Employee was standing beside cone crusher after assisting to push some material through it with a frost bar. Another employee went around the crusher to manually open the cone further. Employee still by cone got right foot caught in pinch point. Employee suffered tendon damage and a broken heel as well as a laceration.

Employee was cutting rubber belting material with a razor blade knife. Employee was pulling knife towards self when it slipped out of the rubber material and EE hit EE's upper left leg with the blade. Employee received a laceration approximately 1 inch deep and 1 1/2 inches long. Employee received sutures to close the wound. Minimal impairment.

Employee was chipping hardened soil from the flange of a stacker inlet so that the flange could be changed. Employee was using a spud bar in a standing position above the inlet. On downward stroke spud bar hit rubber belting and bounced back up causing a small laceration above employees right eye. Employee was taken to medical clinic and received 7 sutures to close the laceration.

Employee and another employee were loading screen that were to be lifted up to the screen deck of a gravel crusher. While lifting the screen a sharp edge cut through the employees leather glove and left a laceration on left index finger on the second knuckle. Employee did receive medical treatment and four stitches to close the laceration.

A miner, engaged in crusher maintenance, was cutting a rubber conveyor belt with a razor knife in order to patch it. He was holding the knife in his right hand when the knife slipped out of the rubber material and back at him. The miner stabbed the palm of his left hand with the knife. Damage was limited to a small laceration.

A pieces of flattened expanded metal slipped out of the employees hand struck him in the leg puncturing his left thigh in two places which required stitches.

Cleaning off a metal plate without gloves on and cut hand.

Employee was running the dozer on grade, encountered ruts and jarred his back. Back became stiff over the weekend requiring employee to seek Chiropractic medical attention beginning 7/16/2007.

Very strong winds on that day, with gusts in excess of 60 mph. Mechanic was hooking up airlines on water tanker truck when the wind blew him off of the truck. Jarring back and neck when landing on ground. Back & neck where stiff, but didn't require immediate medical attention. Receiving Chiropractic treatments.

Employee was cutting drive blets on standard cone with a utility knife. He slipped and cut his right forearm. Cut required three stiches.

Employee strained his back. Employee was standing in chute, pulling on belting. He slipped on rock in chute and fell forward into front part of chute and twisted his back.

Employee was walking to car for lunch, scraper was headed to fill bin at same time. Both saw each other at same time. Both went right and then turned to left. Employee jumped away. Right front scraper tire ran over his right leg and left foot. Safety glasses broke, laceration on face. Bruising, swelling, abrasions, full extent of leg injuries not known at this time.

Employee was unloading metal screens from supplier's truck. He had his fingers through the openings in the screen when the supplier's employee let go of the screens. The screens slid down the truck pinching his finger and bending it sideways.

While using a sledge hammer employee missed the washer screw paddle. Sledge hit his right index finger smashing onto framework of wash plant. Fracturing right index finger.

WHILE MOVING &/OR BENDING TO PICK UP A LARGE ROCK OFF OF A CONVEYOR BELT, EE HEARD A POP IN HIS UPPER BACK. HE DID NOT ACTUALLY PICK UP THE ROCK HE WAS JUST MOVING/BENDING TO REACH FOR ROCK.

WHILE DISLODGING MATERIAL WITH A METAL TAMPING BAR, THE BAR SLIPPED AND HIT HIS FOOT CUTTING HIS LARGE TOE.

CHANGING GEAR BOX ON 10' TRANSFER BELT, THE GEAR BOX SMASHED HIS MIDDLE FINGER. HE PULLED BACK AND HIT HIS ELBOW ON A PIECE OF ANGLE IRON. AT THE TIME HE THOUGHT IT WAS NOT INJURED BAD ENOUGH TO REPORT AS WORK RELATED INNJUGY. HIS ELBOW CONTINUED TO HURT, HE SOUGHT MEDICAL ATTENTION IN APRIL. REPORTED TO EE AS A WORK RELATED INJURY 5-2-02. COMPLETED CO INJURY REPORT BUT DOES NOT WA

WHILE CHANGING SCREENS, EMPLOYEE WAS TIGHTENING A BOLT. THE RATCHET SLIPPED CAUSING HIS RIGHT ELBOW TO HIT 2 SMALL BOLTS BRUISING IT. AT E.R DOCTOR ADVISED WORK FOR ONE WEEK, IMMOBILIZED ELBOW & PUT ARM IN A SLING. TYLENOL AS NEEDED FOR PAINONE WEEK LATER HIS HAND WAS SHAKING SO INDEFINITE RETURN TO WORK DATE.

A FROZEN CHUNK OF GRAVEL FELL FROM TOP OF TRAP THE FROZEN CHUNK HIT EE IN BACK OF HEAD, KNOCKING HIM UNCONSCIOUS. THE FROZEN CHUNK FELL APPROX15 FT EE RECOVERED CONSCIOUNESS QUICLY REFUSED IMMEDIATE MED ATTENTION BUT DID GO HOM WHERE HE LATER DECIDED TO SEEK MED ATTENTION SLIGHT CONCUSSION

AFTER FIRST START UP THE TRAP PLUGGED AND THEY HAD TO SHUT DOWN, CLEAN OUT AND RESTART. UPON RESTART THE ALARM WAS SOUNDED AND A CHECK FOR ALL CLEAR SIGNAL WAS DONE. THERE WAS A MISCOMMUNICATI ON RECEIVING ALL CLEAR. EE WAS STANDING ON BELT WHEN RESTARTED. HE LOST FOOTING AND FELL TO THE GROUND. LANDING ON LARGE ROCK JUST REMOVED ON THE BACK OF HIS HEAD, NECK, SHOULDER AND LEFT SID

WHILE CHANGING CONVEYOR BELTING, BELTING FELL APPROX. 6 FEET LANDING ON RIGHT SHIN AND ANKLE.

HE WAS BARIN GOUT CLAY MATERIAL FROM CONE CRUSHER WHEN BAR GOT CAUGHT SLAMMING HIS RIGHT HAND AGAINST CONE BASKE.T SMASHED RIGHT HAND BY POINTER FINGER KNUCKLE, LACERATING HAND. REQUIRING ME IDCAL TREATMENT APPLYING STAPLE SUTURES TO HAND AND BANDAGES.

HE WAS DOING GENERAL LABOR DUTIES AROUND THE CRUSHER WHEN THE WIND BLEW DIRT AND METAL SHAVING IN HIS EYE.

I NAME WAS CLEANING AROUND A CONVEYOR BELT WHEN HE NOTICED AN OVERSIZED ROCK WAS CAUGHT BETWEEN THE BELTING AND FRAMEWORK. HE THEN DECIDED TO TRY TO REMOVE THE ROCK WITHOUT SHUTTING OFF THE BE LT.
