NO INJURIES OR EQUIPMENT INVOLVED.

WAS SAWING TIMBERS WHEN SAW JUMPED OFF OF TIMBER CUTTING HIS FINGER.
UNINTENTIONAL ROOF FALL. THERE WAS AN UNDTECTABLE FAULTY AREA IN THE ROOF.
OPENED AIRLOCK DOOR BEFORE OTHER DOOR WAS CLOSED AND IT KICKED BACK ON HAND.
SUPPLY CAR WAS WRECKED AND WAS PUTTING BACK ON TRACK AND MASHED FINGER. WAS BLOCKING CAR WITH HALF HEADERS AND MASHED FINGER BETWEEN HALF HEADER AND RAIL.
SAWING TIMBERS, SAW JUMPED CUTTING LEFT INDEX FINGER.
AT APPROX 5:00 PM ON 5-17-84 THERE WAS A PILLAR FALL ON 1ST RIGHT OFF 1ST LEFT OFF 1ST NORTH BACK. WATER BEGAN COMING IN FROM ABOVE FALL, AS FAR AS WE CAN TELL TO KNOWLEDGE, THE WATER WAS SLO W AT START, AND INCREASED GRADUALLY OVER THE NEXT (HOOT OWL) SHIFT. THE EVENING SHIFT CREW COMPLETED THEIR SHIFT. ALL WATER WITH THE EXCEPTION OF A FEW SWAGS IS RUNNING TO THE M OUTH OF SECTI
THE FALL WAS 50\ LONG 20\ WIDE & FROM 3\ TO 5\ HIGH. IT FELL ON THE INBY END OF THE STANDARD S/C, FROM END OUTBY THE CANOPY. DISTANCE OF 8\4". COULD SEE NO DAMAGE EXCEPT THE LEFT INBY WHEEL UN IT LOOKED AS THOUGH THE BOLTS MAY BE BROKEN OUT. THIS FALL WILL BE CLEANED UP AS THE SECTION IS BUTTED OFF.
LIFTING MINER CABLE OFF HOOK ON MINER AND HURT BACK THIS MAN HAS A HISTORY OF BACK PROBLEMS
FALL OF ROOF FROM FACE ON NO 3 ENTRY TO APPROX 60 FT OUTBY
MR COOPER WAS HOLDING UP SHUTLE CAR CABLE FOR UNATRAC TO GET UNDER IT OTHER CAR WHICH HAD BEEN BROKEN DOWN, STARTED UP AND CABLE PULLED HIM JUST ENOUGH TO PULL HIM INTO SHUTTLE CAR.
MR. PATTERSON PULLED ON OIL BARREL AND IT WAS STUCK IN MUD AND HIS HANDS SLIPPED OFF OIL BARREL AND HE FELL ON BACK.
MOVING ROCK FROM ROADWAY TO SHUTTLE CAR. HE MOVED ROCK, BUT SAID HE DID NOT KNOW IF THIS IS WHAT CAUSED HIS PROBLEM. MR. STOVER HAS HAD A HISTORY OF BACK TROUBLE.

FOREMAN (JERRY SHOWALTER ) UPON ENTERING KIDD BRANCH SECTION NOTED TEO BLUE FLAMES EMERGING FROM THE MINE FLOOR THREE FEET INBY THE END OF THE SUPPLY TRACK WHERE METHANE HAD BEEN LIBERATING FO LIBERING FOR MONTHS IT IS SUSPECTED THAT THE LAST PEOPLE LEAVING THE SECTION IGNITED THE FIRE FROM AN ARC ORIGINATING FROM THE JEEP WHEELS AFTER THE JEEP HAD BEEN SITTING OVER THE METHANE FEE
WAS DRIVING TRACK SPIKE IN WOODEN TIE AND MISSED HITTING ON SPIKE AND SPIKE FLEW UP AND HIT OVER HIS EYE.
DRILL STEEL HUNT IN HOLE AND HE LAID HIS HAND ON DRILL HEAD TO CHECK SUCION AND DRILL STEEL FELL STRIKING HAND.
RUNNING BOLTER STARTED TO DRILL HOLE & WHEN STARTED ROTATION ON DRILL PIECES OF ROCK SLIPPED OFF & HIT HIM ON ARM.

MR GRAGG WAS MOVING TIMBER AND STRAINED HIS BACK. MR GRAGG DID NOT LOOSE ANY TIME UNTIL 10-27-83 DUE TO THIS INJURY

HE WAS LIFTING TIMBER OUT OF UN-A/TRAC AND HE LET TIMBER SLIP AND CAUGHT HIS FINGER AGAINST THE UN-A-TRAC
PIECE OF ROCK FELL OUT BETWEEN BOLTS HITTING HIM ON HIS LEFT FOOT
HE LEFT S/C AND RAN THROUGH BREAK-THRU AND FELL HURT HIS KNE E
HE WAS OPERATING THE MOTOR AND IT STARTED RUNNING AWAY ON WET, SLICK RAILS AND HE JUMPED OFF AND SLIPPED AND FELL. THE CORNER OF THE CAR HIT HIM AND SCRAPED HIS SIDE.
HE WAS GOING TO THE BATHHOUSE AND HE SAID THAT THE STEPS WERE WET AND HE SLIPPED AND FELL
HE WAS LOADING TIMBERS INTO A S/C & STRAINED HIS BACK.
HE WAS CHECKING A POT ON THE MINER AND A PIECE OF ROCK FELL AND HIT HIM ON HIS HAND
CARRYING S/C FLIGHTS FROM ELEVATOR TO YELLOW JEEP TWISTED ANKLE LEFT FOOT.
HE BENT OVER TO PICK UP A BUNDLE OF WEDGES AND HE FELT SOMETHING POP IN THE SMALL OF HIS BACK

CHANGING MOTOR AND CHAIN CAME LOOSE ON MOTOR AND DROPPED MOTOR ON HIS FOOT
