Deep Mine No 7 Coal

Employee stated that while installing roof support a piece of rock fell from the mine roof striking him on the head.

Employee was shoveling #2 conveyor head and strain his back.

Employee stated that while cutting a piece of suction hose, the knife slipped and he cut his thumb.

EE was on a ladder moving wood, wood got caught on the ladder slipped and fell from ladder.

Putting insert in miner and a piece of hammer broke off and went into knuckle on right hand.

An unintentional roof fall occurred in the No. 1 entry (return) 5,300 feet outby the section. The fall measured 30' long, 20' wide and 4.5' high. The airway and travel of men was not affected.

EE stated that while installing roof support, the drill steel flew out and hit his hand.

Employee stated that while loading supplies, he lifted some straps and felt pain in his back.

While shoveling around the No.3 tailpiece a piece of coal bounced off the belt, striking him on the forehead.

Was loading structure and dropped it on right hand.

Walking down entry hit his head on a curtain board in a low spot!

Employee stated that while operating the shuttle car, he hit a hole in the roadway and bumped his head against the canopy.

An unintentional roof fall occurred in the no 5 entry at spad no. 3248. The fall measured 20'long, 20'wide and 4'thick. The fall does not affect the airflow on the travel of mine.

Employee stated that while walking out of the change room trailer, a gust of wind hit the door causing it to close hitting him in the chest.

EE stated that while operating a red ride, he ran it into a table located outby the face area, injuring his hand (started missing work on 4/5/11).

EE was on a ladder while building a stopping. The stopping started to fall, so he jumped off the ladder and landed on the ground straining his lower back.

An unintentional roof fall occurred in the No. 5 entry, one break inby spad No. 7797 on the no. 1 section. The fall measured 35' long, 18' wide and 6' high. The fall did not affect the air flow or the travel of men.

EE stated that while working on the belts, he stepped in mud and twisted his leg.

An unintetional roof fall occurred in the no. 7 entry two breaks inby spad no. 7154. The fall measured 75' long, 19.5' wide and 4' to 6' thick. The travel of men and airways were not effected.

While loading belt structure, twisted around and twisted his knee.

Employee stated that when the roof bolter operator took his cable up, it struck him, knocking him down.

EE stated that he bumped his knee against the scoop while working on it.

While operating the Cont. Miner, a piece of rock fell from between the bolts striking the EE on the head and feet.

EE stated that while pulling a miner cable he slipped and twisted his knee.

While striking a belt knocker with a hammer, a piece of the hammer chipped off striking the employee in the left wrist.

A roof fall occurred on the no. 1 section in the no. 1 heading. The fall measured 20' wide, 20' long & 6.5' thick. The fall has been breakered off and mining is no longer being done in this area.

Driver stated while going down hill, brakes were good, then half way down air pressure dropped, engine started to overcue and he drove it into the ditch.

An unintentional roof fall occurred on the #1 section one break above spad #5362 in the crosscut between #2 and #3 entries. The fall measured 50' long, 19' wide and 0' to 8' thick. The fall does not affect the air flow or the travel of men.

Employee stated that when he threw a chain across a set of batteries to secure them on a truck, he felt a sharp pain in his back.

EE stated that while walking toward the continuous miner, a piece of rock fell from the mine roof, striking him on his neck and shoulders.

An unintentional roof fall occurred 5,500' outby the working section in the no.2 return entry, one break inby spad m.2555. The fall measured 180' long, 20' wide and 7' thick. The fall does not affect the travel of men or the air flow.

Operator was pinning top pulled wrong lever and let pot on bolter down and his rt shoulder

Laying track lifted rail & strain neck & lower back. Did not stop work until 6/1/07. Reported injury to companyon 6/6/07.

Low top pinner got hung. Operator tried moving pinner. While pulling with scoop, pinner slid sideways fowling him against the control inside deck.

Employee stated that when he started to pick the pinner cable up to move it, the operator took the reel up and the cable struck him in the face.

An unintentional roof fall occurred 700' outby the section in the No. 2 entry one break to the left of spad No. 5319. The fall measured 110' long, 19' wide and 0' to 8' thick. The fall does not affect the air flow or the travel of men.

Employee got off golf cart and hit head and neck on fire valve. Felt pain in neck and left arm. Employee started missing work on 3/31/07 due to this injury. It was not reported to management until April 13, 2007.

EE stated that while walking down an incline, near mine office, he slipped on some gravel and twisted his left knee.

Working on #4 belt head bearing, cleaning off, cut right thumb on shaft; had to have 6 stitches.

Bolting top stepped on a slick rock and twisted ankle.

Turned on golf cart, hit a rock and went under belt.

Employee stated that while making a belt splice a chip of metal broke off from mailboard and struck him in the corner of left eye. Employee was wearing safety glasses.

EE stated that while walking across the section he stepped on a rock and slipped and fell to the ground injuring his ribs and rght side.

An unintentional roof fall occurred 4, 200'outby the section in the no.3 entry at spad #3542. The fall measured 20'long x 20'wide x 6'thick. This area was developed 1/15/05.

Employee stated that while receiving belt structure, he dropped a piece of the structure on his right foot.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE NO. 1 SECTION, 180'FROM THE FACE IN THE NO.1 ENTRY (RETURN). THE FALL MEASURED APPROX. 30'LONG X 20'WIDE & 4 1/2'- 6 1/2' HIGH. THE FALL DID NOT AFFECT AIR FLOW ON THE PASSAGE OF MEN. THIS AREA WAS DEVELOPED 4/26/06.

An unintentional roof fall occurred in the #8 return air entry 2, 900' outby the active section at spad # 3485. The fall did not affect passage of men or air flow. The fall measured 25' long, 20'wide and 4'-6' high.

An unintentional roof fall occured in the intake airway No. 7 entry 300' outby the section. The fall did not impair ventilation or impede passage The fall measured 20' in L 20' in W and 4' to 6in high.

An unintentional roof fall occurred in the #9 return entry 240' outby the face. The fall measured 25'long, 20'wide and 4'-6'thick.

An unintentional roof fall occurred 120' outby section in #9 entry of old panel. The fall measured 140'long, 20'wide and 4'-6'thick.

EMPLOYEE STATED THAT WHILE OPERATING SHUTTLE CAR A PIECE OF ROCK FELL FROM THE MINE ROOF STRIKING HIM ON THE LEFT HAND.

Employee stated that he injured his back while lifting a miner cable (started missing work on 11/15/05).

Employee stated that a piece of rock fell from the mine roof striking him on the right shoulder. He started missing work due to this injury on 10-31-05.

Section foreman was standing in an entry when a #10SC turned and the cable struck him under the chin causing him to lose consciousness and giving him a severely sore jaw and dizzy head.

An unintentional roof fell occurred in the no.4 Track entry 2200 outby the no. 1 section at spad no. 3767. The fall measured 20'l x 25' w x 4' to 6' high.

EMPLOYEE STATED THAT WHILE WORKING ON THE CONTINUOUS MINER A PIECE OF DRAW ROCK FELL FROM THE MINE ROOF STRIKING HIM ON THE LEFT ARM

AN UNINTENTIONAL ROOF FALL OCCURED 1600' OUTBY THE SECTION IN THE NO.8 ENTRY AT SPAD # 3548. THE FALL MEASURED 300' LONG, 19' WIDE & 6-7' THICK. THE AIRWAY OR TRAVEL WAS NOT AFFECTED BY THE FALL.

While playing around with EE and mine foreman grabbed him around the neck and they fell into a refrigerator in the mine office injury his head and neck.

Employee stated that while bolting top a piece of rock fell from the mine roof. He started to run and he bumped his knee on the roof bolting machine.

Employee was a passenger in a Chevy Blazer, when the Blazer left the mine site and onto the portal road the driver noticed two coal trucks parked in the middle of the road, the driver could not stop in time and hit one of the parked coal trucks.

EE stated that while tramming the miner to set up in eight right the miner was crossing over a hump in the roadway, after topping over the hump the boom end of the miner swung to the left striking the EE on the right shoulder. (The boom on the miner was swung to the left while tramming.)

Employee states that while changing bits on the miner he tripped over a bucket of bits that were setting on the mine floor.

An unintentional roof fall occured in the intersection between no.7 & no.8 entries in the return airway 800' outby the section located at spad no.2613. The fall measured 25'L x 25'W x 6-7' thick. The airway or travel is not affected by this fall.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #8 RETURN AIRWAY ENTRY, 7000' OUTBY THE ACTIVE WORKING SECTION LOCATED AT SPAD #0458. THE FALL MEASURED 20' LONG X 20' WIDE X 3-5' THICK. THE AIRWAY OR TRAVEL IS NOT AFFECTED BY THIS FALL. THERE ARE 2 RETURN AIRWAYS IN THIS AREA.

DRIVER STATED HE RAN OVER A ROCK WITH A 980G END LOADER & HURT HIS BACK. HE DID NOT REPORT THIS INCIDENT UNTIL 11/05/04.

A 3 WHEELER RIDE RAN INTO EE'S FOOT, ANKLE STRIKING HIM ON THE ANKLE CAUSING IT TO BREAK

Cutting belt with utility knife blade broke cut right hand had to have 3 sitches.

EE STATED HE DEVELOPED CARPAL TUNNEL SYNDROME IN BOTH WRISTS (EE CONT'D WORKING, STARTED MISSING WORK 2/22/05)

Employee stated that while backing the roof bolter out of the No. 1 entry, he slipped on a rock and fell, landing on his right knee.

EE STATED THAT HE TRIPPED OVER THE SHUTTLE CAR CABLE & INJURED HIS LEFT LEG

While operating the supply motor on the main line track, water dripped from the mine roof striking the ee in the right eye.

CARRYING GLUE AND STEPPED ON A SLICK ROCK & FELL. HURT LOWER BACK & RT. KNEE.

Employee stated that while shoveling around. The tail piece he struck an anchor pin with his right knee.

THE ROOF BOLTER WAS STUCK IN THE ROADWAY. WHEN THE SCOOP OPERATOR PUSHED ON THE BACK OF THE BOLTER TO STRAIGHTEN IT OUT, THE FRONT OF THE BOLTER SLID INTO EE WHO HAD WALKED TO THE FRONT SIDE OF THE BOLTER FOULING HIM AGAINST THE RIB.

WHILE WORKING ON A TAILPIECE USING A SLATE BAR, THE BAR SLIPPED AND JAMMED THE LITTLE FINGER BETWEEN THE BAR AND THE STRUCTURE.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #4 ENTRY, 3,500' OUTBY THE WORKING FACE. THE FALL MEASURED 20'LONG, 20'WIDE & 4'-6'HIGH.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO. 8 RETURN ENTRY 3,200' OUTBY THE SECTION. THE FALL MEASURED 20' LONG, 20' WIDE AND 4' TO 6' THICK.

EMPLOYEE STATED THAT WHILE WALKING IN THE RIGHT RETURN AIRWAY, HE SLIPPED AND FELL INJURING HIS RIGHT KNEE. THE EMPLOYEE CONTINUED TO WORK UNTIL6-11-03 AT WHICH TIME HE WENT TO THE DR. RETURNE D TO WORK 6-12-03. NO LOST DAYS.

THE EE STATED THAT WHILE OPERATING THE SHUTTLE CAR HE HIT A ROUGH PLACE IN THE ROADWAY INJURING HIS LOWER BACK.

WAS GETTING OFF OF LOADER WHEN HIS FOOT SLIPPED AND HE FELL.

EE STATED THAT WHILE WORKING ON A WATER LINE HE TURNED TO WALK AWAY AND FELT PAIN IN HIS LEFT KNEE.

EE STATED THAT WHILE USING A SLATE BAR TO CLEAR GOB FROM UNDERNEATH A SHUTTLE CAR HE FELT PAIN IN HIS LT. SHOULDER. STARTED MISSING 10/31/02.

EE WAS CARRYING A PIECE OF METAL 3'X3' TO A MANBUS, WHEN HE TRAVELED UNDER THE STACKER BELT HE FELT PAIN IN HIS BACK AND LEFT SHOULDER. STRATED MISSING WORK ON NOV 5, 2002

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO.6 ENTRY (INTAKE AIRWAY) 5000'OUTBY THE WORKING SECTION. THE FALL MEASURED 35'LONG X 18'WIDE X 4 & 1/2'THICK. THE INTAKE AIR COURSE WAS NOT EFFECTE D BY THIS FALL.

WHILE BOLTING THE TOP OF A PIECE OF ROCK FELL FROM BETWEEN THE BOLTS STRIKING THE EMPLOYEE ON THE RIGHT SHOULDER.

WHILE CHANGING WATER SPRAYS ON THE MINER THE EE GOT HIS FINGER CAUGHT BETWEEN THE RATCHET HE WAS USING AND A BIT ON THE MINER.

X-CUT IN #8 ENTRY, 1 BRK OUTBY SPAD #422 ROOF FALL MEASURING 19'W X 20 L X 5 TO 6'H FALL NOT AFFECTING AIR COURSE OR INVOLVING INJURY.

VICTIM WAS TAKING TIRE OFF SHUTTLE CAR. TIRE FELL OVER KNOCKING HIM DOWN AND FALLING ON HIS LEG HC CONTINUED WORKING. BEGAN MISSING TWO DAYS LATER, INFORMED MANAGEMENT THAT HE WAS ILL WITH KIN DNEY STONES DID NOT TURN IN TO LOST TIME UNTIL 1-8-02

AN UNINTENTIONAL ROOF FALL OCCURRED 3000' OUT BY MMU 001-0 IN THE TRACK ENTRY. THE FALL MEASURED80' LONG 7'AND 20' WIDE.

DRAW ROCK FELL FROM THE MINE ROOF STRIKING THE EE ON BOTH ARMS.
