On 7/26/2024, while walking on a travelway, EE moved to opposite side of the road due to conditions ahead being uneven. EE stated that upon moving to the other side of the travelway, they felt a pop near the arch of their left foot. EE was seen by company doctor and put on restricted duty and was prescribed the use of an immobilization device.
On 5/13/2024, at approximately 3:50pm, Management received a report of an EE having difficulty breathing after having worked to adjust a gate for a feeder in the surge tunnel. EE was assessed by Plant Mangement and taken to local ER for assessment for CO poisoning. EE was admitted and treated for elevated levels of CO in EE's blood.

On 4/16/2022, EE reported a slip and fall while working in the Secondary Plant. EE continued working through the shift. On 4/18/2022, EE reported swelling in the right hand and was taken to the doctor on 4/19/2022. EE was subsequently provided with a sling to keep the right arm elevated and immobilized for 8 days.

On 12/30/2021, EE reported feeling a burning sensation in lower abdomen following an attempt to break a hydraulic hose on a Loader. EE was subsequently taken to the Dr. and allowed to work pending evaluation by surgeon for potential hernia. On 3/17/2022, EE underwent surgery to repair hernia in lower abdomen.

EE was cleaning screen possum belly, a 2'x2' piece of hardened fine material came in contact with the EE's leg, EE thought that EE sprained it. Other EEs helped EE out, when EE came out the pain increased. Supervisor took EE to the ER, X-rays were taken and determined that it had a fracture on shinbone. EE was wearing proper PPE.

EE was attempting to remove a ripper tip from the D10 dozer. EE was striking the tip with a hammer and a sliver of metal broke off striking EE in the left thigh. EE was taken to the clinic for observation by another EE

EE was pulling on a wrench trying to remove a plug from an L250 Volvo Loader. While pulling EE's left shoulder popped. EE was taken to clinic by another EE. The plug was real tight causing EE to pull extremely hard.

EE was removing rocks from tail pulley when a rock from above fell and hit employee on hard hat causing lacerations.
Employee was performing post operation inspection of impact crusher. While kneeling on one knee, EE was shutting the hatch doors on the crusher. EE lost balance and fell backwards landing on the catwalk.
Employee was shoveling aggregate material off of screen in primary plant, felt shoulder pop followed by a lot of pain. Employee reported it to supervisor immediately.
