Freeze Fork Surface Mine Coal

EE was using a ratchet with chain attempting to re-buckle the track back together on an Ingersoll drill. While pulling back on the ratchet, the chain broke causing ee to fall backward striking his head on the catwalk of the frill. This caused his hard hat to fall off as he continued to fall striking his head on the bucket of an excavator that was parked close by.

Employee was attempting to dismount a Caterpillar 785 rock truck. While climbing down the ladder to the ground, his foot slipped between the steps causing him to lose his balance and fall to the ground.

Employee was on top of vehicle loading it, when driver pulled out from under bin. Employee went to hospital, was released with no restrictions. Employee still complained of pain, went to doctor and was told "No work until you revisit doctor", resulting in lost time.

Employee was pulling highline to get slack out of the line. Upon pulling the highline, employee felt a sharp pain in his lower back. MRI results were not read until 2/16/09 causing his first missed shift of work to be 2/17/09.

Employee was attempting to dismount a D11 Caterpillar dozer. He placed his foot on the step on top of the tilt jack and his foot slipped causing him to fall back onto the tracks of the dozer. Note: Employee was seen by an orthopedic specialist on January 6, 2009 who on that dat took him off of work and prescribed physical therapy.

EE was getting out of the back of the Western Star Lube Truck at the rear of the truck. EE's foot slipped, causing him to strike his right knee on the step.

Highwall miner operator was setting jacks on the launch to prepare to mine. The left rear tower jack broke causing the launch to drop suddenly. EE hit his forehead on the computer screen.

Employee was coming down ladder on bulk truck. Cut palm on protruding piece of metal.

EE was working on the back of the operator's cab of a 785 rock truck. He lost his footing and fell about 12 feet to the ground, landing on his feet.

Member was walking and stepped on a metal chute causing him to trip and cut his wrist on the metal.

Member was found in cab of 992C loader with a large laceration to the head. Due to memory loss by member and insufficient evidence, the cause of this accident is unknown. Assuming member was mounting loader and slipped, hitting head.

Employee was eating lunch in the cab of a bulk truck. He began choking on a bite of sandwich, passed out, hit head on door of cab as he fell out. Regained conciousness and was taken to local medical facility where he was treated for a light concussion. Released with no restrictions.

Member was dismounting a D-10R dozer when he slipped and fell.

Member was removing the pin from a hi-wall miner beam, when he turned to throw the pin he slipped and fell, landing on is hip and tailbone.

EE was cleaning out an axle housing on a 785B Catepillar Rock Truck with a cloth rag. When he wiped across the edge of the housing, the sharp edge cut through the rag and cut his right index finger.

Mechanic was working on D575 dozer when he was struck in the forehead, causing a small laceration.

Member was in the process of mounting his coal truck. When he grabbed the handrail to pull himself up, he stated he felt a "pop" in his right wrist.

EE was walking across the parking lot as another member was pulling into the parking lot, bumping ee with his personal vehicle. He did not begin missing work until 2/26/07.

Employee was operating a coal truck with the bed in the raised position. The coal truck overturned, landing on the left side as he was going around a right hand curve.

member was changing hydraulic filters on #rd907 drill. He was in a sitting position, leaning to his right. The filter housing came loose, and as he suddenly tried to catch it, he felt a sharp pain in the left side of his lower back.

EE was bringing truck off drill bench. He lost control of vehicle. Prior to unit rolling over the driver jumped out. Driver hit his head on rock He was transported to ER. Treated with 3 stitches and released. No lost time or restricted.

EMPLOYEE WAS HELPING TO CHANGE THE RT. FRONT SPRING ON #526 COAL TRUCK WHEN THE CRIB & THE JACK HOLDING THE TRUCK UP STARTED TO CREEL OVER. THE TRUCK FELL APPROX. 8", STRIKING HIM BELOW HIS RT. SHOULDER & THE RT. SIDE OF HIS BACK.

Member was walking toward fuel/oil containment area looking over his shoulder. He stepped in a hole in the containment, cutting his left leg.

Member was using (2) cresent wrenches to change a pressure gauge. One of the wrenches slipped off, striking him in the mouth, breaking his right front tooth.

EMPLOYEE FELL ON DRILL BENCH. HE STATED TO MANAGER THAT HE TRIPPED OVER HIS DOG THE NIGHT BEFORE AND TWISTED HIS KNEE CAUSING HIS LEG TO GIVE WAY RESULTING IN THE FALL. SAW DOCTOR, TREATED WITH MEDS AND GIVEN 2 WEEKS OFF. I REQUEST A REVIEW PRIOR TO REPORTING ON THIS INCIDENT

EMPLOYEE WAS EXITING EXPLOSIVE STORAGE MAGAZINE. HE HIT HIS HEAD ON TOP OF DOOR FRAME CAUSING PAIN TO HIS NECK, SHOULDER AND LEFT ARM

EMPLOYEE WAS WALKING AROUND THE REAR BUMPER ON A CAT 992G LOADER TO SHUT THE ENGINE COMPARTMENT DOOR. HE SLIPPED & FELL ABOUT 6' TO THE GROUND.

EE was pulling pins from beams at the highwall miner. In the process, his right arm received a laceration from a metal burr on the beam. He was taken by ambulance to Logan regional medical center where he received stitches.

EE was attempting to remove a shield off the miner, when the shield pinched his left pointer finger. he later received 5 stitches.

EE WAS TIGHTENING BOLTS IN A "DODGE" COUPLING ON THE MINER HEAD WITH A 1/2" RATCHET & A 3/4" DRIVE WRENCH. EE STATED THAT HE FELT A STRAIN IN HIS LOWER BACK WHILE PULLING ON THE WRENCHES.

EMPLOYEE WAS DISMOUNTING A FUEL TRUCK AT THE START OF THE SHIFT WHEN HE STATED THAT HE STEPPED ON THE GROUND AND FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE WAS SITTING INSIDE THE ROCK TRUCK WHILE THE LOADER OPERATOR WAS FILLING HIS BED WITH ROCKS. THE OPERATO PUT TOO LARGE A ROCK IN THE BED AND IT JOLTED THE TRUCK, HURTING HIS BACK.

WHILE DISMOUNTING 992C LOADER EE TURNED HIS RT. ANKLE AS HE MADE HIS FIRST STEP TO THE GROUND.

THE EE WAS TURNING MINER CUTTER HEAD IN REVERSE TO ALLIGN BITS FOR THE HIGHWALL MINER. AS EE WASTURNING HEAD HIS GLOVE ON HIS LT. HAND GOT CAUGHT ON BIT PULLING HIS HAND BETWEEN BIT AND FRAME.

EMPLOYEE WAS UNHOOKING CATHEAD FROM ADDCAR. PUSHER JACK CAUGHT HIS FOOT & PULLED IT INTO THE ADDCAR BRUISING FOOT.

EE WAS REPLACING A LIGHT BULB ON THE HIGHWALL MINER LAUNCH. HE HAD A BULB IN HIS LEFT HAND WHEN HE SLIPPED AND FELL BREAKING THE BULB, CAUSING A LACERATION TO HIS LEFT WRIST.

EMPLOYEE WAS PREPARING TO LOAD EXPLOSIVES INTO A DRILLED BLASTHOLE. THE OVERHEAD AUGER DESIGNED TO DELIVER BULK BLASTING AGENT INTO THE DRILL HOLE BROKE AT THE COLLAR CONNECTION. AUGER HOUSING STRUCK EE IN BACK OF RIGHT LEG.

DRIVER ALLEGED THAT HE STOPPED IN THE TRUCK PARKING LOT TO TIGHTEN HIS WHEEL ON THE FRONT OF HIS MACK 800 COAL TRUCK. HE ALLEGED WHILE PUSHING DOWN ON WRENCH, IT SLIPPED AND HE FELL BACKWARDS, TWISTING & FRACTURING HIS ANKLE AT THE JOINT.

WAS CONSTRUCTING A 12 KV POWER LINE, USING DOZER WINCH TO PULL POLES TO LOCATION. INJURED EMPLOYEE GOT LEFT HAND PULLED UP IN WINCH ROLLERS AT DRUM CAUSING INJURY TO LEFT THUMB.

MEMBER WAS CLEANING HIGHWALL WITH A 992 G LOADERTHE BUCKET BECAME FOULED & BROKE LOOSE CAUSING HIM TO JUMP IN THE SEAT. HE RECEIVED A BRUISED HIP.

EMPLOYEE WAS IN THE PROCESS OF REMOVING A ROCK STUCK IN THE INCLINE BELT ON THE REAR OF THE HIGHWALL MINER LAUNCH, THE REAR OF THE LAUNCH WAS RAISED OFF THE STACKER BELT ABOUT 6 FT WHILE STAND ING ON STACKER BELT BELT TAILPIECE TRYING TO FREE ROCK, ROCK FELL ON HIM ACROSS UPPER CHEST AND SHOULDERS.
