Talon Concrete & Aggregates LLC · Struck by falling object

Employee was bar scaling in the back of a room when a piece of rock from behind where he was working came down and struck him on the backside of his head. His hard hat did not cover this portion of his head, it struck him about two inches above the hairline on his neck. Resulting in a laceration to the back of his head.

Employee turned a corner while driving a haul truck and ran over a rock. The rock shook the truck, causing the driver to strike his shoulder on the left side of the cab. Employee suffered a dislocated shoulder, which will require surgery to repair.

EE HAD POSITIONED ON THE GUARD RAIL OF TAIL PULLEY AND QUARRY MANAGER INSTRU CTED HIM TO GET OFF GUARD. WHILE DOING SO, A WELD BROKE ON THE PIPE GUARD SUPPORT BRACE AND EE FELL TO THE GROUND.
