Employee was putting new gears in a shaker. The gears slipped off of the shaft and employee's right thumb got caught between the gears and the shaft when they fell. Employee's right thumb was fractured and cut.

Employee was cutting a piece of buckrubbber for VS-2 screen with a lock blade razor knife. The employees left arm was posting down on the rubber and the right hand was holding the knife. As the employee was cutting the rubber, the knife became stuck. The employee pulled back on the knife causing the knife to unhinge, slip from the cutting surface, and cut the left forearm.

Employee was performing maintenance work on CC-2. He was ascending the stairs to access the crusher when he fell up the stairs striking his knee.
Employee was climbing down from the loader after noticing his strobe light was not on. As he was climbing down the ladder he reached to shut the door and as he did, the hand holding the rail slid off and he fell to the ground. The fall caused bruises to his back and the doctor assigned him to miss 2 days of work.

A HOOK ON THE BACK OF THE TRUCK CAUGHT THE P LATFORM AND TURNED IT OVER. THE EMPLOYEE RODE THE PLATFORM TO THE GROUND AND CUT HIS HEAD ON LEFT SIDE AND BRUISED HIS LEFT LEG ALONG WITH OTHERBRUUISES ON HIS SHOULDER AND ARM.
EE WAS LIFTING A 3 GAL. BUCKET OF 57'S TO POUR INTO A SMAPLE SPLITTING DEVICE IN LAB. WHEN DOING THIS, GHE FELT A TWINGE IN HIS RIGHT SIDE. HE PULLED A MUSCLE IN HIS RIGHT RIBCAGE. EE STATED T HE BUCKET WEIGHED 30-40 LBS.
