Miner's personal pocket knife came open in miner's pocket and tore through the material of miner's jeans exposing the blade. Upon sitting down the miner ran hand down leg contacting the exposed blade and cutting hand. 3 stitches were given by medical to close the wound.

Miner was climbing into service truck when they felt something shift in lower back. Miner requested medical attention and EMS was called. Miner was taken by ambulance to the ER where they were admitted for lower back strain.

Employee was installing return rollers when they felt a pop in right groin area. Miner reported it as a first aid only report at the time. On 03/24/2020 the miner requested a medical evaluation due to it not healing. EE was sent to urgent care where they were placed on restricted duty pending a specialist follow up for a hernia.

At the end of the miners shift on 9/12/2019 the miner felt EE had something in EE's right eye. The next morning 9/13/2019 at work EE's right eye was red and sore so EE requested medical evaluation. EE was sent to the doctors office where the doctor prescribed eye drops to be placed into EE's right eye.
Miner was climbing out of the cab on a 621 G water pull when EE missed one of the steps on the way down. This resulted in the miner falling from the equipment landing on EE's backside and causing a contusion to EE's mid and lower back. Miner was placed on restricted duty by the urgent care center.

Driver was cleaning side window did not have three points of contact when stepping off the semi. Twisted and broke right ankle on flat surface.
Miner was carrying a stack of five 12" x 12" screen panels weighing approx 25 lbs up the stairs to the screen deck. At the top of the stairs when the miner attempted to raise the panels above their head to place them onto the screen deck they felt a pop in the lower back. This resulted in a strain to the miners lower back.

Scale operator opened the scale window and dust blew into EE's left eye. Employee rubbed EE's eye to remove the dust which resulted in an eye irritation. On 5/3/2017 the employee called "Teledoc" and the doctor prescribed an eye drop antibiotic as a precaution. Employee did not see the doctor and needs no future medical treatment. Employee reported incident to SRMG on 5/3/2017
Miner was climbing into a IT-28 wheel loader and missed step. As employee started to fall they caught themselves with right arm and felt a pop straining right shoulder. No medical treatment was requested at this time and a report was made. On 4/11/17 miner requested to go to the doctors and per doctors request was placed on restricted duty on 4/11/2017.

The miner was trying to remove a return roller bracket, as EE was trying to loosen the bolt, the 3/4" deep socket slipped off the nut and hit EE in the forehead above EE's right eye resulting in a cut that required stitches.

EE was closing a water valve and the wrench slipped off the valve causing him to rest all his weight on his right rib cage resulting in a rib contusion. He was sent for x-rays Negative. Doctor ordered two days of restricted duty. EE was previously scheduled to be on PTO the end of this shift for the rest of the week. No missed scheduled days or restricted days worked.
Was preparing to repair Komatsu 750 tracks. Placing chock blocks in front/rear of the tracks. Discovered something unusual next day went to doctor. Possible hernia. An MRI ordered; employee immediately placed on "no lifting status." Not scheduled to work 11/13/15 (Fri normal day off). Reported to work on Monday. Allowed if he didn't lift anything.
Miner (A) was helping another miner (B) lift and position a 3/8"x50x40 piece of plate steel onto a cut table to make cuts on. Miner B felt his hand starting to get pinched so he let go of the metal plate. This resulted in the metal plate falling onto miner A's left ring finger resulting in a cut and fracture of the finger tip.

Good weather; Employee was standing on the back of his truck and went to step down from truck and apparently his shoe got caught on something causing him to fall to ground. He was not more than two feet above ground when he fell. He claims that he injured his shoulder, arm and knee.
Apparently, employee was replacing hoses on truck and was positioned in an awkward position and sprained his left shoulder.
While removing and replacing bucket tips on a 988K Cat loader miner was attempting to remove the retainer pins with a 3 lbs sledge hammer. Upon striking the pin it fractured a sliver off of the retainer and pierced the miners right knee cap. Upon a visit to the clinic it was discovered that the metal was still in the miners leg. Miner had to have surgery to remove the sliver.

While working on Stacker # 21-117 and trying to telescope in the tail section you must remove the (2) locks on the idler pulley sections to fold them over. Employee removed the first lock when one of the idlers sections unexpectedly folded over and contacted his left arm resulting in a bruised arm.

While walking up stairs carrying a shaker screen he felt a pop in his right big toe. Worker notified supervisor of the situation and an injury/incident report was filed. The employee later went to the doctors office and was given a full release to return to work. On 06/01/2011 employee was referred back to the doctor and was placed on restricted duty.

He was adjusting the clam shell in the tunnel when suddenly the turn buckle flew apart and hit him on the right wrist.
While cleaning under the belt a rock bounced of the conveyor and struck him on the wrist.
He was carrying a wheel barrow filled with rocks and it shifted to one side and as he tried to keep it from falling over he injuried his back.
While walking up the stairs he struck his left hand against the handrail.

Employee slipped on a board while pushing a wheelbarrow full of concrete.
He strained his lower back while removing a drain plug on the loader transmission.
While the employee was shoveling she pulled her left hand finger back. She did not report this injury until the following day on 10/3/07.
While removing the toggle seat on a jaw crusher the seat moved out of its position and hit him in the head.
He was lifting a pipe and it dropped on his finger.
On 4/12 he strained his back while installing a battery in a truck, but did not feel the need to seek medical attention. Then on 4/17 while flipping corner bits on a dozer blade he felt a pop in his back.
Missed the step and his right leg went in between the steps causing him to fall backwards putting pressure on his leg.
While installing screens, he smashed his finger between the screen and the handrail as the screens were lifted up to the top deck.
Changing oil on asset #12-700 forklift when he was bit by an insect or spider twice.
He was cutting the screen bolts off and when he shifted to cut the next bolt the torch tip got caught in his glove.

While watching over his crew perform various welding, cutting, and using the plasma ark through out the day, he injured his eyes.
Cleaning the grizzly bars with a sledge hammer, the following day he felt pain on his lower back.
HE WAS WRITTING ON THE BOARD OUTSIDE AND DROPPED THE MARKER, HE BENT DOWN TO PICK IT UP AND AS HE STOOD UP HE HIT HIS HEAD ON THE CORNER OF THE BOARD.
Was welding he got flash burns.
The employee was marking the inside of a manifold cup plate, when lifting the plate the eye broke and as it was moved the plate struck the employee on the top of his hard hat.
He was pouring concrete and suddenly felt pain in his left knee.
He placed flashing on his leg and proceeded to cut through it. The knife slipped and he cut his leg.
While putting a nut on an eye bolt the pry bar slipped and the metal plate fell on his finger.
A ROCK APPROX. 6" IN SIZE FELL FROM 10' OFF THE ROCK CRUSHER AND HIT THE EMPLOYEE ON THE BACK.
While cleaning under the jaw conveyor he jammed his right thumb against the side of the crusher frame. This employee did not report his injury until March 31.
He was shoveling material at the wash plant when he felt pain in his forearm and his fingers went numb.
He was shoveling material when a digging bar fell over and hit him on the wrist.

He was walking away from the trash can at the wash plant after throwing away some trash and slipped on the mud falling on his right hand.
He was removing material from the tail pulley by throwing it over the belt and after two hours felt pain in his lower back.
He tried to keep the wheel barrow from tipping over and strained his lower back.
He was checking the under cone, and then jumped down off the conveyor belt which was 3 feet on to a dirt pile causing injury to his neck.
Employee was installing a screen when it jammed up. He used a bar to get it loose and the bar slipped, causing him to hit his hand on the screen.
While hooking up steam cleaner to hitch of truck. Steam cleaner rolled backward and hitch on steam cleaner struck hand.
He lifted a wheel barrel full of sand, which put most of the pressure on his R-foot and the ground was unstable causing injury to his foot. Employee didn't report his injury until 6/24/05
A co-worker threw a fire cracker into his work area.
As he was pulling on a pry bar at the welding shop table the bar slipped causing him to fall.
As he was drilling a hole in a belt, the drill slipped and went into his finger.
He was holding a piece of wood and as he proceeded to shoot a nail through the wood he missed and the nail went through his thumb.

As he was cutting belting in an upward motion with a razor he cut his left hand thumb.
As he eas climbing off a conveyor his thumb caught a sliver on the side of the hopper.
AS EE WAS MOVING THE GATE FROM LEFT TO RIGHT, HE GOT HIS HAND BETWEEN THE MOVING PARTS, SMASHING HIS FINGER.
WALKING THRU THE MECHANIC SHOP SHE TRIPPED AND LOST HER FOOTING DUE TO A DAMAGED FLOOR CAUSING HER TO FALL ON HER RIGHT WRIST.
AS HE WAS ASSISTING THE MECHANIC REMOVE THE BELLY PAN FROM A LOADER, THE PAN SLIPPED SMASHING HIS HAND.
AS HE WAS ATTEMPTING TO LOWER THE BELLY PAN OF A LOADER, THE PAN SLIPPED AND FELL ON TOP OF HIS KNEE.

AS HE WAS TURNING THE MOTOR PULLEY TO MOVE THE V-BELT OVER ONTO THE PULLEY, HIS THUMB GOT CAUGHTUNDER THE V-BELT AND WENT INTO THE PULLEY.
HE WAS HAMMERING THE CHUTE AND ACCIDENTLY SMASHED HIS FINGER.
EE FILLED A WHEELBARROW WITH MATERIAL AND THEN TRIED TO LIFT IT AND HE SUDDENLY FELT A SHARP PAIN IN HIS LOWER BACK.
AS HE WAS PULLING ON A WRENCH HE FELT A SHARP PAIN WHICH FELT LIKE TEARING NEAR THE END OF HIS MIDDLE FINGER ON HIS LEFT HAND.
AS HE WAS WALKING DOWN A LADDER HE MISSED THE LAST STEP DOWN AND LANDED HARD ON ONE LEG BUT DID NOT FALL. HIS KNEE GUST BENT BACKWARDS AS HIS FOOT TOURCHED THE GROUND.
EE WAS CUTTING REBAR & A PIECE SPRANG BACK & HITTHE TORCH. THIS CAUSED THE TORCH TO TURN & HIT EE ON THE NECK CAUSING 2ND DEG BURNS
HE WAS RUNNING THE LOADER, MOVING MUD, BUILDING UP THE DIKE ON THE POND, RUNNING OVER ROUGH TERRAIN BOUNCING & JARRING HIS BACK.
AS HE WENT TO REMOVE A BOLT WITH A RACHET 6 POINT SOCKET WRENCH, HE PUSHED UPWARDS AND THE BOLT ROUNDED OUT CAUSING HIM TO HIT HIS HAND ON A PIECE OF METAL WHICH CAUSED A DEEP WOUND ON HIS RIG HT HAND.
AS SHE WALKED UP TO THE TIME CLOCK THE TOE OF HER BOOT CAUGHT ON THE EDGE OF THE SIDEWALK CAUSING HER TO FALL AND INJURE HER SHOULDER.
LIFTING A CASE OF BOTTLED WATER HE FELT A PULLING ON THE BACK OF HIS LEFT LEG.
AFTER WASHING HER LOADER AT THE WASH RACK SHE WAS GETTING BACK ON THE LOADER SHE PUT HER FOOT'ON THE STEP AND SHE SLIPPED OFF THE LOADER AND SHE HIT THE CONCRETE ON THE GROUND
AS HE WAS CHECKING THE OIL ON THE EQUIPMENT HE CAME TOO CLOSE TO THE HOOD HITTING IT, AND FELL ON HIS LEFT HAND.
LIFTING A HEAVY BOOK OF A TOP SHELF & TWISTED THE WRONG WAY. THEN SAT DOWN & WHEN SHE WENT TO GET UP SUDDENLY FELT A SHARP PAIN IN THE CHEST.
HE WAS REBUILDING DIFF, TORQUEING BOLT, AND HE SLIPPED AND FELL ON HIS KNEES.EMPLOYER WAS NOT AWARE OF ANY LOST TIME UNTIL JULY 2, 2003.
AS HE PULLED HIMSELF OUT FROM UNDERNEATH THE TRUCK HE WAS WORKING ON HE STRAINED HIS LOWER BACK.
A GUST OF WIND BLEW DUST INTO HIS EYES.
EE WAS WALKING AROUND THE TRACKHOE & STEPPED INTO A HOLE FALLING TO THE GROUND CAUSING HIS ANKLE TO TWIST.
AS HE STEPPED OFF THE COMPANY VEHICLE HE STEPPED ONTO UNEVEN GROUND AND TWISTED BOTH OF HIS KNEES.
AS HE LIFTED A WHEEL BARREL FULL OF MATERIAL HE PLACED ALL HIS WEIGHT ON HIS RIGHT ANKLE PUTING EXCESS STRAIN ON IT.
HE WAS WELDING AND GRINDING BOLTS TO INSTALL NEW PULLEYS FOR THE HOPPER BOOT. A PIECE OF METAL FLEW INTO THE EE'S RIGHT EYE.

THE CLAM SHELL AT THE BAR MAC WAS PLUGGED & HE PROCEEDED TO HAMMER ON THE SIDE TO UNPLUG & AS HE SWUNG THE HAMMER HE STRUCK A SHARP PIECE OF METAL CUTTING HIS FINGER.
HE SAID HE DOES NOT KNOW WHAT HAPPENED. HE SAID AS HE WENT TO TAKE A STEP ON CEMENT STEP HE FELL. HE DOESN'T KNOW IF HE MISSED THE STEP OR HIS LEGS FOLDED UP ON HIM.
WHILE HE WAS WELDING AND CHIPPING AT SLAG WITH A HAMMER A SMALL PIECE FLEW INTO HIS LEFT EYE.
AS HE SQUATTED DOWN TO SPRAY PAINT THE SHOOT TRAP DOOR AND ON THE WAY DOWN HE FELT A SHARP PAIN AND TUG ON HIS KNEE.
AS HE WAS CLEANING UP HIS WORK AREA HE WAS BENDING AND LIFTING PIECES OF METAL (REPETITIVELY)
AS HE WAS CLEANING THE CLAM SHELL ON THE FEEDER THE CONVEYOR BELT CAUGHT THE PRY BAR AND TWISTED IT OVER CATCHING HIS THUMB.

AFTER PARKING TRK #42-106 AT THE FUEL STATION HE CROSSED THE ROAD, TO GET TO HIS OFFICE WHEN HE WENT TO STEP UP FROM THE NORTH SIDE OF MEGA MIX/LUBE BLDG, THE NEXT THING HE REMEMBERS WAS ROLLING DOWN THE SMALL GRADE. WHILE OPERATING A SCRAPER HE WAS HOLDING THE STEERING WHEEL WITH HIS LEFT HAND. SUDDENLY HE HIT A BUYMP[ AND THE STEERING WHEEL PULLED SHARPLY TO THE LEFT CAUSING HIM TO STRIKE HIS WRIST AND E LBOW ON THE CAB DOOR.

HE WAS BENDING OVER TO GET THE LEG UP ON THE SILO, SUDDENLY HE FELT BURNING PAIN IN THE LOWER BACK AREA.
IT IS UNKNOWN HOW THE INJURY OCCURRED. HE SUDDENLY FELT HIS ARM COLD AND NUMB. THAT S WHEN HE SAW THAT HE WAS BLEEDING.
HE WAS RESEALING THE DRUM MOTOR. HE WAS USING HIS LEFT HAND TO BALANCE HIMSELF AS HE LEANED OVER TO GIVE DIRECTION TO LINE UP THE DRUM MOUNT. THE DRUM FELL OFF THE JACE AND HIS FINGER WAS SHEA RED OFF.
WHILE REMOVING A WING WALL FROM SAND SCREW, EMPLOYEE WAS IN A PINCH POINT AREA. WHEN WING WALL WAS CUT FREE FROM THE SAND SCREW IT SWUNG OUTON THE COME ALONGS THAT SUPPORTED IT AND PINCHED HIS LEFT FOREARM BETWEEN THE WING WALL AND SAND SCREW.
