Employee was squatting, working on a rail guard on the crusher when they felt a pop in knee. After the initial doctors office visit, EE was prescribed self care. The knee did not improve and doctor requested an MRI. The MRI revealed a torn meniscus that would require surgery to repair. Employee underwent surgery on 10/7 was returned to modified duty following surgery on 10/10.

Employee was walking to the picnic table for lunch and twisted right ankle. X-rayed and given Ibuprofen and an ace wrap and told to keep elevated with ice. Released to regular duty.

During a thunderstorm, EE was at desk in scale house doing paperwork. Lightning struck nearby radio tower & power went off. The floor was damp due to heavy rain. About 5-10 min later, EE began to tingle & burn. EE went to ER that evening and they treated for burn of nerve endings from waist up. Administered steroids and Benadryl by IV. EE was discharged with no further treatment.

Employee was loosening bolts with an impact and wrench. Wrench slipped and hurt his middle finger. Employee didn't mention the injury until next day. X-ray shows no break but MRI has been ordered to check for tendon damage.
Employee was climbing up ladder and lost his grip, falling against his right shoulder. Diagnosed with right shoulder contusion.
Employee was loosening the bolts to open the impactor door. He didn't realize he hurt himself and reported the injury on 3/12/2014. Diagnosis was left shoulder strain. On restricted duty and receiving physical therapy treatments.

Employee was cleaning out a lime surge bin and a chunk of packed lime fell on his hard hat. He told his supervisor but declined medical attention and went home. He decided to go to the local hospital ER that evening for evaluation without telling anyone. They performed a scan for a concussion but found nothing wrong. Employee did not file claim until 11/11/08.

Employee was unscrewing a drill bit from the drill and it unscrewed the entire 18' drill steel from the drill and it fell down on her left hand. Several stitches were required to the lacerations of the last three fingers on her left hand. There were no broken bones but there is still concern that amputation of part of her ring finger may be required.
EMPLOYEE WAS TIGHTENING THE BOLTS ON THE IMPACTOR WITH A WRENCH. THE WRENCH FELL, HE SPUN AROUND AND LOST HIS BALANCE AND FELL (APPROX 8 FEET) TO THE GROUND AND LANDED ON HIS FEET, FRACTURING BOTH FEET AND REQUIRING SURGERY.

EMPLOYEES WERE REPLACING BELTS ON THE FINISH SCREEN. THE ELECTRIC MOTOR WAS PULLED BACK WITH A LOG CHAIN AND A COME-ALONG AND IT SLIPPED AND SWUNG INTO THE INJURED EMPLOYEE'S CHEST CAUSING A CONTUSION TO HIS LEFT RIB CAGE AND LUNG.
Employee was unloading anfo bags from the powder truck and he slipped and fell, landing across the wooden bed frame. Diagnosis was contusion of right chest.
EMPLOYEE WAS CLIMBING THE LADDER INTO THE LOADER, SLIPPED AND FELL, LANDING ON HIS LEFT LEG CAUSING STRAIN TO HIS BACK AND LEFT BUTTOCK. HE WAS EXAMINED, TREATED WITH MEDICATION AND RETURNED TO WORK WITH NO RESTRICTIONS. HE RETURNED AFTER A FEW WEEKS COMPLAINING IT WAS NOT COMPLETELY HEALED. AN MRI AND EPIDURAL STERIOD INJECTION WAS GIVEN ON 5/4 AND ANOTHER INJECTION ON 5/24/04.
EE WAS HELPING UNPLUG THE CONE CRUSHER WITH A STEEL ROD. HE HAD HAD PREVIOUS SURGERY ON HIS WRIST AND HE COMPLAINED THAT IT WAS HURTING AGAIN. HE WAS SEEN BY ORTHOPAEDIC DR WITH A DIAGNOSIS OF EXACERBATION OF RIGHT WRIST. HE WAS PLACED ON RESTRICTED LIFTING, PUSHING, OR PULLING.
