Received a Cumulative Trauma claim on 11/20/2013. Employee was terminated on 4/28/2013 and is no longer employed with the company. Accident date of 4/28/2013 is to accommodate online reporting per MSHA (Denver) instructions.

Employee filed a hearing loss claim on February 26, 2013, with the Kentucky Labor Cabinet, Department of Workers' Claims. As per the audiologist's report, employee suffers from "moderate to severe loss." Employee's last day of work was July 24, 2012. **THERE WERE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM.**

Loud Noise/Hearing loss

EE and foreman were moving miner cable. EE fell backwards while holding the cable. He stated he felt pain in his back. This EE had a prior injury to his back in the same area on 11/17/11. This is possibly a continuation of this same injury.

EE was drilling a hole for a roof bolt. As he removed the bottom piece of steel the top piece fell out of the hole striking him on his index finger. This resulted in a small hairline fracture.

The miner was transporting steel jacks on a low trac. As he traveled outby the fire curtain on a medium grade, the steel jacks slid off of the forks, As he was loading the jacks back onto the forks, one of the steel jacks slid off striking his left ring finger.

2 BREAKS OUTBY THE FEEDER AT THE PONY DUSTER- SECTION REFUGE EMPLOYEE WAS CUTTING THE HOSE OFF OF THE DUSTER AND THE UTILITY KNIFE SLIPPED AND CUT HIS RIGHT HAND. REQUIRING STITCHES

The EE was tramming the continuous miner from the #1 heading to the #5 heading in preparation for the next cut. When he approached the #2 intersection in the last open crosscut, he disengaged the power to the miner & bent over to pick up rope attached to miner cable to place on the tail of the miner. When he went to raise up, he felt sharp in lower back, left hip, & into his leg.

A non-injury roof fall was discovered on the MMU No. 003-0 Section. The roof fall was immediately reported to MSHA hotline. The roof fall is located at CC #7 of the 2-North West mains. The roof fall began in the #5 entry and cont'd. through the #7 CC into the #6 entry. The roof fall is approx. 90-95' l. x 5-15' w. x approx. 6-8' h.

A roof fall was discovered in the No 1 return entry of the #2-northwest mains, between break 63 and break 65, just inby survey station #7905. The roof fall was approximately 30'long, by 15' wide and ranged in thickness from 4 1/2' to 5 1/2'. No injuries resulted from the roof fall, and no ventilation effects were detected.

Employee was operating a 10SC shuttle car. He hit a bump in the road and it made him hit his head. This resulted in him chipping his left upper eye tooth.

Note: on June 27, 2011, EE filed a coal workers pneumoconiosis claim with the Kentucky labor Cabinet. This notice was received by our company on July 18, 2011. There is no an accident associated with this claim or report.

EE was attempting to enter a set of air lock doors. He opened the first door and air pressure caused it ti reclose catching the EE's right forearm.

EE was preparing to bolt 2 left crosscut, as he was swinging the drill head around a section of the rib rolled off and struck his right leg from the knee down. The rib roll broke apart upon impact. This resulted in severe bruising to the EE.

EE was helping to re-build a brattice, the blocks rolled out from the pallet and one of the blocks struck EE's right foot behind steel toe. EE finished shift and went to the Dr. 3/21/11. X-rays did show a hairline fracture in his tarsal bone.

Employee got off mantrip to close airlock doors, when the employee turned around the mantrip was upon him. Pinning the employees left leg between the mantrip and the metal air lock doors. This put pressure on the employees left leg and knee.

EE was helping move miner cable out of roadway in the #5 entry, when they started moving the miner cable caught his feet knocking them out from under him. This caused him to fall on his left hip, hurting his lower back. The EE did not go and see a doctor until 12/17/10, the doctor took him off of work.

Employee was helping to make a belt splice. He was reaching on the belt removing the loose belt nails when the employee's co-worker accidentally hit him on the hand with a hammer. The injury resulted in a broken index finger on the employee's left hand.

EE was operating an Joy CM in the #7 entry when a rib rolled off hitting him on his right ankle.

On 1/1/10 an unintentional roof fall occurred on 002 Pillar section inby spad #9888. This fall caught the #3 M.R.S. Fall was 5'X15'wX15'l. MSHA issued K order which was modified to allow cleanup and t recover M.R.S. Area was bolted with 10' cable bolts. M.R.S recovered at 7:30am on 11/2/10 and K order was terminated. Area was dangered off and timbered

Employee was moving M.R.S. cables when he caught his left ear on a nail that was sticking out of a roof bolt plate.

The employee was hanging continuous miner cable in the #8 entry when his back started to cramp and hurt.

The employee was using a hammer and chisel to knock a pin out of a belt drive boom. When the pin came out the boom suddenly moved forcing the chisel down into the boom pin hole, pinching the employee's little finger on his left hand between the boom frame and the chisel. This resulted in cutting the employee's finger requiring stitches.

Employee was helping a Strata employee close door on a refuge chamber. When he pushed the door closed, his right knee popped loudly and buckled causing severe pain to his right knee. Note: The doctor did not take the employee off work until 10/18/2010.

Employee was hand loading belt on a trailer. He stated that he felt something "pop" in his right leg. Within a few minutes, he said his toes went numb and his leg was hurting.

EE was working on a scoop in the No. 3 right crosscut, when a rib rolled off, pinning him against the scoop.

A non injury unintentional roof fall occurred on the MMU No 1003 section.The roof fall occured during retreat mining. The roof fall was located in the #13 entry 2 crosscuts inby survey station # 8263. The roof fall entrapped the cont. miner and the shuttle car. The roof fall was approx. 4-8' thick X 18' wide X 18' long. MSHA was notified.

Employee was helping bolt a lid on diesel scoop. A piece of draw rock approx. 10" x 4" x 1" fell striking him on his left middle finger.

Employee was helping repairman to change a broken torque shaft on the continuous miner. When He removed the bolts from the cover to remove the torgue shaft, hot oil blew out burning his right hand and fingers. He did not receive inhal medical treament until 2/11/2010. This way only first aid until the beeler broke the blister and cleaned burns on 2/17/2010

Unintentional roof fall occurred on the MMV #003 section during retreat or pillar mining. The roof fall entrapped the continuous miner in the #4 entry two crosscuts inby spad # 3648. Roof fall was reported to MSHA by phone on 02/04/2010.

MMU N/003 section was pillaring on retreat mining in the #4 entry. The entrapment of the miner was 12 crosscut up the 3rd/lt panel off the 2-lt submains. Draw rock fell on the head of the miner causing the entrapment of the machine. (MSHA was notified by the telephone on 12/18/09).

Employee was taking rub rail off miner when it fell down catching his left hand injuring the middle part of the index and middle finger required stiches.

Employee was on off side of beltdrive rockdusting. Accident site was muddy. He slipped and fell backwards. Trying to catch himself with left hand he fell over own SCSR on right side of lower back.

Employee was getting out of shuttle car when he stepped into a hole twisting his ankle and breaking it.

EE was in the shop welding. EE put his gloves on when he felt a sting on his left hand. EE's hand turned red and swollen. EE went home and returned back to work with a bad red streak running up from his hand up to almost his arm pit.

EE was installing bolts on fresh turn in at ZRT break, a piece of rock fell from the top and rib bouncing off the canopy. The rock struck EE on the left shoulder and forearm and slid down his back. This resulted in a fracture of the left radius.

The employee picked up a cardboard box and threw it onto the batteries on the scoop. The box did not go all the way upon the equipment. The employee pushed the box with his right hand and felt something pop in his chest.

EE was bolting when he started drilling a hole a piece of rock broke loose catching his little finger between the rock and the drillpot.

An unplanned roof fall occurred on MMU No. 003 section entrapping the continuous mining machine while or during retreat mining in the No. 3 entry. The roof fall occurred in the rt. run of the coal pillar inby survey station #9086. The roof fall is approximately 8 ft long x 15 ft wide x 2 ft thick. (Note: Roof fall reported to MSHA by phone.)

Miner was stuck in #3 entry. Employee and others hooked a chain to a s/car in an attempt to pull it out. Chain broke and part of it hit employee on left hand requiring stitches.

EE was walking down the #5 entry to the power center, when he hit a stap with his left ear.

Employee was changing out a piece of chain on the pick breaker on the feeder. When he hit the chain with a hammer, a piece broke off striking him in the left arm (bicep area).

Employee was on a mantrip when the mantrip struck a timber causing a piece of draw rock to fall and his him on his right wrist.

The person was closing a knife and cut his leg with the blade.

Shoveling at #9 head drive, when he pulled back on the shovel he had a sharp pain go thru his lower back.

EE was switching seats on the shuttle car when a piece of rock fell out hitting him on the lower left shoulder.

Employee was operating a 21sc shuttle car. He rounded a corner and had his knee hanging out. He struck against rib.

EE was bolting on his knee's, when he went to stand up his right knee popped. He said it felt like it popped out of place

Employee was pulling miner cable beside the miner, when he twisted he had a pain in his lower back.

EE was shoveling belt with a #2 shovel when he turned around he hurt his back.

Employee was roof bolting in the n 0/5 heading on mmun 0/003 section when the rib rolled out hitting his left ankle which resulted in a fractured left ankle.

Employee was drilling the inside hole when he hit the fast feed lever. The head caught his knee pad causing the boom to hit his left thigh, twisting his knee.

EE was pulling cable when his hand slipped causing him to fall back landing in his left hand.

Employee was surface when he picked up a spring for a mac-8 diesel ride. When he started to pick the spring on a pallet he felt pain in his back.

Employee was moving cable anchor when it fell catching his right hand between the anchor and the shuttle car. Employee did not go to doctor until 7/15/08.

EE was making a hammer splice in the belt. He hit the splicing tool with a 41 lb shop hammer and a piece of the hammer broke off, striking him on the right arm, above the elbow.

EE was turning around in shuttle car deck, when he lost his footing, falling into the instrument panel and struck his left rib cage.

Employee was opening a bag of rock dust with a utility knife when he cut the top of his left hand with the blade

EE was unplugging a cathead on scoop battery and when he pulled the cathead out, it hit him in the mouth.

Employee was operating a scoop when the rib rolled out striking him in the back cutting his neck and ear.

EE was pulling a strap over so he could bolt it up, when the strap slipped, cutting his right hand middle finger.

Employee was using a knife to cut a cathead out of a box. The knife slipped and cut employee's right leg.

Employee was roof bolting, when his right arm got caught on the roof bolt, twisting his right arm.

EE was moving rocks out from in front of shuttle car, when the rib rolled out and caught his leg between the rock and the car.

EE was running the miner when a piece of rock rolled off the rib onto his right foot.

EE was shoveling headdrive, when he took his foot to push the shovel into much, he felt a sharp pain in his lower abdomen. Did not start missing work due to injury until 12/19/07, having hernia surgery.

Employee was roof bolting, when the rib rolled out, pushing his right leg into the roof bolter.

Employee was walking down the roadway on #6 belt, when he stepped in a hole, causing his right knee to pop out of place.

Employee was welding and grinding on a mini-trac muffler when he burned both eyes. He also got a small piece of metal in his left eye.

While roof bolting, employee swung the head of the bolter over to put up last bolt when a rock fell off the rib striking him on his left elbow.

Employee was letting air out of waterline so he could change a piece of it. When he opened valve, the air rushed out & when the water came through the line it smacked the water hitting the employee in the mouth, knocking him to the ground.

Employee was pulling a 10' timber across the scoop when he caught his left pinky finger between the scoop and timber.

EE was making a splice on shuttle car cable when knife slipped & cut left leg (knee area). Rec'd 8 stitches.

Employee was walking to get cable when he stepped on a piece of pallet. A piece of the pallet flew off and struck him in the left eye.

EE was loading roof bolting supplies onto roof bolter when he twisted around and felt a sharp pain in his back.

MRS was in #4 entry; had finished right run when operator let shield down. Top came down on MRS. This was a non-injury roof fall.

Was getting the 2nd run in front of block between the #5 & #6 entry. When the miner head cut thru a piece of rock fell on the head pushing bits into bottom lodging in the miner.

Employee was throwing roof bolts when he felt his shoulder pop.

Employee was working around the welder when his eyes got welding burns.

Employee was getting ready to bolt the top when the rib rolled out pushing him into the roof bolting machine.

Employee was loading a structure trailer when it slipped off of block and caught his left foot.

Employee was helping with a stuck miner. He was throwing cribs when a piece of rock rolled off the rib and struck his left ankle.

EE was beating on a piece of metal, when a piece of it flew off, striking him on his right thigh. EE did not start missing work until 10/12/06.

Employee was holding slate bar while unrolling cable off of spool. The spool caught the bar and threw employee down. When he fell the slate bar cut his right hand palm. He received 2 stitches.

Employee was working on a scraper on the headdrive when a piece of screen flew back and hit him in left eye.

Employee was roof bolting when a piece of rock fell out from the rib and struck him on the left shoulder and lower back.

The miner cut thru on the first run in new pillar in #2 entry, when it went thru top set down on miner head. This was a non-injury fall.

While roof bolting, drill steel fell out of hole & smashed his right finger against the drill head of roof bolter.

EE was getting out of shuttle car when he hurt his right shoulder. He said he hit a rock in roadway earlier & hit his head but he said his shoulder did not hurt until he pulled himself out of shuttle car.

Employee was picking up a pump when he pulled something in his lower back. Employee was also experiencing chest pain.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #7 ENTRY ON #2 BELTLINE. IT MEASURED 20' X 30' X 4'. THIS FALL WAS TIMBERED AND DANGERED OFF AND WILL NOT BE CLEANED UP.

EE was picking up a pallet when he felt something tear in his right shoulder.

Employee was moving miner cable when he felt something pop in his left shoulder.

EE was picking up the belt splice board when he turned around and fell over the belt knife. *EE started missing again due to this injury on 2/7/06. **EE started missing again due to this injury on 3/27/06.

Employee was driving mantrip when a foreign object flew into his left eye.

EMPLOYEE WALKED INTO 2 ROOF BOLTS HANGING FROM THE ROOF JARRING HIS NECK.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED DURING WEEKLY EXAM. 001 SECTION AT 29 X-CUT ALONG #1 BELT IN THE #2 ENTRY WHICH MEASURED 5' X 20' X 20'. TIMBERED AND DANGERED OFF.

employee jerked a piece of cable down from roof bolter pulling a muscle in his back.

EMPLOYEE WAS STEPPING OFF MANTRIP WHEN HE FELL & INJURED HIS LEFT WRIST.

THE MANTRIP STALLED GOING UPHILL AND EMPLOYEE WAS THROWN FORWARD. HE CAUGHT HIMSELF WITH RIGHT HAND INJURING WRIST. PLACED ON RESTRICTIONS FOR 3 WEEKS

An unintentional roof fall occurred on the pillar line of 002-section which temporatily caught 2 mrs's both were recovered fall area abandoned.

EE was tramming a mac 8 down the roadway when his right wheel caught on a piece of belt causing the steering wheel to spin and catch his right hand fracturing his right thumb.

Draw rock fell hitting employee in head and shoulder.

EMPLOYEE WAS PUTTING A PIECE OF SHEET METAL ON A TAILPIECE WHEN THE METAL SLIPPED OFF THE HOOK & SMASHED HIS RT. MIDDLE FINGER. RECEIVED 8 STITCHES TO FINGER.

EMPLOYEE WAS PULLING MINER CABLE WHEN THE RIB ROLLED OUT AND A ROCK FELL OUT STRIKING HIS LEFT WRIST. RECEIVED 9 STITCHES IN LEFT WRIST. NARH.

EMPLOYEE WAS TRAMMING MANTRIP UP HILL WHEN IT FELL OUT OF POINT, STARTED ROLLING BACKWARDS AND ROLLED OVER HIM. HE WENT TO DOCTOR ON 9/14 AND RETURNED 9/15 WITH RESTRICTION.S

EE was getting ready to set timbers on his pillaring section when a piece of drawrock fell out between the bolts striking his left shoulder.

EMPLOYEE WAS REMOVING A BIT INSERT FROM THE MINER. THE PORTA POWER TOOL HE WAS USING BROKE, FLEW BACK & HIT HIM ON HIS HEAD. HE HAD 1 STITCH/ STAPLE PUT IN HIS HEAD. ON RESTRICTED DUTY FOR 1 WEEK.

EMPLOYEE WAS PUTTING A PIECE OF WOOD UNDER THE TAILPIECE WHEN THE MINITRAC FORK SLIPPED AND HIT HIS LEFT ARM. WHEN HE GOT HIT WITH THE MINITRAC FORK, HE FELL BACK AND BRUISED HIS RIGHT HAND.

The repairman was positioned about 20'in front of the machine & instructed the miner operator to stop because the curtain was caught on the ripper ring. Machine operator heard repairman say OK & assumed he was clear of the machine. He started tramming backwards & repairman was caught between rib & left ripper ring.

EMPLOYEE WAS WALKING OUTSIDE WHEN HE HIT HIS HEAD ON THE OUTSIDE CANOPY REQUIRING 8 STITCHES TO HIS FOREHEAD.

EE WAS AT THE BOLT MACHINE MAKING UP EXTRA BOLTSWHEN A PIECE OF ROCK ROLLED OUT OF THE RIB LANDING ON HIS FOOT

EE WAS MAKING BELT MOVE WITH STRAINED HIS LOWER BACK WHILE PUTTING IN BOTTOM ROLLER.

EMPLOYEE WAS HANGING ROOF BOLT CABLE WHEN THE BOLTER BACKED UP A LITTLE BIT CAUSING THE CABLE TO STRIKE HIS NECK. EMPLOYEE WAS THROWN TO GROUND.

EMPLOYEE WAS GOING ACROSS HANGING CURTAIN WHEN HE STOOD ON A ROCK, SLIPPED OFF AND FELL ON HIS RIGHT SHOULDER. ON RESTRICTED WORK FOR APPROX 2 WEEKS.

A NON-INJURY ROOF FALL OCCURRED ON THE 002 MMU PILLAR LINE. THE ROOF FALL WAS IN THE #4 ENTRY AT CROSSCUT #3 ON THE #2 LEFT PANEL. THIS FALL WAS APPROX. 10'X 20'X 4'.

EE WAS CUTTING BELT WITH A CORD BELT KNIFE AND WHEN HE PULLED BACK ON IT HE SAID SOMETHING POPPED IN HIS LOWER BACK.

EMPLOYEE WAS CAUGHT BETWEEN A ROCK AND THE MINER HURTING HIS LEFT LEG AND FINGER. (HE WAS SEEN IN E.R. ON 1/28 AND FOLLOWED UP BY DOCTOR VISIT ON 1/29 WHO RELEASED HIM FOR WORK).

EE WAS CARRYING TOOL BOXES ACROSS SECTION WHEN HE HIT A ROOF BOLT PLATE HANGING DOWN.

EMPLOYEE WAS HELPING REPAIRMAN PUT A PIECE OF METAL ON A SHUTTLE CAR WHEN HIS BOTTLE JACK FLEW OUT AND CAUGHT HIS FINGER. RECEIVED STITCHES.

EMPLOYEE WAS ROOK BOLTING WHEN THE RIB ROLLED OUT STRIKING HIM IN HIS RIGHT SHOULDER.

A NONINJURY ROOF FALL OCCURRED ON THE 001 MMU PILLAR LINE. THE FALL WAS IN #6 ENTRY AT #16 X-CUT IN 2 LEFT PANEL OFF OF 1 WEST MAINS. THE FALL WAS APPROX. 30' X 20' X 4'.

EMPLOYEE WAS BOLTING WHEN A PIECE OF THE RIB ROLLED OUT STRIKING HIM ON HIS RIGHT LEG AND ANKLE.

EE WAS MOVING A BOX OF GLUE FROM BACK OF ROOF BOLTER AND TWISTED HIS LOWER BACK.

EE WAS INSTALLING A LIGHT HOUSING ON A SHUTTLE CAR WHEN IT SLIPPED OUT AND HIT HIM IN THE MOUTH, BREAKING HIS TOOTH AND CHIPPING OTHERS.

EMPLOYEE WAS PULLING DRAW ROCK WITH A SLATE BAR WHEN HE PULLED SOMETHING IN HIS MID BACK AREA.

EE WAS DEBELTING WHEN HE TURNED AROUND AND FELL DOWN ON HIS RT. ARM.

THE BOLT MACHINE WAS TRAMMING FROM #2 ENTRY TO #1 ENTRY INTERSECTION. A ROCK FLEW FROM UNDER THE TIRE OF THE MACHINE AND STRUCK EMPLOYEE IN LEFT EYE.

EE WAS CUTTING A BAG OF DUST WHEN HIS KNIFE SLIPPED CAUSING HIM TO CUT HIS RIGHT WRIST REQUIRING3 STITCHES.

EE WAS PULLING BELT WHEN HIS VISEGRIP SLIPPED & HE FELL TO THE GROUND INJURING LOWER BACK.

EE WAS GETTING OUT OF SHUTTLE CAR WHEN THE CAR ROLLED AND TWISTED HIS LEG BETWEEN THE CAR AND THE GROUND.

EMPLOYEE WAS GETTING A ROOF CAP OFF THE ROOF BOLT MACHINE WHEN THE STACK OF CAPS FELL OVER ON HIS HAND. EMPLOYEE FRACTURED LEFT INDEX FINGER.

EMPLOYEE WAS SCOOPING OLD WORKS WHERE IT WAS LOW. HE WAS BACKING UP, LOOKED AROUND AND HIT AN INSULATING BOLT IN THE ROOF CAUSING HIM TO STRAIN HIS NECK. DID NOT SEE DOCTOR UNTIL 1/21/02. RTW SLIP FROM DR 1/28/02.

EMPLOYEE WAS WALKING DOWN A HILL WHEN HE LOST HIS FOOTING AND FELL CAUSING HIM TO TWIST HIS RIGHT KNEE.

EMPLOYEE WAS UNHANGING BOLTING MACHINE CABLE WHEN HE DROPPED THE CABLE HANGER AND IT CAME AROUND AND CUT HIS RIGHT EYELID. EMPLOYEE HAD OUT-PT SURGERY TO SUTURE HIS EYELID.

EMPLOYEE WAS HANGING CABLE AND WATERLINE WHEN HE EXPERIENCED A SHARP PAIN IN HIS GROIN. A PHYSICIAN HAS DETERMINED THAT HE HAS A HERNIA AND IS SCHEDULED FOR SURGERY 11-8-01.

EMPLOYEE WAS ENTERING SHUTTLE CAR WHEN HIS HAND SLIPPED CAUSING HIS TAILBONE TO HIT THE FRAME OF THE CAR.

EMPLOYEE WAS GETTING SUPPLIES WHEN A THIN METAL STRAP CAME LOOSE FROM SUPPLIES STRIKING HIS LEFTFOREARM.

EMPLOYEE WAS WALKING UP BELT, STEPPED IN HOLE AND FELL INJURING HIS BACK AND HIP.

EMPLOYEE WAS UNHANGING MINER CABLE WHEN THE HOOK FROM THE HANGER FLIPPED DOWN AND CAUGHT HIS NOSE. WHEN THE MINER CABLE FELL, IT CUT HIS NOSE.

EE WAS IN #2 ENTRY PUTTING UP A ROOF BOLT WHEN THE DRILL STEEL CAUGHT HIS LEATHER GLOVE AND TWISTED HIS RIGHT WRIST.

EE WAS CLEANING UP AROUND #1 NORTHWEST DRIVE. HE WAS WALING BEHIND START BOX TO GET A BAG OF ROCKDUST WHEN A PIECE OF DRAWROCK FELL OUT OF TOP AND STRUCK HIS FOREHEAD.

EE WAS PULLING A PIECE OF DRAWROCK WHEN IT CAME OUT BEHIND HIM STRUCKING HIS RIGHT LEG.

EMPLOYEE WAS HELPING MAKE A BELT SPLICE WHEN THE SPLICE MACHINE FELL OFF THE RAILS AND CAUGHT HIS THUMB AND FINGER UNDER THE RAIL WHICH CAUSED THE SPLICE MACHINE TO LACERATE HIS FINGERS.

EE WAS THROWING PLATES ONTO THE TOP OF THE BOLTER WHEN HE HIT HIS ARM ON A STRAP WHICH WAS STICKING OVER THE EDGE OF THE BOLTER.

EMPLOYEE WAS IN THE PROCESS OF RUNNING CENTER LINES. HE WAS WALKING OUT OF #4 ENTRY RIGHT BREAK WHEN HE STOPPED TO TELL THE SCOOPMAN WHAT TO DO.THEN A PIECE OF ROCK FELL OUT BETWEEN BOLTS STRI KING EMPLOYEE ON BACK AND RIGHT ANKLE.

EE WAS ROOF BOLTING & WAS PUTTING UP A ROOF MAT ACROSS THE ENTRY WHEN A ROCK FELL & HIT THE OTHER SIDE OF THE STRAP. THE ROCK PUSHED THE STRAP AWAY FROM HIS HAND & SLICED HIS FINGER.

EE WAS LAYING STRUCTURE OUT OF A SCOOP BUCKET. WHEN HE TURNED TO PLACE THE STRUCTURE ONT HE RIB., HE STRAINED HIS RIGHT KNEE.
