Employee was repairing a piece of plant equipment and, while using a cutting torch to remove an overhead bolt, the wind blew a piece of hot metal into left ear causing an inner ear burn.

A hydraulic ram was being used to push a shaft out of a pulley when the ram slipped off the shaft, as a result of this the spacer separated from the ram bounced off pulley then struck the employee under his chine, requiring three stitches.

Employee cut the base of his left thumb with a utility knife when cutting a conveyot belt.The employee pulled the knife towards his body when the knife cut loose from the belting & came in contact with his left hand and cut through his leather glove. Five stitches were required and the employee returne to work the same day.

Employee was stepping down on the bank next to the dredge when his knee gave out. The employee and foreman did not report the incident until 1/26/09. The employee went to the doctor on 1/26/09 where it was diagnosed as a sprain. The employee returned to work on the same day and did not miss work.

Tightening bolts on a piece of pipe using a ratchet. Right hand turned red and was sore. Sprained right hand from stretching or tearing ligaments while tightening bolts.

EE WAS STEPPING UP ON PLATFORM. FOOT SLIPPED OFF EDGE. HIT SHIN CAUSING DEEP CUT ON SHIN OF LEFT LEG.

CUT LITTLE FINGER ON LEFT HAND WHILE REPLACING A SCREEN ON SHAKER. HAND SLIPPED & CUT FINGER ON SHARP EDGE. REQUIRED TWO STITCHES.

WRENCH SLIPPED FROM NUT HE FELL BACK AND HIT HIS TAIL BONE ON A BRACKET. HE HAD PAIN IN LOWER BACK NEAR TAIL BONE.

REPLACING DIGGER LINK CHAIN ON DREDGE WAS DRIVING PIN OUT WITH HAMMER WHEN HAMMER HIT ROLLER CUASING ROLLER TO BOUNCE & HIT THUMB LEFT THUMB WAS SPLIT OPEN & BRUISED

WELDING & ATTACHING HARDWARE IN TUB ROTARY, TURNED HEAD TO AVOID FLASH BURN FROM WELDER ON OTHEREND OF TUB & RAN HEAD INTO SCREENS BEING INSTALLED SCRATCH FOREHEAD CUT UNDER EYE REQUIRING 9 STITCHES

EMPLOYEE WAS CUTTING A PIECE OF PIPE WITH A CUTTING TORCH, HE SHUT OFF THE TORCH, PUSHED THE AIR RELEASE TO COOL OFF TIP,THE TORCH SELF-IGNITED SETTING HIS GLOVE ON FIRE AND BURNING HIS LEFT HAND ON THE PALM BETWEEN THE THUMB AND FOREFINGER.

AN ELECTRICAL CABINET UNLOADED OFF OF A FLATBED TRAILER TIPPED AND FELL OFF CAUSING EMPLOYEE TO FALL CABINET LANDED SCRAPING EMPLOYEES LEFT FOOT-PEELED SKIN OFF FROM TOP OF FOOT CRUSHED AND MA NGLED ALL 5 TOES TOES HAD TO BE SURGICALLY REMOVED.

HE WAS CLEANING UP THE WORK SITE AFTER POURING CSOME CONCRETE FOOTINGS & TRIPPED ON A WOODEN STAKE FELL BACKWARDS & HIT HIS RT ELBOW ON ANOTHER STAKE

EMPLOYEE WAS REMOVING TIRES FROM THE ROTARY TUB ON THE PLANT TWISTED & HURT HIS BACK RESULTING IN LOWER BACK PAIN & MUSCLE SPASMS

MOVING A PIECE OF PIPE USING A LOADER WHEN CHAIN SLIPPED CUTTING EMPLOYEES FINGER

HE HAD PLACED A 4X4 WOOD TIMBER IN GEAR TO HOLD DIGGER CHAIN IN POLACE UNTIL IT COULD BE CLAMPED SO LINK COULD BE REPLACED WHEN OLD LINK CUT OUT GEAR STARTED TO MOVE BACK AND 4X4 FELL OUT HE T HOUGHT IT WAS RUNNING AWAY AND TRIED TO PUT 4X4 BACK IN IT SWUNG AROUND STRIKING HIS LEFT ARM
