Employee was drilling a hole and felt pain in left shoulder

two maintenance technicians were removing a wear plate out of hammermill B. While the employees were removing the first wear plate, the injured employee placed their left hand near a worn bolt head to hold the wear plate, while the other employee was utilizing an impact wrench to free up the nut.

Employee's right knee locked up when EE attempted to stand from a kneeling position

While working on plattco valve 7089 underneath bag house 7888, tech was finishing up job, stepped back hitting head on the surrounding structure. As tech was turning around tech lost footing, cause tech to fall. As tech was falling tech tried to brace fall using right arm causing the injury.

At approximately 9:30 am on Monday, October 11, 2021, EE was helping other miners clean underneath a conveyor line. EE made contact with the conveyor belt and as a result, appeared to have friction or abrasion burn on their arm. EE refused medical doctor and returned to work. Went to Dr off company time. No Dr report has been given at this time.

The employee tripped over electrical wires that were being used to perform our annual grounding checks. The employee extended right hand to catch self from the fall which led to EE's right pinky finger contacting the grated flooring.

While cutting a coupling on the vertical band saw the coupling rotated in the maintenance tech's hand causing the sharp edge of the coupling to cut left middle finger.

Witness: EE was conducting shovel labor work around Nestle Purina processing area when EE fell backwards on own. EE was non-responsive. Nestle Safety team applied defibrillator to EE. Its instructions were to give EE chest compressions. This continued until Ambulance arrived.

Employee was using a hammer to clear a blockage in a chute, while striking the outside of the chute the employee became distracted and incidentally struck left index finger with the hammer.

Part time employee fell from height, fractured ribs, perforated lung, cuts and abrasions.

Employee was carrying two pieces of liner materials on right shoulder. When EE began to lay the material down one of the pieces slid making contact with left forearm causing a laceration. The liner material has a thin metal sheet on one side that caused the laceration.

Miner was using a hammer to break off the shank of self-drilling screws (Tek screws) when one broke, deflected off of a beam and struck the miner in the left eye. Miner was wearing dark safety glasses and based on the type of injury, was not wearing them properly at the time of the injury. Miner should have been wearing a face shield with safety glasses per company policy.

EE was climbing down an access ladder using 100% tie off procedures. He was wearing a double retractable lanyard. One side attached overhead and other hanging loose. Loose end became hooked on ladder's protective cage. As EE descended, lanyard extended before coming loose. When unused side retracted, it struck the EE in the face and resulted in a laceration to his lower lip.

Employee was part of a team involved in removing an auger from a conveyor for replacement using a chain fall to elevate it up and out of the conveyor. The flights of the auger were very sharp from wear. The auger shifted a bit as it was lifted out of the conveyor bed and contacted the employees left forearm resulting in a laceration requiring 7 sutures.

Employee tripped over a stud prodruding from the packaging floor and fell, hurting his right leg, thigh and back.

Worker reached into his tool bag to retrieve a pair of scissors and cut his right index finger on a paint scraper

Concrete slab walking surface covered in wet clay causing slippery footing.

Employee was properly dismounting CAT 416 backhoe. While stepping down onto ground level, employee "rolled" his ankle resulting in a sprain. Employee was evaluated by a physician and placed on light duty.

Employee bumped his elbow while pulling a stuck cardboard box out of a box erecting machine.

Employee leaned the edge of his left hand against the back wall of the chute he was clearing and cut it, requiring stitches, tetanus shot and antibiotics.

Employee felt back pain lifting pallet on 7/14, but declined medical attn. On 7/28 he sought treatment and requested a back specialist. Employee missed two days of work because a specialist was unavailable. Specialist diagnosed lumbago, administered pain and anti-inflammation medication and restricted employee to light duty on 7/30.

Employee had recurring pain from handling packaging materials the week before and was diagnosed with a finger sprain and administered prescriptions and a finger splint

Employee was raising the conveyor on line 6 when the air-ratchet slipped off the bolt, fell and caught his gloved hand between the ratchet handle and the frame, creating a laceration to the right pinky finger. The Hi-flex glove was not cut. The employee received 3 stitches and antibiotics to prevent infection.

Laceration to right arm from chute while cleaning on conveyor 6174 in engineered litter process building required 6 stitches and antibiotics.

Associate was found unresponsive in her office at 10:05AM. The plant emergency response team was called and performed rescue breathing and CPR until the King Willliam EMT service arrived and removed her to the Mechanicsville Regional Memorial Hospital where she died.

Employee was burned when he removed the drain plug of a hot melt glue system reservoir and the glue splashed onto his ungloved left thumb and finger.

Employee was walking through our additives department when he started itching and difficulty swallowing. The operations had been shut down at 7:00AM for preventive maintenance activities. He was transported to the local medical clinic where he was medically treated with steroids and released to full duty.

While feeding the box erector machine in packaging, the employee quickly bent down to retrieve falling cardboard packaging materials when he felt a pain in his upper right side.

Employee was pushing pallet across concrete floor when pallet stopped abruptly, jamming his left wrist causing pain and a small knot on the back of his hand.

Employee was shoveling clay from floor when he felt a pain in his lower back while twisting and bending.

Contract trailer driver came into trailer drop lot to pick up loaded trailer. The trailer he was picking up was too high, so he went to adjust the landing gear to allow him to pick up the trailer. Driver reports that during his attempt to adjust the landing gear, the crank handle slipped out of his hand and struck him in the face, causing a laceration which required sutures.

Employee was trying to insert a connecting wire joining two sections of a conveyor belt. While pulling the wire through the connecting splice, his pliers holding the wire slipped off, striking him in the mouth and causing the lip laceration.

Employee, upon approach to employee entrance gate, lost control of motorcycle he was riding, slid into control box and under gate, striking head resulting in fatal injury.

Employee was using a multi-tool knife to cut a cover . Knife(open) was placed on top of the tarp covering burner opening approx 3.5 feet above ground. The wind blew the tarp up and off the burner, causing the knife to fly up and then fall, impaling his hand in the slot between thumb and forefinger on his left hand.

Employee tripped on deck floor grating and while falling, reached out with left hand to arrest fall. The trauma from grabbing railing pulled something in upper left arm. (Employee is left handed)

She stepped in the area to pull out a bag that had jammed and the machine cycled and the support forks rotated up and lacerated the top of her right index finger causing an avulsion. Employee was treated at a local hospital where she received stitches and was discharged with no restricted duty.

Warehouse employee putting away pallet of finished goods, felt something under his shirt. Employee swatted at area and continued working. Approximately 1 hour later, employee felt nauseous and went to check on location where itching occurred. Employee found 4 bumps that were red and swollen (2 under left arm, 2 on back/shoulder) and reported to his supervisor.

Employee notified of fault on conveyor, utilized LOTO, opened gates to return rollers. While opening gate, it struck left wrist. Did not break skin or swell. Ice provided, he continued to work. Saw doctor 9/4/09, original diagnosis was sprain. Employee returned to work 9/4/09. Follow up appointments for pain in Nov, Dec. MRI on 1/22/10 revealed tear. Surgery on 2/1/2010.

EE clearing 40 lb. boxes from pallet line. EE reached for a box between two pallets, boxes on the front pallet fell onto his right thumb causing a contusion and hyper-extending the tendons in the first knuckle of the thumb. Originally treated as first aid, follow-up evaluation with physician revealed the strained tendons and a cast was applied to immobilize the thumb.

EE working with another technician installing a coater bowl, manually moving the coater bowl, in an inverted position. EE moved his hands from under the edge of the coater bowl, the bowl shifted down. Middle finger of the EE's right hand was caught between the bowl edge and the floor. EE had a laceration and a tuft fracture of the tip of the finger. Laceration required stitches.

2 Workers walked up to box conveyor to check the tracking of the belt following the replacement of some rollers. Inspection door removed to visualize the belt. EE drop a wrench & Bent down to pick it up. He lost his balance as he stood up & tried to catch himself. His arm entered conveyor opening which pulled him into it. He sustained multiple fractures right lower arm

An employee states he was climbing down from the hammermill when his right foot rolled off the edge of the grating causing his knee to twist inward. He claimed to be stepping down about 2 feet. He said that the pain was not all that bad and continued working. The pain continued for several days before he went to see the dr.

EE walking north along the west side of the main building towards the Maintenance Shop. He reached the shop door & turned right through the open roll-up door. A Genie aerial lift had been left parked just inside the doorway with the boom lowered. The EE struck the edge of the boom with the bridge of his nose. Incident reported to the Mine Foreman, first aid was administered.

While walking across the concrete pad near the shop, the worker slipped on wet clay and sustained a pulled muscle to the groin area.

Employee was lifting 27# pails to and from check scale on Line 1 in the Packaging area. Employee had pain in her back. Initially not reported by the employee as work related. Employee was checked immediately by EMS and refused transport. After a check by a physician later it was identified as work related. The injury is a strained disc in the employees back.

Employee was cleaning the photo eye lens in the pail stop section of Line #1. The stop air cylinder extended and as it retracted, the employee's left ring finger was pinched and lacerated. The laceration required 4 stitches for closure. The air cylinder isolation valve was not locked out prior to the cleaning process.

On 1-10-08 at 10:00 PM, a packaging operator was clearing a jam at the second level of the line 3 palletizer. The operator was attempting to carry 3, 10 pound bags of product down the steps from the second level. With both hands full, the operator missed the last step of the stairs and fell to the floor. The operator suffered a sprain / contusions on his right knee, and leg.

Employee was clearing a discharge chute on the EL wet screens with a shovel. While using the shovel to dislodge the clay, the handle struck the EE's left hand at the base of the thumb causing a contusion of the area. EE was taken to Emergency Room where he was prescribed medication. EE reports he was wearing gloves.

On 10-23-07 an electrician was cutting holes in an electrical control cabinet. EE was using a grinder with a cut off wheel. The grinder kicked back and the blade touched the left index finger. EE was taken to Patient First and received 3 stitches, and returned to work that day.

Employee was attempting to replace a slat on a conveyor system using blocking tatics. Conveyor tension was tightened slightly and blocking slipped. Employee's finger was struck by conveyor as it jumped back resulting in a deep contusion of the finger, minor laceration.

Employee was in the process of cleaning up. He was walking down a mound of clay. When he reached the bottom, he slipped. He struck his left elbow on the concrete floor causing a fracture to his elbow.

Employee was changing a motor assembly inside the bailer on line 3. He had taken the motor loose and was moving it out from under the machine. His finger was mashed between the motor and the frame of the machine.

Thursday night 11-2-06 at approximately 9:45 PM a maintenance associate was using a crowbar while changing a belt on the duck plucker conveyor in the Mill Bld. While applying force on the take-up roller, the crowbar slipped and struck employee in the lower right chin. This caused a laceration that required 4 stitches to close.

EE was working with a crew on a pin mixer and was in the process of identifing why it was hanging up. As other EE's were attempting to free the spinning shaft, it broke loose and spun towards the EE's hand, which was resting on the frame of the unit. One of the pins caught his gloved hand and crushed his right middle finger.

Employee smashed her thumb when a tilt hopper caught her thumb between the hopper and the rack on a forklift. Employee was pulling the hopper back into the locked position and did not move her hand in time.

AT 8:15PM ON FRIDAY 7/1/05 EMPLOYEE WAS DUMPING A HOPPER INTO A DUMP TRUCK WITH A FORKLIFT. THE HOPPER SLIPPED TOWARDS THE END OF THE FORKS CAUSING THE FORKLIFT TO TILT FORWARD INTO THE DUMP TRUCK. EMPLOYEE STRUCK HIS LEFT KNEE ON THE DASH OF THE FORKLIFT AT THIS TIME. HE WAS DIGNOSED WITH A CONTUSION OF THE LEFT KNEE.*ORIGINAL EDOC # 219294*

Associate was operating a forklift to load a tractor trailer with loaded pallets while his foot extended beyond the frame of the forklift. When entering the trailer his foot was caught between the forklift and the trailer and twisted. Fractured outside bone in left foot. Note: accident occurred at warehouse, not in the mill.

Associate was rebuilding a pallet - twisted - felt something in the back muscle. Did not report as work related. Dr. asked for more information on 8/24/04 & stated it was work related - a severe muscle pull - requested an MRI which occurred on 9/2/04. Waiting on results from that.

WHILE USING A RAZOR KNIFE TO TRIM A PIECE OF NEOPRENE BELTING THAT WAS BEING USED AS A PROTECTIVE SKIRT FOR THE TOP OF THE FORKS ON A FORKLIFT, THE KNIFE SLIPPED AND CUT THE ENDS OF THE LAST 3 FINGERS ON THE LEFT HAND. EE WAS TAKEN TO HANOVER MEMORIAL HOSPITAL WHERE HE RECEIVED 9 STITCHES AND WAS RELEASED. 3 ON END OF MIDDLE FINGER, 4 ON END OF 4TH FINGER, 2 ON TIP OF LITTLE FINGER

EE WAS RECEIVING TRAINING ON THE ROUTINE MAINTENANCE OF THE BAMIS BAGGING MACHINE. HE HAD REMOVED HIS LOCK FROM THE INLET AIR VALVE AND MOVED TO THE FRONT OF THE MACHINE. THE TRAINING MECHANIC OTHER TOLD EE HE WAS REMOVING HIS OWN LACK AND TURNING ONT HE AIR. HE REACHED INSIDE THE MACHINBE AT THIS TIME AS A CYLINDER ACTUATED AND CAUGHT HIS FINGER BETWEEN A PINCHED POINT. THIS CAUSE

WHILE UNLOADING 4' BY 8' SHEETS OF EXPANDED METAL AND PLACING THEM IN THE METAL RACK, THE EMPLOYEE LET A PIECE SLIP IN HIS HAND. THE CORNER OF THE METAL CUT THE SKIN BETWEEN THE THUMB AND INDE X FINGER.

EMPLOYEE WAS OPERATING A "LARGE PACK" PACKAGING LINE WHEN AN EMPTY 20# BAG WAS ATTACHED TO THE END OF A FULL ONE. HE ATTEMPTED TO PULL THE EMPTY BAG OFF BEFORE IT ENTERED A BAG SEALING UNIT. H E OVERREACHED WHEN GRABBING THE BAG AND HIT HIS LEFT THUMB CAUGHT IN THE BELT AND PULLEY ASSEMBLY THAT GUIDES THE BAGS INTO THE SEALER.

EE WAS DRILLING HOLES IN A PIECE OF ANGLE IRON IN AN OVERHEAD VERTICLE POSITION.JUST BEFORE THEDRILL BIT WAS TO EXIT OUT OF THE OTHER SIDE OF THE METAL, IT GRABBED OR BOUND UP CAUSING THE TOR QUE OF THE MOTOR TO TWIST IN HIS HAND FRACTURINGAREA OF THE RIGHT HAND, MIDDLE FINGER

WHILE DUMPING CLAY INTO THE CRUSHER BUILDING, EE HEARD THE CRUSHER MAKING AN UNUSUAL NOISE. THEY DETERMINED THAT THE SHAFT WAS PERIODICALLY LOCKING UP. HE & A CO-WORKER TRIED TO LOOSEN THE SHA FT WITH A PRY BAR BUT IT SLIPPED & HE FELL BACK INTO THE GUARD RAIL OF #4 BELT HITTING HIS RIGHT HIP. HE RECEIVED A BAD BRUISE, WAS GIVEN A MUSCLE RELAXER BUT WAS UNABLE TO RETURN THE NEXT DA

WHILE REPLACING MOTOR AND REDUCER ON CONVEYOR #971, THE INJURED PERSON HAD POSITIONED HIMSELF UNDER THE UNIT AND WAS REMOVING THE BOLTS THAT WERE HOLDING THE UNIT IN PLACE. WHEN THE LAST BOLT WAS REMOVED THE UNIT DROPPED APPROX. 16" CRUSHING HIS LEFT RING FINGER BETWEEN THE MOTOR UNIT AND THE FLOOR.

WHILE STACKING EMPTY PALLETS, THE INJURED PERSON LISTED FELT A PULL IN THE MUSCLE AREA OF THE RIGHT GROIN AREA. WHEN THE PAIN IN THIS AREA PERSISTED AFTER ABOUT AN HOUR, IT WAS REPORTED TO THE MINE FOREMAN IN CHARGE. AN INCIDENT REPORT WAS FILED. FIRST AID ONLY UNTIL 8-23-01.

REMOVING A SECTION OFF A ROLL UP DOOR BETWEEN SHIPPING AND PACKAGING DEPARTMENTS, A SECTION OF THE DOOR SEPARATED AND FELL STRIKING THE EMPLOYEE ON THE HAND RESULTING IN A LACERATION.

REMOVING A BOX JAM ON LINE 5 PALLETIZER. STEPPED ON IDLER ROLLER, SLIPPED BACKWARDS AND LEG WENT INTO OPENING AT END OF ROLLER. TWISTED AND CAUGHT SELF IN MID FALL. '

WAS PMING THE TRAILBLOZER ON LINE 3, POWER & AIR WERE LOCKED OUTL. THE BAG FLATTENER CYLINDER WAS IN THE AIR. HIS HAND WAS RESTING ON THE BOUNT WHEN THE CYLINDER DROPPED DOWN ON HSI THUMB.

WHILE MOVING/REPOSITIONING VENDING MACHINES, A CROWBAR WAS USED FOR LEVERAGE. EE MOVED OVER THE SHOULDER OF COWORKER AND STEPPED INTO THE HANDLE OF THE CROWBAR-IT HIT HIM IN THE MOUTH CHIPPING ONE OF HIS FRONT TEETH.

EMPLOYEE WAS RUNNING OVER A TRUCK. HE HAD TAKEN A TRAILER TO BE LOADED TO THE SHIPPING DOCK WHEN HE WENT TO UNHOOK HIS AIRLINE, HE PUT HIS LEFT HAND ON THE TRAILER AND HIS RIGHT ON THE AIRLINE . THERE WAS ICE ON THE FRONT OF THE TRAILER AND IT CAUSED HIM TO LOSE HOLD AND FALL OFF THE TRUCK ONTO THE GROUND, HITTING THE STEP WITH HIS RIGHT HAND AND ELBOW.

WHILE EE WAS GETTING OFF THE LIFT, COWORKER PULLED UP NEXT TO HIM. THE FRONT RIGHT TIRE OF THE FORKLIFT RAN OVER THE OUTSIDE OF EE'S LEFT FOOT. EE WAS WAS WEARING

WHILE CLEANING IN AN AREA AROUND A CONVEYOR A CLOD OF CLAY FELL OFF, STRIKING EE ON THE HEAD. HE WAS WEARING HIS HARD HAT AT THE TIME OF THE INCIDENT.

WHILE OPENING A 5 GALLON PAIN CAN, KNUCKLE OF TH EMIDDLE FINGE RON TH ELEFT HAND STRUCK THE TAB OF THE LID. CUT REQUIRED STITCHES.

EE WAS ATTEMPTING TO CLEAR A PALLET FROM THE DEPALLETIZER, HAND STRUCK THE GUARDING. INCIDENT REPORTED ON 6-14-00, BUT EE DID NOT SEE A DR. AND KNOW THE EXTENT OF INJURY UNTIL 6-28-00.

WHILE OPERATING THE DISPOSABLE BOX LINE A WEIGHTFELL OFF THE SCALE & HIT EE ON THE TOP OF THE HEAD.

WHILE ATTEMPTING TO STRAIGHTEN A STEEL POST ON A GTE, THE BRACKET WHICH ATTACHES THE POST BROKE. EMPLOYEE LOST HIS BALANCEAND FELL TO GROUND STRKING HIS ELBOW.

EE WAS ATTEMPTING TO REMOVE THE STORM GRATING TO CLEAN BAGHOUSE. WHEN EE APPLIED FORCE TO REMOVEGRATE, HE FELT PAIN IN HIS RIGHT HAND.

EE ATTEMPTED TO STRAIGHTEN FOUR PALLETS ON THE FORKS OF A FORKLIFT. EE TWISTED BACK INSTEAD OF USING LEGS; RESULTING IN A BACK SPRAIN.

EMPLOYEE WAS WALKING THROUGH MAINTENANCE SHOP WHEN LEFT KNEE GAVE OUT. EMPLOYEE STATES THERE WERE NO HAZARDS ON FLOOR; I.E. WATER, MUD.

EMPLOYEE STEPPED OFF A PALLET AND CAUGHT THE CORNER OF AN ADJACENT PALLET WITH HIS FOOT.

EE WAS WASHING TRUCK #434 WITH POWERWASHER WHEN HE GOT DONE HE TOOK OFF HIS SAFETY GLASSES DIRT ON GLASSES FELL IN HIS RIGHT EYE

EE WAS FEEDING BAGS INTO THE SEALER. WHILE FEEDING A BAG, HER LEFT THUMB BECAME INVOLVED WITH THE COMPRESSION MECHANISM.

EE RAN TO RESET THE HALF PALLET STACKER. A TWO INCH PIECE OF METAL WAS PROTRUDING, HE CAME IN CONTACT WITH THIS METAL RESULTING IN A SIX INCH LACERATION.

EE WAS FEEDING BAGS INTO THE SEALER, LEFT THUMB BECAME INVOLVED WITH THE COMPRESSION MECHANISIM, CAUSING A SEVERE PINCH AND BRUISE.

EMPLOYEE WAS PULING ON A BELT IN AN ATTEMPT TO FREE IT. HIS LEFT HAND ENTERED A PINCH POINT AT ATRANSITION POINT BETWEEN 2 CONVEYORS. HIS HAND ENTERED THE OPENING BETWEEN THE ROLLER AND CONVEY OR.

EMPLOYEE WAS MOVING A TRAILER MOUNTED MOTHER 40 G WELDER. HE AND TWO OTHE REMPLOYEES ATTEMPTED TO DRAG THE WELDER APPROX. 75 FEETTO THE HEAD OF THE DRYER/ THE LEFT TIRE OF THE TRAILER STUCK IN THE CONCRETE FOOTING OF THE BURNER PLATFORM SUPPORT. THE FLOOR WAS SLICK FROM RAIN.THE TONGUE OF THE TRAILER SKEWED TO THE LEFT AND PUSHED AGAINST HIM. THE LANDING GEAR STRUCK HIS KNEE.ALL 3.

THE MECHANICS HAD A PROBLEM WITH THE CASE SEALERIT WAS DECIDED THAT THE PROBLEM WAS RELATED TO ALIGNMENT OF THE PHOT EYES OR PROXIMITY SWITCHES EE ENTERED THE MACHINE TO CHECK THE LOWER SWITHC ES HE ACTIVATED THE REG FEED PLATE RETURN WHICH MASHED HIS LEFT HAND.

EE WORKS A SPECIALTY PRODUCTS LINE. THIS LINE IS AMANUAL OPERATION WHICH PACKAGES BAG-N-BX, PAILS, AND CONVENTIONAL LARGE PUCK PRODUCTS. THE WORK IS OF REPETITIVE NATURE DUE TO FALLING, FOL DING, TO PIN, AND STACKING OF HEH PRODUCT. THE LINE CONSISTS OF 6 TO 8 TEAM MEMBERS WITH ROTATION OF TSKS EACH PALLET OR EVERY 15 TO 20 MINUTES. ALTHOUGH NO ACCIDENT .

DURING THE PROCESS OF CHANGING A PROPANE BOTTLE ON A FORK LIFT, THE VALVLE ON THE PROPANE BOTTLELEAKED AND THE LEAK CAUSED EE TO INHALE THE PROPANE.
