Employee hooked lifting strap to water pump. Injured Employee was still standing on tongue to pump. Strap connected to excavator. Excavator operator rolled the bucket of the excavator so other employee to remove chain from bucket. This exerted tension on strap which caused pump to become un-hooked. Pump spun, Injured operator slipped between A-Frame causing cut on shin.

Operator was performing pre shift on Front End Loader. When accessing the top compartment to check coolant level, the access door fell forward and landed on his hand, breaking his middle finger.

While cutting a 16" HDPE pipe with a chainsaw, the pipe snapped midway through the cut and fell onto the employee's left foot, causing a bruise to his ankle.
While stepping over a 14" HDPE pipe, the employee placed his foot on slightly uneven ground, which caused his ankle to turn.

Pond 1135 experienced a failure of the west wall with approximately 9 inches of tailings and water leaving the pond, traveling through a stormwater conveyance ditch, filling a sediment basin and leaving site through the rip rap outfall.
Employee was dumping oversize material with FEL when right front tire sunk in mud. Employee's hand slipped off steering and was caught by spoke resulting in his wrist twisting & causing slight pain. On June 15 employee's wrist still hurt & he was seen by company doctor and was diagnosed with hairline fracture of bone in his hand.
Employee was cutting grass with zero turn lawn mower. Tire dropped in ditch causing mower to turn on its side. Roll bar prevented mower from turning completely over. Employee stuck arm out to stop roll over and strained his shoulder.
Employee was attempting to line up 2 pipe flanges so they could be bolted together. As he pulled up on pry bar to align bolt holes, he felt something pull in right shoulder blade area. He did not report this but @ 1530 hours his back began cramping & he notified the Foreman. He was seen in local ER and diagnosed with a muscle strain. Pain med prescribed.

Employee was walking on a slight slope, slipped and twisted left ankle. Seen in local ER and diagnosed with sprained ankle. Returned to work on restricted duty.
Employee was walking on flat, level ground and left knee began to hurt. Went to lunch and noticed swelling. No slip/trip/fall/bump/twist occurred prior to pain. Seen by Dr. who diagnosed swelling on left knee - gave ice pack, ace wrap, prescription pain med & 3 days light duty. Employee does not believe it is work related.

Employee was tightening a flanged suction pipe connection to a diesel pump when the open ended wrench slipped and he fell backwards landing on the frozen ground. Employee suffered a contusion to his buttocks. Seen by Dr who provided prescription and 2 shots as well as 2 weeks "restricted duty."

Employee was dumping a bag of lime in process pond weir. Wet lime splashed on his face. He removed his hard hat & safety glasses to wipe lime off face and caused it to fall into his eye. Employee immediately flushed his eye with water but suffered a small chemical burn under top eye lid. Employee taken to Occupational MD and given prescription eye drops for irritation.
Electrician & mechanic were changing junction box on motor. Electrician threaded 2 of 4 mounting bolts & was having difficulty starting threads on 3rd bolt. As he started threads, mechanic tapped top left corner of box with hammer. This was at the same time the electrician placed his gloved hand on top left corner of box and was struck by hammer.
Employee went to step over 18" pipe in mining field. As he stepped on the pipe, his foot slipped off, causing him to fall to the ground. He stuck his right arm down to break his fall & jammed his shoulder. At that time he refused medical examination & said he was fine. On 6/11/09 he was diagnosed with torn humeral ligament & given cortisone injection & 4 weeks of physical therapy.
Employee was clearing blockage from mining unit pump suction. He slipped & fell from a standing position, landing on his left hand. Unknown object punctured his left palm. Seen by company Occ. Med Center & sent to hand surgery specialist for stitches.
