Employee was guiding and directing loader operator to move 150' Stacker, was watching loader and stepped on tin cover for conveyor that was on the ground and cover flipped up and struck his face.

Employee was trying to tighten hose fittings on backhoe tractor with crescent wrench. Wrench slipped off and struck him in face. Cut cheek.
Truck was travelling on downgrade travelway into mine.Operator lost control of speed and had to steer truck into safety berm to stop.Resulted in minor injury and damage to truck.Setbelts were worn.Contributing factors may have included;truck travelling too fast at approach to downgrade slope,proper gear not used,truck fully loaded,excessive use of air brakes.

CHANGING OUT TROUGH ROLLER, EE REACHED IN TO MOVE ROLLER AT THE SAME TIME EE WAS MAKING ADJUSTMENT WITH SLEDGE HAMMER.
TRUCK DRIVER WAS UNLOADING MATERIAL TO ADJUST HAUL WEIGHT AFTER DUMPING SOME MATERIAL OFF. DRIVER FOUND A ROCK STUCK BETWEEN THE TAILGATE & BED, SHE REACHED IN & PULLED ROCK OUT CAUSING THE TAILGATE TO CLOSE ONTO HER RIGHT HAND.
CRUSHER OPERATOR CLEANING UP SPILLAGE MATERIAL. HE TWISTED TO SIDE AND THREW BACK OUT OF PLACE.

LOADER TOOTH OF 988F BUCKET CAME OFF, WHICH WAS NOT DETECTED, WENT UP BELTS TO SURGE PILE, FROM SURGE PILE TO SCALPING SCREEN, WHERE IT ENTERED OUR 1352 NORDBURG OMNI CONE CRUSHER. THE CRUSHE R MOTION OF THE OMNI CONE AND THE TAPER OF THE TOOTH CAUSED IT TO EJECT FROM THE TOP OF THE CRUSHER, APPROX 15' IN HEIGHT. EMPLOYEE WAS NEAR PLANT AND WAS STRUCK BY BUCKET TOOTH IN LEFT LOWE
