EE WAS TURNING THE JAW AT THE WASH PLANT WITH A PIPE WRENCH. JAW WAS HARD TO TURN. IT FINALLY GAVE AND EE'S MIDDLE FINGER WAS CAUGHT IN BETWEEN PULLEY AND PIPE WRENCH. WASH PLANT WAS SHUT DOWN AT THE TIME.
EMPLOYEE WAS STANDING IN A LOADER BUCKET APPRXO 12' H, MOVING WIRES OFF OF A CONVEYOR. EMPLOYEE FELL APPRXO 12' TO THE GROUND, POSSIBLE ELECTRICAL SHOCK. EMPLOYEE LANDED ON SHOULDER, NECK NEC K AND HEAD.

EMPLOYEE WAS WELDING HAND RAIL ON WASH PLANT, HE THEN STEPPED DOWN. WHEN HE STEPPED BACK UP TO WHERE HAND RAIL WAS, HE GRABBED HAND RAIL WHERE HE HAD WELDED, HE THEN RELEASED RAIL QUICKLY AND FELL 2' TO 3' TO THE GROUND. EMPLOYEE LANDED ON HIS FEET.

WAS RUNNING HOT MIX PLANT, DOOR WAS OPEN ON SHCACK AND AS HE WAS TRYING TO SHUT IT THE WIND CAUGHT THE DOOR AND SLAM THE DOOR SHUT. THE EMPLOYEE THUMB WAS IN THE DOOR JAM.
WAS WALKING FROM MIXER TOWARD CONTROL SHACK, SLIPPED ON 2" PIECE OF HARD ASPHALT AND DISLOCATED VERTABRAE.
