Employee was lifting a 4" thick power cable for a quarry saw with another employee over the saw in set-up operations. At the top height of the lift the cable was on EE's fingertips and the weight of the cable bent employees right hand back. Sent to clinic for evaluation, diagnosed with strained right wrist, given lifting restriction.

Employee asked a loader operator to lift 600 gal plastic tank. 25 degrees (F)outside. Employee asked a co-worker to assist in tilting the tank back to let forks slide under. The tank was tilted, lifted by loader and put in truck. Employee then stated EE thought EE strained back. Sent to clinic. Rx received/No lost work time.

Quarry Stone Marker enrolled in company Hearing Conservation Program showed a Standard Threshold Shift in hearing over the previous year relative to baseline audiogram, loss >10dB.

While beginning to mark a turned block, employee noticed a chunk of stone debris attached to the side of the block. Employee used left foot to stomp downward on excess piece. Excess piece was barely attached & fell easily but downward momentum & lack of resistance by the stone caused employee to lose balance, falling down onto block landing on right shoulder.

Employee exposure to high levels of sound resulting in Standard Threshold Shift of approximately 30 decibels at 2K frequency.

The miner bent over to lift up an object and hurt his back. Diagnosis is determined to be muscle spasms or thoracic spine, thoracic spine strain.

On 7/29, miner was operating an excavator with door closed, window open; he got a piece of mud/dirt/rock in his left eye. A witness noticed irritated eye and offered miner eyewash solution but miner declined. On 7/30, miner reported to work but later left to see eye doctor. Miner returned to work next day on 7/31.

The miner picked up a 100-pound piece of stone and dropped it on his right pinky.

Miner was using hand drill when a piece of metal shaving became lodged in his eye. Foreman states that he turned accident report into Quarry Director at that time. No record of original report can be found, however. This came to my attention yesterday, and so I am now filing. Miner was wearing proper PPE.

The scabble saw operator was on top of the saw pad, lining up a block with the bar. Operator stepped back and his leg became caught between the track and the pad, which caused him to lose balance and fall to the ground. The operator sustained stretched ligaments in the leg and/or knee areas.

Employee slammed left two fingers on left hand in the door of crane. Upon noticing hand was bothered, employee's supervisor sought a cold compress to be applied to the affected area. Employee refused further treatment at that time and went back to work. Next day (10/20/2012), swelling was worse and employee sought attention from ER. Diagnosis was fractured pinky finger.

The employee was crossing a steel saw track when he slipped and twisted his knee. At that time, the employee complained of pain but did not see the need to go to the doctor. Ultimately, the pain did not subside and the employee went to the doctor. After a couple visits an MRI was done which showed a torn ligament in his right knee.

The employee reported that he had been experiencing pain in his left shoulder, arm and chest for two prior days but had not reported it to anyone. On this day he reported to his supervisor that he had the pain and it was too severe to work. The employee's position requires repeated swinging of an 8 pound sledge hammer, which apparently caused the injury.

The employee bent down and picked up a bucket with metal wedges. Something "poped" in his lower back.

Rock struck loader bucket and deflected into cab, broke front windshield and pinned the operator

Drill operator noticed hydraulic fluid leak and went to inspect leak. At same time the operator found the leak, the hydraulic hose ruptured spraying hot hydraulic fluid into the face and eyes of the operator. The force of the spray knocked the hard hat from his head and safety glasses from his face resulting in abrasions to his corneas and conjuctivitis in both eyes.

EE WAS STANDING ON A TURNED CUT WHEN A LOADER OPERATOR WHO WAS TRYING TO BACKFILL THE TURNED CUT, ACCIDENTALLY STRUCK THE CUT CAUSING EE TO FALL STRIKING HIS BACK ON THE EDGE OF THE TURNED CUT.

Crane operater was outside his crane attempting to secure a chain hook to saw track when he felt pain in his lower back. EE stated he was pulling and twisting at the same time. Initial doctor examination indicated a strained lumbar. EE out 10-14 days.

EE was using a cold steel wedge to plit/break a quarry block. When EE struck the wedge, a piece of metal from the wedge struck the EE in the forearm causing a muscle laceration and lodging in the top of the EE's forearm. This required minor surgery to be required to remove the metal and repair the damage to the muscle.

Worker was using a "mushroomed" wedge to split a quarry block. When the worker struck the wedge a piece of metal from the wedge struck the worker on the upper forearm just above the inner elbow area causing a laceration which required 3 stitches to close.

THE TOP OF THE WEDGE BEING USED TO BREAK THE QUARRY BLOCK WAS MUSHROOMING AND THE EE INJURED FAILED TO NOTICE, WHICH RESULTED IN A METAL FRAGMENT SPLINTERING FROM THE WEDGE AND PENETRATING THE WORKER'S BOOT AND LODGING IN HIS LEFT FOOT JUST BELOW THE ANKLE. EE WAS TRANSPORTED TO A PROMPT CARE FACILITY WHERE HE RECEIVED A TETANUS SHOT AND SIX STITCHES.

CRANE OPERATOR AND ANOTHER WORKER WERE LIFTING A 6 INCH WATER PUMP FROM A PIT, WHEN THE CABLE HOISTING THE PUMP BROKE CAUSING THE PUMP TO FALLAND STRIKE THE WORKER WITH A GLANCING BLOW TO THE LOWER RIGHT BACK AREA. WORKER SUSTAINED SERIOUS BRUISING AND SWELLING OF THE AFFECTED AREA. WAS TREATED WITH I.V. FLUIDS, X-RAYS, AND RELEASED FROM E.R. LATER THE SAME DAY.

DRILL OPERATOR WAS OPERATING DRILL WHEN DRILL SHANK BROKE CAUSING SHANK TO HIT OPERATOR'S FOOT.

EE DRIVING TRUCK. TRUCK OVERTURNED AFTER BACKING OVER BERM. ROLLED DOWN EMBANKMENT. EE HAD ABRASIONS TO LOWER BACK, MULTIPLE BODY BRUISES. SENT TO HOSPITAL RECIEVED X RAYS NEGATIVE FOR BROKEN BONES. RELEASED FROM ER IN 3 HOURS. EE WAS WEARING SAFETY BELT SEAT BELT WHEN ACCIDENT OCCURRED.

SET UP PETTIBONE CRANE WITH ONE LEG ON TOP OF AN ELECTRIC WIRE RUNNING TO A WATER PUMP. INJURED TOUCHED THE CHAIN ON THE GOOSE NECK AND GOT SHOCKED. HE &/OR CHAIN WAS ALSO TOUCHING THE TRACK T HEY WERE GOING TO PICK UP.

EE OPERATING HAND HELD DRILL, DRILLING HOLE FOR BREAKING STONE. WHEN WENT TO MAKE NEW HOLE GOT LEFT PANT LEG CAUGHT IN DRILL BIT, BRUISING KNEE. AFTER INSTRUCTION OF WHAT TO DOO, TOOK ADVICE F ROM DAD & HIS MOTHER & HIS SISTER (EACH WITH SOMETHING DIFFERENT). ON 9-19-95 SENT TO PROPT CARE WITH INFECTED KNEE.

INJURED WAS DRIVING A WEDGE TO REMOVE ANOTHER WEDGE, WEDGE CAME LOOSE (OUT) HITTING INJURED JUST BELOW GLASSES. INJURED SAW "PROMPT CARE", PHYSICIANS CLINIC, INC-ANOTHER DOCTOR CHECKED X-RAYS (AT A LATER TIME) AND HAD HIM SEE A SPECIALIST.

INJ DRIVING SLIP TO BREAK STONE STEEL CAME OFF HAMMER OR SLIP HITTING INJ IN CALF OF RIGHT LEG.

INJURED HAS BEEN SPRAYING WEEDS OFF AND ON FOR LAST FEW WEEKS. INJURED SAID HE WAS SICK, DID NOT KNOW IF WEED KILLER OR FLU.

INJURED & THREE OTHER MEN MOVING BANJO FROM 71A TEREX INTO SHOP INJURED SLIPPED & FELL TO GROUND.

INJURED WAS HELPING BLOCK TRACK FOR SAW AND HAD STOPPED TO GO SHUT OFF SAW. INJURED TURNED, STEPPED IN LOW SPOT IN QUARRY FLOOR, SLIPPED, FELL OVER TRACK, PUTTING HIS RIGHT HAND DOWN TO CATCH HIMSELF, HURTING RIGHT ELBOW.

INJURED CRANKIN1 DOWN WIRE. INJURED USING WRENCH TO TIGHTEN AND LOWER WIRE. WRENCH SLIPPED, CATCHING INJURED'S FINGER BETWEEN WIRE STAND AND WRENCH. MASHED RING FINGER ON LEFT HAND BREAKING BO NE. 3

INJURED WAS MOV3NG FIREWOOD ON LEDGE. INJURED HAD PUT AN OLD PINE 2X6 ACROSS CORE HOLE. INJURED STEPPED ONTO 2X6 AND IT BROKE. HE FELL ABOUT 13 1/2 FT.

EMPLOYEES TURNI3G CUT. INJURED WAS HOLDING AIR BAG (BAG ON ROPE) TO KEEP FROM SLIDING TOO FAR INTO CUT. TOP OF AIR BAG BUSTED, THROWING AIR AND WIRE UP. AIR BAG CONTROLLED BY HAND-AIR BOGS ARE USED INSTEAD O0 HOOKS TO MOVE BIG CUTS OF ROCK AS THEY CAN BE CUT INTO PIECES.

OTHER OPERATOR 4AS ON TOP OF LEDGE LINING UP HOLE. INJURED WAS PRYING UP SIDE OF DIAMOND DRILL AND REACHING TO REMOVE CHOCK, PUTTING HIMSELF IN A BIND.
