Slipped off or missed step on truck. Rt Arm full of personal items, stepped down and missed a step. Grabbed the grab handle of the truck and pulled right arm.
Employee bumped left knee on conveyor belt; piece of the conveyer frame. Employee reported 3/5/21 that employee did it and was a little sore but would let supervisor know if it needed attention. 3/20/21 reported it was getting worse; not better. 3/21/2021 went to immediate care.

Employee was hammering in a pin and part of the pin broke off; pierced through boot and went into right shin bone.

There was a rock stuck in the drop box tailgate. Went to remove the rock form the tailgate with hand, tailgate closed and pinched finger.
Climbed up on a conveyor belt to change product; slipped on small rocks; with a chain hoist in one hand, the hoist whipped around and hit worker on the mouth when worker slipped. Accident caused a broken tooth and caused some damage to bridge in mouth due to the broken tooth.
Employee was cutting into a conveyor belt with a saw knife and cut on left forearm. EE received several stitches at urgent care.

Employee was operating an excavator with the front window in the "up" position when a small rock flew in the open cab adn struck his front tooth.
The employee was performing routine task of moving gates in the screen blending chute to change size of product. While using a pry bar to move gate, his hand slipped and bumped his elbow on the steel frame of the screen. Impace caused a brusing & swelling to left elbow. He has been placed on "off duty" status per dr.Follow up on 6/2/06.
