Employee strained EE's wrist while changing out a bag in a baghouse.

While working, the employee experienced pain in groin area. A doctor's examination identified an inguinal hernia.

Mechanic from other operation was helping and strained their back while removing a side panel from an excavator. Reportable due to doctor prescribing prescription medication.

Employee was shifting a 55 gallon drum on a pallet when EE's finger got pinched between two drums.

Employee reported feeling lightheaded and dizzy. Seen at clinic and cleared for work but referred to an ear specialist later for noncommunicable condition resulting in swelling in employee's ear. Employee reported a possible connection to a loud noise employee heard earlier in the shift as a loader filled an empty hopper.

Employee was team lifting rocks off a catwalk when they felt a pull in their stomach.

The dozer operator was moving material on the yellow layer when the operator backed off a highwall approximately 20 feet tall.

Employee was helping another driller place steel back into the rack when they felt a pop in shoulder. Injury reported on 3/10/21 and Dr. visited on 3/12/21 resulting in light duty restriction.

Employee contacted the supervisor on 1/25/21 to report that EE would be out due to blood clots in EE's lungs and contacted the supervisor again on 1/27/21 notifying supervisor that EE believed the clots could be a result of the muscle strain reported in 12/2020 and EE would be examined again on 2/3/21.

Employee was raking clinker when a piece of dust got behind face shield and safety glasses and in eye causing irritation. The employee was seen by a Dr. and issued a prescription.

Employee was opening a door on a crusher and pinched a finger between it and a nearby chute causing a cut requiring stitches.

A plant support specialist was operating a CAT 980M loader and separating larger boulders from the stockpile. EE drove with the bucket in the air and as they dumped the bucket, the loader tipped and laid over on the right side. The operator made contact with the inside of the cab causing pain to arm and knee.

Employee was dismounting equipment. When EE stepped on the ground EE's ankle rolled causing EE to lose balance. EE tried to catch self, and upon doing so broke wrist.

Plant Support employee slipped on a patch of ice and fell injuring wrist.

Employee was cleaning out the back of a service truck. While exiting the back of the truck employee slipped and fell on the truck mounted vice bruising ribs and shin.

The employee opened a sample port valve to determine if a hydrated lime blockage had cleared the elevator. While the valve was open the blockage broke free sending material out the valve and onto the employee. The employees was wearing safety glasses but the material got behind the foam seal of the glasses and caused injury around and to employee's eyes.

Employee was cleaning hydrated lime out of an elevator pit when ee felt irritation on the back of neck. EE attempted to clean the hydrated lime off with vinegar which caused a reaction and a 2nd degree burn.

An excavator operator was swinging to load haul truck when they struck the sideboard of the haul truck they were loading jarring the truck. The operator of the haul truck later reported a pain/discomfort in their back and sought treatment for back spasms on 9/18/2018 resulting in them being prescribed muscle relaxers.

Employee was shoveling material on the right hand side of ee body when ee twisted and felt ee left knee pop.

I was cutting a "C" channel while in a man basket. At the end of the cut the material folded and caused the grinder wheel to backlash and cut my finger. My helper witnessed the cut. I saw the laceration and knew I needed stitches. I drove myself to the emergency room and received three stitches on my right hand index finger.

EE was loading admixture to a ready mix truck drum using 5 gal. buckets, on 3rd or 4th bucket EE stated felt pain in arm/shoulder. The injury was not reported until 12-15-2017 when the employee sought treatment. EE was placed on restricted duty limiting lifting and activity using right arm until cleared by a physician.

EE lacerated the little finger on right hand while attempting to pry open a sliding gate on a coal chute with a bar. Hand contacted an electrical bracket, cutting finger. The wound required 4 sutures to close - the employee returned to full duty with no restrictions.

4/15/16 EE was reeling up a torch hose when they felt a pop in their back followed by cramping. 4/19/2016 EE received chiropractic examination and adjustment - returned to work without restriction or follow-up appointment required.

EE was performing maint./clean-up at Conveyer No. 0004. As EE shoveled, felt back pain and reported the incident to supervisor. EE preferred to wait and see if condition improved-which it did not. EE was treated by a physician on 4/11/16 for back spasms with Rx medications and restricted from lifting over 10 lbs until cleared by physician.

Employee was stripping a metal-clad jacket from a cable with a Linesman Pliers. The lineman's pliers slipped and struck the employee in the mouth chipping front tooth.

EE was descending down an exterior office stairway that was under maintenance, when a board/stair tread shifted causing him to fall forward and down the stairs resulting in damaged/torn tendons in his knees.
