Audiologist report of 4/4/2023 on annual hearing test indicated a reportable hearing loss of 32.33 dB in the Left Ear. Employee is currently enrolled in BMC Aggregates Hearing Conservation Program and will be retrained on hearing conservation program and proper use of hearing protection.

Employee was working on dredge. While walking on land near dredge, employee tripped over cable used to anchor dredge. Employee fell, landing on left knee causing injury to knee.

Employee was using utility knife to cut rubber hose in order to install new fitting on end of hose. In process of cutting hose, utility knife slipped, causing laceration/puncture wound approximately 1" long on the inside of right thigh about 2"-4" above knee. Wound required 5 stitches.

Was kneeling on knees pulling old belt out of bottom of leg #2 while the new belt was being pulled in above me. There was a piece of plywood that the new belt was laying on. When they pulled the new belt, the plywood slipped off of the beam it was resting on and hit him in the back of the head on his hard hat and snapped his head ahead.

A crane was holding a dredge impeller in an upright position with a nylon strap tied around the impeller. The crane operator put slack in the cable. The victim had his hand between the strap and the impeller. When the slack was taken up, his hand was pinched and pulled his finger out from between the strap and impeller.
