Employee was dumping a wheelbarrow on flat ground and felt a pain causing them to let go of the wheelbarrow, allowing it to pull on right arm causing pain to right shoulder. The initial pain was due to a hernia caused by dumping the wheelbarrow.

operator of truck dumped on a grade causing the tri-axle to roll to the side. weather conditions were clear. truck is still operational master dump cylinder needs to be replaced.
employee was assisting in previous truck roll over. Employee was chaining the dump bed together to upright the truck. employee slipped and sustained an injury. weather was clear.

Crew was working to remove old fence from entrance area. Sections of fence were cut down and carried to the bucket of a backhoe. The backhoe was then used to remove the fence from site. While placing one section of fence into the bucket, the cut end of the section of fence caught the employee in the forearm/wrist area and caused a laceration.

Moving machine guard into place, hand slipped off of guard causing right hand to contact guard. When right hand contacted guard, right thumb was cut.

On November 12th EE was oiling the mill when his foot slipped and his right knee landed on a piece of angle iron and lacerated his right knee. Miner was treated at a care facility and returned to work.
On March 14th the miner was cutting steel with an oxy-acetylene torch to be installed in a silo. A hot piece of slag was trapped in the fold of his arm and ignited his sweatshirt causing a burn to his left forearm. He also burned 2 fingers on his right hand trying to pat out the fire.

On 9/9 the mechanic was loosening bolts to a hydraulic pump on an excavator when he twisted his back. He was sent for medical evaluation and was returned to full duty with no restrictions.
On 8/19 the miner was changing the doors on the screen at AP1. In doing so, he strained his back. He was sent for evaluation. He was returned to full duty with no restrictions or medications. On follow up at care facility on 8/24, they now referred him to a chiropractor, but is continuing normal duties with no restrictions or medication.
Employee had maintained the oiling points in a mill though an access door. Upon standing up after finishing, the employee felt a sharp pain in his lower back. He was examined, given medication and released for duty with minimal restrictions which did not effect his duties.
On 2/9 the miner washed his vehicle and slipped on an icy spot injuring his left knee. The miner reported the this to his supervisor and returned to work. On 3/9 he requested medical attention and was examined and released with no restrictions or medications. On 3/21 during a follow-up exam he was given medication for pain and was released with no restrictions

Aggregate Lab technician was using the restroom. The toilet was not flushing properly, so the technician had to manually move the flush valve, activating chain inside the tank. While moving the lid of the water tank, a sharp edge of the porcelain lacerated her thumb. The laceration was treated with skin glue.
