Employee was helping other employees reattach a bull hose to a pump using a crow's foot which became unlodeged letting the bull hose swing around & knock employee to the ground hurting his side, shoulder & back.

EE was climbing up on his boom truck to get blocks to crib up truck and he slipped and fell.

While operating Dozer, employee ran over rock with blade up and it threw him into door handle, where he hit his face. Contusion to nose & mouth.

Employee was pulling fuel hose out of truck & pulled something loose in Right Shoulder.

While loading coal trucks, employee backed loader his was driving into coal crusher.

Employee was climbing down off of Dozer equipment, slipped & fell down hitting his back on the push arm.

While walking to help with pump hose, employee's left foot slipped in mud causing his right foot and leg to fold under him breaking his right ankle and twisting his right knee.

While climbing up on equipment employee hurt his back.

Employee was steam cleaning truck # 212 while climbing front ladder on truck. He fell backwards approx. 4 feet landing on Tail & Back complaining of back pain.

Employee was climbing off motor grader & his foot slipped off step. He complained of back pain, but did not want to go to doctor at that time.

While replacing bolts in Air Compressor, employee grabbed hot exhaust, burning his left hand.

While loading pump hose into loader bucket, a rock fell on his right foot.

While pushing coal with Back Hoe, employee jarred his back, causing pain to radiate up to his neck.

While repairing the light on his hard hat, the utility knife the employee was using slipped causing deep laceration to his left middle finger.

While employee was providing service to Backhoe equipment, when he was climbing off, he slipped causing a contusion to his right knee.

Employee fell while pulling on hose at pump.

Employee was climbing onto Dozer, slipped & fell hitting right ankle.

While moving a pump hose, employee slipped & fell hitting his back against a rock on the ground.

Employee was riding in rock truck when rock truck slid down road & hit dozer on his side of the truck, causing injury to his Right Shoulder.

While Employee was moving a pump, the suction hose hung in rocks under pressure. The hose flipped back, hitting employee in the back & back of the Head.

Employee pulled ladder down on equipment in order to climb up. Ladder did not go all the way down, when employee attempted to go up ladder, his foot slipped and caused his Left Knee to hit ladder.

Employee was climbing off of equipment & fell off ladder landing to the ground. He complained of Lower hip & back pain.

Employee was sitting in rock truck cab while being loaded. Cab of truck shook causing employee to claim back pain.

Employee was turning on equipment, slipped & fell, hurting his Back & R.Shoulder.

Employee was closing lid on tool box when lid fell on his right thumb causing laceration.

Employee was climbing up on Dozier pulling himself with his arm when he felt a sudden pain in his left upper leg.

Employee was fueling equipment and smashed his right middle finger.

Severe jarring to lower back while pushing rock with motor grader.

Employee was in the process of changing the oil in a rock truck. He cut his finger on the oil filter packaging.

Employee was servicing equipment (dozer), tripped and fell eight feet.

Employee was driving a water truck. The truck turned over. Employee reported bruising to both arms and the middle of his back.

While walking to troop truck, employee slipped and fell, twisting his ankle.

Employee slipped and fell while walking to troop truck.

Employee fell off catwalk of hoe (hurt knee & elbow).

Employee was on a drill and was having trouble with the chain guide. His left third and fouth fingers were smashed between the chain and the chain guide.

Employee was placing diesel in his truck. He inserted the nozzle into the tank and turned on the switch. The nozzle popped out and sprayed into his right eye.

Employee was helping place an 8" pump suction hose in the water. He lifted it to his shoulder and felt a pain in his back.

Employee was placing a suction hose in the bucket of a loader. The hose slide back and struck the employee on the right knee.

EE WAS TRAVELING IN A WATER WAGON TOWARDS CARTER #2 MINE AND WAS TRYING TO SLOW DOWN. THE BRAKES LOCKEC UP AND THE TRUCK ROLLED OVER.

EE FELL OFF AN EXCAVATOR WHILE CLEANING THE WINDOW.

EE HURT KNEE CRAWLING OUT FROM UNDER A TRUCK.

EE STEPPED DOWN FROM A DOZER AND SPRAINED HIS LEFT ANKLE.

EE WAS LOADING WOOD BLOCKS. ON 6'6" FELL ON HIS LEFT FOOT/BIG TOE.

EE SLIPPED & FELL FROM A WATER TRUCK
