Employee was dismounting machine, had both feet on the ground. When he turned to leave the machine his feet slipped on muddy ground. The slippage caused employee to fall to his knees causing a stress fractur in the lower leg.

Unloading trash/boxes from back of service truck into dumpster. Footing slipped & the employee fell from the truck. He hit the back of his head on the dumpster & his buttocks hit (dirt) the ground.

EMPLOYEE WAS ATTEMPTING TO CLIMB DOWN FROM A 5TH WHEEL UTILITY TRAILER APPROX. 3'FROM THE GROUND SURFACE. HE PLACED HIS LEFT FOOT IN THE APPROPRIATE STEP & THEN PLACED HIS RT. FOOT ON TOP OF A PIPE RESULTING IN HIS RT. FOOT SLIPPING OFF PIPE & EE FALLING THE REMAINING 12"TO THE GROUND.
While changing edge and trying to get bolts out with torch, the bolt snapped causing the edge to pivot and swing down, hitting his leg. Bruising and swelling right leg above the knee.
DOZER OPERATOR SUSTAINED A NECK STRAIN WHILE BACKING OVER A 2' LIFT. OPERATING IN REVERSE, THE D9R BACKED OVER A LIFT THEREBY JAARRING THE OPERATOR'S UPPER TORSO. EE INDICATED HE WAS EXPERIENCING DISCOMFORT AND WAS TRANSPORTED TO THE PHYSICIAN FOR EXAMINATION. DIAGNOSIS WAS A NECK STRAIN.

While mounting the D7 bulldozer, mobile equipment operator slipped from his hand hold (3-points of contact) and fell backwardard striking a HPDE pipe. He sustained a sprained lower back.
Mobile equipment operator Lee Lamb sustained a right ankle sprain when he fell from a water wagon while conducting water filling operations
Mining Unit Operator EE was walking in a crouched position and struck a pipe. This action caused EE to turn and slip thus sustaining a sprained ankle.
EE- Concentrator Operator/Electrician, was securing a Sodium Hydroxide pump. The discharge line of the pump seperated and sprayed line residual sodium hydroxide on the miners face and torso. Chemical shower flushed the sodium hydroxide from the miner. Sodium Hydoxide did drip into the miners right eye.
Scraper operator sustained upper back strain when his scraper passed over an irregular surface. This action resulted in Lonnie bouncing up and down in the cab thereby causing the upper back strain.
Employee lost consciousness due to heat stress. Oxygen was administered and he was transported to a local emergency room for observation.
Mobile equipment operator was shutting down a water pump on ground level. The operator slipped and fell into the frame of the pump and sustained a chest contusion.
DOZER OPERATOR BACKING OVER UNEVEN GROUND AT HIGH SPEED RAN OVER A TREE STUP & JOLTED THE DOZER. OPERATOR SUFFERED PAIN IN LOWER BACK ON THE FOLLOWING DAY. NO DAMAGE TO DOZER WAS SUSTAINED.
EE SUSTAINED A SPRAINED RIGHT ANKLE. INJURY OCCURRED WHILE THE EE WAS STEPPING ACROOSS 3 18" PIPES THE INJURY OCCURRED NORTH OF THE CONCENTRATOR, AFTER DUSK, PRIOR TO END OF SHIFT. EE STEPPED UP, CROSSED & WAS IN PROCESS OF STEPPING DOWN FROM THE PIPES. AS THE EE STEPPED DOWN FROM THE PIPES, HIS RIGHT ANKLE ROLLED OUTWARD FROM LANDING ON UNEVEN GROUND. EE REC'D
