EE was using a cat telehandler to move urethane sheets from the shop to chem ad, load shifted and EE stopped and was attempting to readjust the load when they heard a small pop in their right shoulder.

Employee was performing a king pin installation, had a shim get stuck and when they pulled on shim to remove, the shim broke and cut their left index finger which required 3 stitches.

EE was pushing a barrel on a roller cart when one of the wheels got caught on a rock/debris on the shop floor. This caused the barrel to begin to tip. EE reached out and caught it and in doing so deeply lacerated the tip of a finger on right hand on the underside of the barrel lip. EE needed five stitches and returned to duty the next day.
Employee 1 was working with employee 2 pulling wire through new conduit. The two employees took turns between pulling and feeding wire. Employee 1 eventually wrapped the mule tape around a piece of conduit to gain better leverage. After the work had been completed employee 1 told employee 2 that employee 1 believed employee 1 had tweaked back.

While removing gear box from shaft ee's thumb was pinched between lifting chain and gear box. EE sought medical treatment the following day (Saturday). Was released to full duty on Monday.

While maneuvering an electrical component employee felt discomfort in EE's back, continued to work the remainder of the shift. Diagnosed with back strain on 1/4 restricted from work until 1/16.

Miner was using a 30 ton ram and a 16 lb hammer to replace pins on a D10 undercarriage. While swinging the hammer to loosen the pins, ee felt some pain in back. EE reported this to employer, but didn't want medical care. Unbeknownst to the operator, employee sought chiropractic care. On 2/20/18 employee asked the operator to provide medical care and filed a claim.

Employee tripped over a box while preparing a package for shipment and fell into a cabinet receiving contusions to right forearm, elbow, and shoulder region. Initial treatment was diagnostic in nature only, returned to full duty. Employee received physical therapy for shoulder contusion on 12/14/2017. Injury became reportable on 12/14/2017.

An employee was installing a tensioner pulley on a D10N motor. The employee grabbed the tensioner pulley without realizing how sharp the outer edge of the pulley was. The employee was wearing latex gloves at the time of the incident.

Employee was using a 4" grinder with flapper wheel attached in a kneeling position. Upon completing his task effected employee released the trigger and started to stand up when the back of his right hand came in contact with the still rotating flapper wheel causing a laceration to the back of his hand requiring 4 stitches.
Effected employee was handing a return roller guard up to a co-worker; the guard slipped out of the co-worker's hand and fell striking the effected employee on the hard hat and shoulder. The force of impact caused the employee to bite down breaking 1 front tooth and chipping the other front tooth, he also sprained his neck and bruised his right shoulder.

Employee was replacing on a brake can; while caging the spring the can slipped and the employee's right thumb was pinched/cut requiring 9 stitches.

Employee slipped on dock catwalk, fell and sprained left knee. It was raining at the time and that section of catwalk was slippery due to the weather conditions and some algea growth.
Affected employee was operating a dozer during his shift. He complained of back pain but indicated that no particular incident had occurred causing this pain but thought it was from operating the dozer. The employee was taken for medical observation where it was determined that he had a strain to the lower back. He was placed on modified duty.

While working on a conveyor belt repair the employee was using a utility knife, the knife slipped and the employee received a cut to his hand. The cut required 5 stitches.
foreign object in eye, employee used eye wash to flush his eye, thought he had rinsed it out. The next morning eye was irritated so he was sent to the clinic where they removed a steel sliver from the employee's left eye.

EE WAS REPAIRING A SCREEN SPRAYBAR, NOTICED AN IRRITATION IN LEFT EYE. THOUGHT HE WAS FLASH BURNED IRRITATION DID NOT GO AWAY. WENT TO HOSPITAL ON 4-9-03 TO HAVE PARTICLE REMOVED.
EE WAS WORKING ON A GEAR BOX DRIVE FOR A WATER CLAIRIFIER. THE EE REMOVED A SECTION OF DECKING TO ALLOW ACCESS FOR OIL DRAIN BUCKET. EE STEPPEDINTO OPENING AND SPRAINED HIS RIGHT KNEE

EMPLOYEE WAS REPAIRING SCREEN DECK, FOOT CAUGHT IN DECK HIT RIB CAGE ON CROSS BEAM BEFORE HE CAUGHT HIMSELF.
LONG TERM CONDITION- POSSIBLE FROM WORKING ON KNEES.
EE WAS WORKING ON A LARGE PUMP REPAIRING LINING. LINER SLIPPED OUT OF HOUSING HITTING LEG, CAUSING FRACTURE.

EE WAS LYING ON HIS BACK LOOSENING BOLTS ON THE TANDEM AXLES OF A TRUCK. DUST/METAL PARTICLES FELL INTO HIS EYE.
WHILE REMOVING BOLT FROM HOPPER WITH RIGHT HAND LEFT HAND WAS UNDER HOPPER. WHEN BOLT WAS REMOVED HOPPER FELL CUTTING FINGERS ON LEFT HAND.
WHILE CLEARING OVERSIZE ROCK FROM SCALPING BURRS ROCK ROLLED ON HAND.
WHILE EXITING CAT D-400 HAUL TRUCK EE S RIGHT MIDDLE FINGER WAS SMASHED IN DOOR JAM.

WHILE RUNNING A POWER GRINDER IN THE SHOP, HE GOT SOME METAL IN HIS LEFT EYE. HE WAS WEARING A WELDING HOOD.
HE WAS HOSING UNDER A TAIL PULLEY WHEN WATER OVERFLOWED THE SAND TANK. THE WATER HIT HIM FROM ABOVE. WHEN HE STOOD UP HE HIT HIS HEAD ON A PIPE.
WHILE TRYING TO CLEAR 7" FEEDER, A LARGE ROCK FELL HITTING HIM IN RT KNEE. EE SOUGHT FIRST AID. ON FIRST VISIT, ON 9-3-99 HE REVISITED BECAUSE OF PAIN AND WAS TOLD THE KNEE WOULD HAVE TO BE SC OPED AT A LATER DATE.
PIECES OF STEEL SLIPPED AND SMASHED FINGERS.
