Employee slipped and fell while walking on the ground. The ground was covered with ice and snow. The area where slip and fall occurred was snow covered and the surface of the ice was not exposed at the time of the accident. The employee broke his leg during the slip and fall accident.

Truck was travelling on downgrade travelway into mine.Operator lost control of speed and had to steer truck into safety berm to stop.Resulted in minor injury and damage to truck.Setbelts were worn.Contributing factors may have included;truck travelling too fast at approach to downgrade slope,proper gear not used,truck fully loaded,excessive use of air brakes.

THE CRUSHE R MOTION OF THE OMNI CONE AND THE TAPER OF THE TOOTH CAUSED IT TO EJECT FROM THE TOP OF THE CRUSHER, APPROX 15' IN HEIGHT. EMPLOYEE WAS NEAR PLANT AND WAS STRUCK BY BUCKET TOOTH IN LEFT LOWE
