While replacing pivots in the scales, I was holding pivot in place adn began to hammer. Hand slipped forward and hit it with hammer in turn pushing hand into pivot. Pivot had been coated with antiseize and grease.

Injury was attempting to clear a jammed belt on conveyor C-16. While it was running a attempted to grab a rock from between the scraper and belt when his glove was caught by the belt and pulled his rt index finger between the belt and scraper and took the skin off the inside of his finger and blackened his nail. He had a remote to stop conveyor

Flipping blow bars in telesmith 5252 HSI impactor. While loosening one of the bars the wedge dropped and was caught causing the mechanics middle finger to get cut. He got 5 stitches.

The chute between conveyors C-1 and C-2 clogged from flat rocks. Two workers were cleaning the chute from the top. IE went to the discharge end of the chute and was using his feet to dislodge the flat rocks. The rocks came loose and hit his legs.

OPERATOR OF TRUCK #337 WAS FUELING HAUL TRUCK. SHE STEPPED FROM TRUCK CATWALK TO FUEL TANK CONTAINMENT WALL AND TWISTED HER RIGHT KNEE.
While removing a boom cylinder from a Cat 988B Company #210. The mechanic was driving the top pin out of the cylinder and boom arm. The cylinder caught the drift and pinched his left palm (under pinky finger) against the cylinder eye. He got 8 stitches and a tetanus shot.

EE WAS MOVING TIRES FROM STORAGE TO SERVICE TRUCK. HE FELL WHILE ROLLING TIRE OUT OF BUILDING. HE HAD ABRASIONS ON HIS ARMS AND LEGS FROM THE TIRE. HE STAYED AT QUARRY UNTIL 3:00 PM CHANGING T IRE. WAS ASKED SEVERAL TIMES IF HE WANTED MEDICAL ATTENTION, HE SAID NO HE WAS OK.
TRUCK DUMPED IN PRIMARY PLANT, RETURNING TO PIT TO BE LOADED. TRAVELED TO RIGHT SIDE OF HAUL ROAD ABOUT 1/2 WAY DOWN. WENT 75 YARDS TO BOTTOM OF GRADE. JUMPED A STONE PILE AT INTERSECTION TO R IGHT SIDE TURNED INTO BERM. CAB LAYED AT 45 DEGREE ANGLE ON LEFT SIDE OF CAB. MINER HAD LACERATIONS ON SCALP FROM HITTING CAB INTERIOR & COMPLAINED THAT BOTH SHOULDERS HURT. PARAMEDICS SAID H
