Employee was using an impact wrench and a combination wrench to unscrew rusted bolts/nuts from a chute on the plant. While engaging the impact wrench on the bolt head, the combination wrench, which was positioned to hold the nut in place, slipped from the employees grip and struck them in the area of the left eye causing a contusion and laceration to the upper eye/eyelid.

Employee was repairing a piece of plant equipment and, while using a cutting torch to remove an overhead bolt, the wind blew a piece of hot metal into left ear causing an inner ear burn.

A hydraulic ram was being used to push a shaft out of a pulley when the ram slipped off the shaft, as a result of this the spacer separated from the ram bounced off pulley then struck the employee under his chine, requiring three stitches.

Employee cut the base of his left thumb with a utility knife when cutting a conveyot belt.The employee pulled the knife towards his body when the knife cut loose from the belting & came in contact with his left hand and cut through his leather glove. Five stitches were required and the employee returne to work the same day.
Employee was stepping down on the bank next to the dredge when his knee gave out. The employee and foreman did not report the incident until 1/26/09. The employee went to the doctor on 1/26/09 where it was diagnosed as a sprain. The employee returned to work on the same day and did not miss work.
