THE EMPLOYEE WAS USING AN AIRHOSE TO CLEAN A SMALL 4' X 4' X 5' SQUARE SURGE TANK AND AIRLOCK. THE AIR HOSE GOT CAUGHT IN THE MOVING AIRLOCK AND DREW HIS RIGHT HAND INTO IT PINCHING THE 3 MID DLE FINGER NOT THUMB OR INDEX.

WHILE PUSHING DOWN MATERIAL WITH A SMALL HOE HIS HANDS SLIPPED ON THE HANDLE AND HE FELL FORWARD APPROX 6" HITTING HIS HEAD ONA BOLT PROTRUDING FROM THE SIDE OF THE B-MILL ORE TANK USED TO SEC URE THE ACCESS HOLE.
