Employee twisted knee while fueling truck due to uneven ground. Was seen by physician on 5/5/22. Diagnosed with sprained knee. Employee was released to work with no restrictions and no medical treatment was given. On 5/23/22 employee was seen again by physician. Was placed on restricted work duty due to continued issues with knee. Injury became reportable as on 5/23/22.

Employee was repairing engine on secondary crusher. Upon exiting the working platform employee tripped over extension cord, causing employee to lose balance and fall down steps, landing on the ground at bottom of steps. Fall resulted in bruising to employee's right side/back and two cracked ribs on right side.
Employee was working on a shot bench and twisted their knee.

Employee was cleaning around crushing plant with the skid loader and hit a big bump causing employee's head to hit the roof of skid loader. Told supervisor 9/28/2017 what happened. Did not go to doctor because employee did not think it was a big deal until headache would not go away. 10/16/2017 went to the doctor.

Was pulling electric cords and hanging them in the rack on a bin. Thought that is was a pulled muscle. On 8/10 went to doctor and said do physical therapy. On 9/6 was placed on light duty because no progress. On 11/9 reached out of dump truck to shut lime bin off and got pain in shoulder. Had MRI same day and on 11/10 was diagnosed with left torn rotator cuff. Surgery 12/13.

Washing the plant shack with a power washer. Stepped backwards and tripped on the hose. Fell on right shoulder. Original injury report was 12/17/2015. The employee was under evaluation until it was determined on 02/01/2016 that the right rotator cup was torn and needed surgery. Thus needed to be reported as an accident.

He was removing a dump truck tire from the tire rack. While pulling the tire out of the rack he felt a pull in his left shoulder. This was last day of work week and was decided to see how shoulder felt the following week. On 8/5/14 he was sent to occupational health because shoulder was still bothering him. He was placed on light duty. 8/12/14 recheck sent to therapy.

Changing bucket tooth on Caterpillar 988F. After removing the pin that holds the tooth, he hit the edge of the tooth with a hammer to loosen the tooth from the adapter when a piece of metal chipped off. The piece of metal struck the right inner thigh of his right leg.
Kneeling on floor sliding crusher liners, when he felt a pop/sharp pain in his back.

APPERENTLY THE D RAIN BECAME SNAGGED IN THE CONVEYOR FRAME. THE STUCK CHAIN MOVED THE CONVEOR IN A WAY THAT MADE EE FALL TO THE GROUND WHEN HE PUT HIS ARMS OUT TO BREAK HIS FALL.
HE WAS PICKING UP HEAVY DECKS AND REMOVING THEM FROM A MACHINE. CHANGING SCREENS.
INSTALLING A BEARING IN A ROLLER. HAMMERING THE BEARING IN WITH A PIECE OF SHAFT WHEN HE MISSED AND HIT HIS FINGER.
