EE was climbing down the ladder of the haul truck when EE either missed the step of foot slipped on the ladder and EE fell approx. 3 feet to the ground.

Employee was grinding on newly installed conveyor guards when a horse fly bit him on the right shoulder. He swatted at the fly and the grinder swung around and contacted his right arm. The resulting injury required surgery to repair a nicked tendon. Will return to work approx. 2 weeks.

Foot slipped off ladder rung while climbing down from 988B Front End Loader. Fell backwards and hit head, elbows and back of neck.

Ee & co-worker were installing a motor on belt #8 using a 990 Trackhoe. After placing the motor, the trackhoe bucket adjusted hitting inj ee's right arm. He was diagnosed with a right wrist fracture.

JUMPED TO GROUND FROM HEIGHT OF APPROX 6 1/2', LANDING ON A SMALL ROCK WITH RIGHT FOOT. CAUSED BRUISING TO HEEL AND FRACTURE OF TOE. EE WAS OFF WORK FOR 4 DAYS, 3 OF WHICH WERE WAITING TO GET IN TO SEE A DR QUALIFIED TO READ AN X-RAY.
