EE was loosening a cap to check oil on the Liebherr Drag Line when the crescent wrench slipped off and the hose clamp cut EE's left wrist. The cut was deep, EE received 6 stitches.

EE needed to put the headache ball up on truck number 43770. EE describes was trying to attach the headache ball to the truck and was struggling. EE made one last effort with all of EE's might and got it attached but when EE did this last effort is when EE felt the strain and pain in the left upper arm and shoulder.

EE was using a winch (model 492) to back off the gravity take up weight when the winch handle slipped out of EE's hand while changing hands. The winch handle then spun around and the handle hit just above EE's right wrist. The handle was free spinning once EE's hand slipped off of it. Right wrist has a fracture of the ulna bone. Injury not in a cast but splint.

Workers were welding a patch on a chute below a screen. A fire was noticed inside the chute. Fire extinguishers were unable to put out the fire and fresh water supply was inoperable at the time. The fire spread to the screen and conveyor components above it. The fire department was dispatched to put out the fire.

Employee slipped and fell on ice while opening the lower entrance gate

Employee was changing a motor on the 2000 gallon fuel tank. As he was climbing onto the tank, he put left foot on 1st step and as his weight went onto left foot he heard a pop in his left knee. *Employee worked full duty until surgery on 6-10-13*

Employee had chained down a John Deere 400 In Dump and then raised the bed in transport position. This caused the chains to stretch and the chain binder to break and hit his nose.

WORKING IN SAND TANK WHEN HE REACHED UP TO GRAB SAND FLIGHT & HIS LT. SHOULDER POPPED.

CHANGING A SPRAY BAR NOZZEL, HE WAS LYING OVER A TUBE PULLING ON A PIPE WRENCH WHEN HE PULLED A MUSCLE IN HIS UPPER STOMACH, LEFT SIDE.

INSTALLING A CHUTE LINER WHEN HE TWISTED HIS MIDDLE BACK.

CROSSING A FIELD CONVEYOR TO REPLACE A ROLLER WHEN HE SLIPPED ON ICE SNOW ON THE GROUND AND FELL TWISTING HIS LEFT ANKLE. HE RECEIVED MEDICAL TREATMENT, DIAGNOSED WITH LEFT ANKLE STRAIN AND GI VEN AN AIR SPLINT.

EE WAS PICKING UP WIRE CLOTH TO PLACE IN SCREEN CUTTING RIGHT HAND ON SHARP EDGE. REQUIRED STITCHES.

EMPLOYEE WAS PUTTING TABLE ROLLERS INTO NW DRAGLINE AND STRAINED HIS BACK.

EMPLOYEE WAS CLIMBING OUT OF A HOPPER, WHEN HE STEPPED FROM THE HOPPER TO THE SCAFFOLDING, HIS RIGHT KNEE MADE A POPPING SOUND. X-RAYS WERE TAKEN.

EMPLOYEE WAS GREASING THE TRACKS ON THE DRAGLINE AND WHEN HE BENT OVER HE PULLED MUSCLES IN HIS BACK.

LIFTING EMPTY CO 2 BOTTLE FELT PAIN IN BACK

EMPLOYEE WAS DESCENDING LADDER FROM FRONT END LOADER. FOOT SLIPPED AND LEFT LEG HIT SIDE OF LADDER.

RIGHT FRONT CABLE ON DREDGE HOOKED ON HEAD WIND, BLEW BACK OF DREDGE AND RELEASED, CABLE SNAPPED INTO THE LEFT LOWER LEG, KNOCKED OFF FEET.

WORK BOAT BUMPED DREDGE THE INJURED'S HAND WAS HOLDING THE EDGE OF THE WORK BOAT CAUSING THE 3RD FINGER ON HIS LEFT AHND TO BE BROKEN AND LACERATED.
