On 9/21/16, evidence suggests that the victim, an experienced mechanic, may have been attempting to remove or loosen the top drive power head, spindle cap on the drill machine to replace damaged seals by attempting to use a pipe wrench and hydraulics of the drill machine, which are controlled by levers in cab next to platform. Victim struck in torso by pipe wrench. No eyewitness.

Employee was picking up to 2 - 2.5 gallons of DEF when EE stated EE twisted back causing pain.
Removing a brass bushing off main shaft of crusher, bushing had oil on it. Another co-worker was helping lift bushing off but hand slipped causing bushing to fall and caught employee's fingers under bushing causing bruising to end of index, middle and ring fingers on right hand.

Customer came into Quarry to get load of stone. Driver backed truck up next to material, loader operator went to load truck, after they loaded truck they went to motion to driver that they were loaded and noticed that drivers head was laid back against cab of truck. Loader operator went to check on driver found them unresponsive, called 911, ambulance took driver to hospital.

Employee was changing a light bulb in the control panel at our pug mill plant. The bulb broke causing a laceration to right index finger tip. Laceration required stitches.

Employee was walking when his ankle rolled over causing him to fall. He sprained his right thumb and had abrasion to his right palm. Doctor put splint on thumb. Doctor's excuse 2 days no work. We could not get employee to come back to work on light duty, he stated he was quitting.

EE WAS HOLDING A BAR ON A SPACER ON A CENTER PIN IN A 980 LOADER WHILE ANOTHER EE WAS TAPPING ON THE BAR TO DETERMINE HOW TIGHT THE SPACER WAS WHEN A CHIP OF METAL FLEW OFF THE SPACER AND WENT IN EMLOYEE'S LEFT ARM JUST ABOVE THE WRIST. WAS INSTRUCTED NOT TO USE ARM FOR 2 DAYS. DECIDED TO LEAVE METAL IN, MAY HAVE TO TAKE IT OUT LATER DATE IF IT FLARES UP.

EE S TATES HE FELL THROUGH THE HANDRAIL, HANDRAIL HASA MID RAIL. EE FELL APPROX. 15'. HE HAS A CUT TO LEFT HAND WHICH NEEDED STITCHES, CUT ON BACK OF HEAD AND BROKEN RIBS.
EE WAS CLEANING UP AROUND PLANT WITH A SHOVEL. HE SLIPPED IN MUD AND FELL INTO HANDLE OF SHOVEL. THE HANDLE HIT HIS RIBS FRACTURING TWO RIBS.
