While shoveling material off of a radial stacker, the employee reported feeling pain in abdomen that continued throughout the day. After following up with the physician, EE was diagnosed with a hernia on 8/27/21. Repair surgery is scheduled for 10/14/21. EE currently has not lost any time and is not on restricted duty.

Employee was cutting material for belt splicing, the knife slipped and sliced EE's thumb. Stitches were required to close the wound.

EE was backing the yard truck under the finished product bins to load out. While turning the wheel, EE caught finger in the vent window latch and dislocated it.
Employee was clearing vegetation around a building on the mine site when they suffered an allergic reaction resulting in a rash on the arms and torso.

Injured employee was changing screen panels and cut a finger on the Unistrut channel that holds the panels in place. Employee needed to go to an urgent care facility where Dermabond was used to close the wound.
Employee was welding while standing on a ladder. The ladder was not secured properly and slipped out of position causing the employee to fall, injuring knee. Employee has been off work since 4/15/16 and has an appointment for an MRI later this week. Will be off until further notice.

WHEN SCALE OPERATOR ARRIVED AT WORK AT 7 AM HE NOTICED SMELL OF GAS AND THE HEAT WAS NOT ONE. HE IMMEDIATELY VENTILATED THE BUILDING AND AT APPROX 10 AM WAS ABLE TO RELIGHT THE FURNANCE. AS NEAR AS CAN BE DETERMINED, APPARENTLY SOME GAS REMAINED TRAPPED IN A FILE CABINET. WHEN EMPLOYEE ATTEMPTED TO LIGHT A CIGARETTE AT 12:40, OUR SCALE OPERATOR NOTICED THAT HIS BEARD WAS ON FIRE
