Operator closed cab door of loader and right index finger was stuck between the door and the door frame, causing a partial amputation of the index finger.

Removed access panel and placed it on the tail pulley guard on the back of conveyor. As they were gaining access to chute the access panel slipped from its position and punctured a finger requiring stitches.

Employee was removing a blow bar on the crusher using a hammer and swung the hammer to strike the wedge to dislodge it, but missed the striking target and got finger between the hammer handle and the door of the crusher. Causing right index finger to bleed a little and swell a little. Was given general first aid at onset, but later went for treatment.

Subject located on mine property deceased from apparent suicide. Authorities notified of abandoned vehicle day prior and came back to search area for missing/wanted man. Found subject at 14:41 and req coroner at 14:44. No mine accident or injury, just trespasser.

Changing pillar block bearing, chain come-along unlatched and shaft landed on my finger and bolt.

Was repairing belts on Lower Dust Collector when hand slipped between belt sheave, cut right small finger.

Employee climbed up the right side of the ladder and slipped on the ladder falling to the ground on knee. Twisted and sprained right knee.

Lumbar strain, repeatd motion of spud bar

Stepped out of Fuel truck, right boot stuck in mud, kept walking and rolled right ankle.

Was using impact wrench and wearing safety glasses, when object got into eye

Had been grinding in the shop, went to restroom to rinse off face and washed object into eye

Impact socket blew apart striking employee in mouth and face. Employee lost a tooth, has a fracture bone in face and a fractured nose

Was working on dozer changing roller cup. Had raised boogie with jack, jack slipped causing boogie to come down pushing down impact wrench and injuring wrist.

EE WAS TORCHING (CUTTING) DOWN A CONVEYOR FROM THE STONE PLANT. HE HAD FINISHED CUTTING PART OF A CATWALK & WALKED ON STUMBLED ON THE CUT PART WHICH GAVE WAY

ICY GROUND-TIRE RAN OVER ROCK AND SQUEEZED IT OUT. THE ROCK ACTED LIKE SQUEEZING SOAP AND FLEW TOWARD EE'S LEG. IT HIT HIM ON THE LOWER LEG.

ROCK TRUCK BACKED INTO SECOND ROCK TRUCK. HURT OPERATOR TRIED TO GET OUT OF THE WAY. BACK OF FIRST TRUCK WAS COMING AT HIM IN CAB. TRUCK HIT AND OPERATOR JAMBED KNEE IN DASH.

EE WAS CLIMBING ON MICHIGAN LOADER LADDER. HIS FOOT & RIGHT HAND SLIPPED. HE SWUNG PIVOTING ON LEFT HAND & FOOT. HE SWUNG WITH METAL SUPPORTINGLIGHTS. HURT HIS LOWER BACK & FELL ON RUBBER TIR E TO GROUND

TRUCK WAS BACKING INTO A BACKHOE OPERATED BY THE INJURED WORKER. TRUCK DRIVER LOST SIGHT OF THE BACKHOE AND HIT IT. INJURED WORKER JUMPED FROM THE BACKHOE BEFORE IMPACT AND INJURED FOOT.

HYDRAULIC HOSE BURST. HYDRAULIC OIL SPRAY ON HOT BULLDOZER ENGINE AND SMALL FIRE STARTED. EE JUMPED FROM DOZER AND WIND CAUGHT OIL PUSHING IT INTO EE'S EYES.

FELL OFF A LADDER THAT IS A PERMANENT LADDER FIXTURE ON A TRUCK IN THE SHOP. THE BADDER RAILS WS BLOCKED BY A STUFFED ANIMAL ON THE RAIL.

WHILE THROWING ROCKS IN LOADER ON HILLSIDE, HE TWISTED HIS RT KNEE & FELL TO GROUND.

WORKER WAS UNLOADING NEW TRASH CONTAINERS TO GET READY FOR PAINING, CONTAINER TIPPED FORWARD WHIEL WOKER WAS WORKER WAS ATTEMPTING TO PULL LIDS OUT, PINNING HIS ANKLE BETWEENCONTAINER AND FLOO R.

EMPLOYEE WAS WALKING TO HIS PERSONAL VEHICLE AFTER HIS SHIFT ENDED AT 6:00 AM AND TRIPPED OVER A FROZEN CLOD AND FELL.

EMPLOYEE WAS WALKING FROM BACK OFFICE TO FRONT OFFICE AND TRIPPED ON A BOX OF COMPUTER PAPER, AND FELL TO THE FLOOR.

NO INJURY-TRUCK WAS BACKED IMPROPERLY TOWARDS SPOIL AND OVERTURNED.DAMAGE TO VEHICLE-BROKEN FRAME,MINOR DAMAGE TO CAB AND TRUCK BED.
