Employee was removing the bucket from the skid loader

Employee was cutting a piece of rope with a utility knife and cut the upper part of the thumb on their left hand.

2 employees working on the plant one of them was hitting a part with a hammer when it bounced off the part and hit the other employee in the mouth.

Drilling out part using a air reamer. Reamer jammed and spun around causing me to lose balance falling to the floor the reamer dislodged from the reamer bit hitting employee in the head causing a laceration and shoulder contusion

Employee was hosing out under the plant. A rock came from the upper level conveyor and hit the employee on the back side of the head, causing a cut from the hard hat which required 8 stitches. He was bend over at the time of impact

Employee was cutting urethane for the plant with a utility knife. Utility knife slipped and he stabbed himself in the thigh.

Employee was exiting the dozer and caught his thumb in the door while closing. Resulted in employee cutting end of thumb and receiving stitches.

Employee was removing bolts on a conveyor chute with a hammer and chisel. Employee struck his left thumb while hitting the chisel.

Employee was cutting bolts off of screening machine, grinder kicked back and cut the knuckle on left hand.

Employee was working on the plant and when he stepped down he stepped on a rock twisting his ankle

Employee was walking across hood of the rock truck when he went to step down to adjust the mirror he slipped and twisted his right ankle

EE was adjusting the brakes on the rock truck. was pushing wrench with his foot and his knee popped.

EMPLOYEE WAS GREASING DOZER & LAYED THE GREASE GUN DOWN ON BLADE. EE STEPPED ON GREASE GUN & LOST HIS BALANCE. HE FELL & HIT HIS KNEE ON THE BLADE SUPPORT.

EE was stacking hay on the back of the pickup. Another guy was throwing the bales down to him and hit his knee. Knee popped out of place. Went to hospital, may have torn ligaments.

The employee was injured while assisting with a processing plant repair and maintenance project. as he was climbing onto the plant structure, his company telephone and his company radio became lodged on the structure temporarily restraining his from further movement. in the process of dislodging himself from his precarious position he strained his shoulder.

UNLOADING 100 LB STEEL PIN FROM BED OF PICKUP, PIN STARTED TO FALL TRIED TO CATCH IT AND FELT PAIN IN LEFT WRIST.

EE WAS REMOVING PIECES OF STEEL FROM CRUSHER & CAUGHT THUMB ON STEEL. CUT RT. THUMB REQUIRING 5 SUTURES.

EE WAS LOADING STEEL IN BUCKET OF HILIFT AND GOT HIS RIGHT HAND CAUGHT BETWEEN STEEL AND CAT WALK ON PLANT.

EE WAS LIFTING DOOR OFF OF HAZEMAG CRUSHER. HE PULLED MUSCLES IN LOWER STOMACH AREA.

EMPLOYEE WAS PULLING CABLE THRU SHEAVE, WHEN CABLE CAME THRU IT HIT HIM ON THE RIGHT ANKLE.

WAS HELPING TO REMOVE OLD WIRE FROM OFFICE BUILDING LOST BALANCE ON LADDER HIT HANDRIAL.

EE WAS CLEANING OFF CATWALK & A ROCK FELL OFF CONVEYOR FROM ABOVE & STRUCK EE ON THE BACK OF THE HEAD

EE WAS FUELING EQUIPMENT WHEN THE CHAIN CAME OFF HOSE REELER DRIVE. WHEN REINSTALLING SMALL CHAIN HE GOT HIS THUMB CAUGHT IN THE CHAIN.

EE WAS HELPING TO OPEN DOOR ON DEISTER ON SAND PLANT AND PULLED MUSCLE IN HER LOWER BACK AREA.

EMPLOYEE WAS REPAIRING SAND CHUTE AND GOT A PIECE OF SAND IMBEDDED IN HIS LEFT EYE.

EE WAS CITTING BOLTS USING A TORCH WHEN A PIECE OF HOT METAL WENT INTO HIS LEFT EYE AROUND THE SAFETY GLASSES.

EE WAS LIFTING PIECE OF STEEL WHICH SLIPPED & HIT HIS LEFT FOOT.

INJURED AND EMPLOYEE WERE INSTLLING THE BLADE ON D85-P-18 KOMATSU DOZER. THE SUPPORT AMR ON THE DOZER BLADE SLIPPED ND SETTLED DOWN ON EE'S FOOT.

EMPLOYEE STEPPED DOWN OUT OF TRUCK & TWISTED BODY & FELT SOMETHING POP IN HIP AREA. EMPLOYEE DID NOT FALL OR SLIP ON STEPS - HE NEVER LOST HIS FOOTING.

NO RAIN OR THUNDER/LIGHTNING APPEARED TO BE IN AREA AT THE TIME OF ACCIDENT. EMPLOYEE WAS ON GROUND WORKING ON THE DRAG LINE WHEN LIGHTNING STRUCK. EMPLOYEE HAD A BAR IN HIS HAD WORKING ON C HAIN THAT HOLDS BUCKET TO DRAG LINE. RIAN SHOWERS 10 MILES OUT NOT IN VICINITY OF PLANT.

USING GRINDER PICE OF METASL FLEW INTO EMPLOYEE'S EYE-EMPLOYEE WAS NOT USING SAFETY GLASSES.

EMPLOYEE LIFTING TANK FROM TRUCK BED PULLED MUSCLES

AS EMPLOYEE STE1PED OUT OF MACK DUMP TRUCK,SLIPPED AND CAUGHT HOLD OF HANDLE WITH ONE HAND.

WHILE LIFTING F9ICTION CLUTCH BAND ONTO DRAG LONE, EMPLOYEE WRENCHED BACK MUSCLES.

STEP BROKE ON S7RAPER PAN MACHINE CAUSING EMP TO F ALL

WHILE SWEEPING PLANT CLAIMANT DROPPED HIS HARD HAT HE PICKED UP HIS HAT AND A STONE DROPPED FROM SCREEN ABOVE AND HIT HIM ON THE HEAD INJURY REQUIRED HEAD X RAYS AND 5 STITCHES UNITED COMM HOSPITAL

PIECE OF RUST FLEW FROM THE SPRAY BAR INTO CLAINANTS EYE WENT TO UNITED COMM HOSPITAL TO HAVE PARTICLE REMOVED FROM EYE NO DAMAGE OR IMPAIRMENT INVOLVED

THE EMPLOYEE WAS STARTING THE ENGINE ON LORAIN DRAG LINE. THE HANDLE CRANK ON THE ENGINE FLEW UP AND STRUCK EMPLOYEE ON LEFT WRIST.

EMP WAS PLACING CEMENT BLOCKS UNDER A TRAILER AS EMPLOYEE WAS KNOCKING MORTAR FROM CEMENT BLOCKS A PIECE OF THE MORTAR FLEW INTO HIS RIGHT EYE
