EE and another employee were removing a crusher bowl for maintenance and while using the ram to unscrew the bowl, the EE reached down to reposition the ram and as EE did the control operator retracted the ram and pinched the EE's finger between the ram head and the cylinder

driver was backing pickup along the outside edge of embankment got to close and slide down embankment, the seat belt caused to strain muscle in left shoulder.

Employee was positioning a crusher adjustment ring with a pry bar. The pry bar slipped, and they stepped back to catch balance. Right heel landed on a 6" transition piece and they felt a twinge in the knee. Employee has been performing regular job until being placed off work by an orthopedic doctor effective 4/25/19 in preparation for upcoming surgery on the knee.
The mine shift ended at 5:30 PM 1/30/19. Last employee left around 6PM. Sometime between 9pm and 10pm, a non-employee called 911 to report a missing person, reported they were allegedly looking for a fishing spot and got separated. Raleigh FD found unconscious victim on mine property. Victim and associate were trespassing on mine property.

Employee was utilizing a small/dremel type tool to grind a shaft when a piece of debris entered employee's left eye. The employee did not report the injury until the following day. Employee received treatment at a local clinic and was prescribed eye drops.

Accident occurred while employee was descending a 24 ft. ext. ladder after having removed crane rigging from a steel weldment. After descending ladder to 9 ft. above ground, right side of ladder sank causing ladder to roll resulting in employee falling onto a small pile of dried concrete and breaking lower right leg.

Annual audiogram revealed a Standard Threshold Shift on an employee. Subsequent re-tests confirmed the shift.
During the process of lifting and pulling a 3" poly discharge hose from a position in the man lift to remove water from the south side of the creek in a sump area the employee through he experienced a potential back related problem. He thought that it might get better overnight but was still experiencing a strain the following morning.

Employee called at 12:48 and stated that he was parking his truck and for me not to panic that he was having chest pains. I called 911 at 12:49. Other employees were involved by radio to assist with the situation. Emergency vehicles were dispatched at 12:50 according to the Fire department personnel. MSHA was contacted at 1:03.

Before attempting to start the pit pump, there was a piece of expanded metal laying on top pump that was picked up by employee and tossed to the side. When he tossed & let go of it, a sharp edge cut his finger.

I.E. was working on a piece of mobile equipment in the pit with a contract mechanic. The i.e. heard a sound that startled him which prompted him to suddenly move away from the mobile equipment. As he moved away he sprain a muscle in his upper left leg.
While installing a troughing idler can the i.e. was using a pry bar to push the conveyor belting; pry bar slipped out of the i.e. hand and the pry bar struck the employee in the chest.

Employee was removing a lodged piece of metal plate and when the plate came loose, it struck him in the wrist. Physical therapy prescribed.

EE notified supervisor that he wanted to see him. When his supervisor arrived, EE then mentioned having chest pains. 911 was called. MHSA was notified. When EMS arrived, they said that the EE wasn't having a heart attack at that time but he needed to go to hospital. EE told EMS that the pain started approx. 15 min. after lunch. EE was taken to hospital where he stayed overnight.
Employee was sitting on a rock and when she went to get up, her foot caught the rock causing her foot to twist. No lost time, no restrictions...only reason reporting is that the doctor prescribed a few days pf physical therapy.
While removing conveyor belting from truck, employee's right pinky finger was pinched between the pallet and bed of truck. Wound required 5 stitches.

While lifting a sample tray of rocks, the QC technician suffered a mild muscle strain in his back.
After using a sledge hammer to remove jaw die bolts, employee experienced pain in his lower back. He was diagnosed with a muscle strain and placed on light duty.
While walking in the plant area, employee stepped on a rock, twisting his ankle. He was diagnosed with a mild sprain and placed on restricted duty.

Two EE's were removing a spring hander from the grizzly feeder. When they moved the hanger, it slid and caught injured's finger between a handrail and the hanger, cutting it between the pinky finger and the ring finger on his left hand.
The employee was walking near a belt conveyor when a high velocity wind gust blew grit off the conveyor into his face, around the edge of his safety glasses and into his left eye. His eye was flushed with solution and he was taken to a physician for examination and received prescription eye medication to prevent infection. He returned to work without restrictions.

Contractor crane operator stepped down from crane to help crew rig a steel plate. The plate slipped out of operator's hand, cutting the right index finger at the top. The laceration required fourteen stitches to close. Gloves were not used when rigging the plate.

Employee had finished filling a water truck, and stepped into a hole that rainwater had washed out from underneath the ground next to the electrical switch at the filling station. The employee fell against the edge of the electric box, fracturing a rib on his right side.

While running rock bolter ee reported numbness on left side of body. Ambulance was called and ee was transported to hospital. Was diagnosed as having a mild heart attack. Ee returned to work with no restrictions two days later. EE scheduled to return to physician for additional tests as he suffers from high blood pressure.
