Miner slipped while walking on snow/ice. Fell forward. Strained left wrist while bracing impact of fall. Ligament strain to left wrist/forearm.

Injured employee is a transport driver for McMurry Ready Mix Co. who was delivering material to the crusher site. Employee received site specific training upon arrival at site. Employee injured his back while unloading plate steel off trailer.

Miner was walking across ground and slipped on ice. Fell and struck back of head on ground. Sought professional medical care as a precautionary measure. No disabling injury. Miner cleared to return to work.

Miner was cutting v-belt with utility knife. The knife slipped and cut the miner's left forearm. Stitches were required. No lost time. Injury report was received in company safety office on 12/04/06.
Miner was postioning himself to perform maintenance and experienced chest pain and difficulty breathing. He was transpoted to local hospital where he was diagnosed with a strained chest muscle. No cardiac issues involved.
On 8/16/06, the injured miner was shoveling mud and rock beside the jaw crusher when the shovel struck a rock covered by mud. The impact of the shovel on the rock twisted his right wrist. The injured miner notified his supervisor on 8/23/06, notified the company safety director on 8/25/06, and submitted his inury report form to the company safety director on 8/28/06.
Miner was using pneumatic tools while performing post shift equipment maintenance. Dirt/debris was blown into miners right eye. Eye wash solution was used to flush the debris out of the eye. Eye was still irritated the following day which prompted the miner to see a doctor. An abrasion on the eyeball was diagnosed.
Descending stairs from button house and slipped off bottom stair. Impact injured left ankle. Notified supervisor on 6/22/06. Saw doctor on 7/5/06. Safety office receievd injury report on 7/11/06.
Employee stepped on a rock and rolled right ankle on 1/18. Notified Safety Director at that time and said it was not serious and continued working. Pain did not subside, so saw doctor on 1/30 which discoverd it was a serious ankle sprain. Notified Safety Director of doctor's findings on 1/30. Filled out injury report form on 2/1/06. No lost time as of 2/2/06.

While performing equipment maintenance, dirt/sand was stirred by wind and entered employee's right eye. The dirt/sand was able to get around the frame of his safety glasses.
Miner slipped when descending snow covered stairs while exiting crusher button house. Miner sustained a slight concussion and bruised right hand.

While performing maintenance on equipment in windy conditions, material (sand, dirt) was blown into ee's right eye. The ee finished his shift with minor discomfort. Returning home, the discomfort got worse. Went to E.R. on Saturday.
Employee was involved with setting up crusher for operation. While hanging electrical cords, his body movement while lifting pulled/strained his right hip (Dislocation).

EMPLOYEE WAS OPERATING DOZER PUSHING ROCKS TO PRIMARY JAW CRUSHER. DOZER WALKED UP ON ROCKS. THEROCKS BROKE BENEATH TRACKS, CAUSING A BACK INJURY TO THE OPERATOR WHEN HE WAS JARRED FROM THE FA LL OF THE DOZER(APPROX. 1 FOOT), WHEN THE DOZER HIT THE GROUND.
EE USING AHYDRAULIC PULLING DEVICE (JACK) TO ASSIST IN REMOVAL OF A BERING HOUSING. THE PULLING EYE BROKE AND HIT THE EE IN THE JAW, CAUSING A LACERATION. Y
