED OFF & PUNCTURED HIS LEFT HAND. WHEN EE JERKED HIS HAND BACK HE HIT THE BACK OF IT ON THE HYDRAULIC TANK

2" WATER PUMP HAD CLOGGED AND OVERHEATED. WHEN EMPLOYEE DISCONNECTED SUCTION HOSE HOT WATER AND MUD SPURTED OUT ONTO THE BACK OF HIS LEFT HAND CAUSING 2ND DEGREE BURN REQUIRING MEDICAL TREATME NT.
THE INJURED WAS HOOKING THE 834 RTD TO THE LOWBOY TRAILER DOLLY. THE 6" HITCH PIN WHICH WEIGHED ABOUT 25 LBS. FELL INTO PLACE WHILE ANDY STILL HAD HIS HANDS WRAPPED AROUND THE TOP PLATE. WHE N THE PIN FELL, IT PINCHED HIS LEFT INDEX FINGERBETWEEN THE TOP PLATE OF THE PIN AND THE HITCH OF THE DOLLY. IT MASHED THE TIP OF THE FINGER AND LACERATED THE FINGER REQUIRING SIX STITCHES.
INJURED WAS STANDING ON THE TRACK OF A D-6 DOZER WHILE STEAM CLEANING ENGINE COMPARTMENT HE SLIPPED & FELL TO THE CONCRETE FLOOR IN WASH PAD LANDING ON HIS RT SHOULDER & HIP

EMPL STEAM CLEANING AND RAN THE WAND OVER TOP OF FOOT. BOOT HAD A HOLE IN TOP CAUSING HOT WATER TO GO INTO FOOT. DISCHARGED SAME DAY TO RETURN TO WORK 9-18-92.

HE WAS ATTEMPTING T O DETACH CREANE HOOK FROM ITS TIE DOWN POSITION WHEN CRANE BROKE LOOSE FROM WELD AT TROLLEY & FELL CRUSHING BARREL & STRIKING INJURED.
EE AND OTHER PERSON WERE ADDING 3/8 X 12 X 7 METAL EXTENSION TO THE BACK TOP OF THE BUCKET ON A 950 FRONT END LOADER. THE TWO WERE STANDING IN THE BUCKET HOLDING THE METAL IN PLACE AND ATTEMPT ING TO WELD WHEN IT SLIPPED AND FELL CRUSHING EES FINGER BETWEEN THE EDGE OF THE METAL EXTENSION AND THE SIDE OF THE BUCKET.

WELDING IN REVOLVING FRAME - WELDING GUN TRIGGER WAS DEPRSSED AND CAUSED WIRE TO ARC, CAUSING HOT METAL TO FALL IN GLOVE, RESULTING IN A BURN ON RIGHT WRIST.
EMPLOYEE WAS USING STREAM CLEANER AND DEBRIS GOT INTO HIS EYE.
HAUL TRUCK STARTED SLIDING WHEN IT HIT A FRESHLY WATERED AREA ON THE HAUL ROAD, WHEN THE TIRES CAME IN CONTACT WITH A DRY SURFACE THEY CAUGHT TURNING THE TRUCK OVER ON IT'S SIDE - OPERATOR WEN T TO THE DOCTOR PUT HER ON LIGHT DUTY TILL 6-5 ON 6-13 SHE WAS PUT BACK IN A HAUL TRUCK.
