Employee tripped and cut the palm of right hand requiring stitches.

Employee was changing v-belts on a crusher and fingers got stuck between the v-belts and the sheave.

Employee was shoveling a tail pulley when employee slipped and fell. When employee fell employee cut forehead above right eye.

Was cutting part numbers out of the box with a knife, it slipped off the box and stabbed my leg.

Was uncovering conduit to be replaced in soupy mud and clay shoveling heavy material in awkward position and felt back pop in middle and lower part of back had to leave ditch and couldn't straighten up. Will give few days to see if better.

Changing liners in impact crusher swinging a hammer and chips from hammer broke off and hit in employee's face.

He came down the steps from the crusher to fast and caught his foot on bottom step and fell onto a rock that was there and struck his leg.

While loading trucks, employee had an apparent heart attack.

CLEANING OUT UNDER CONVEYER, ROCK HIT HIM ON SHOULDER BLADE.

EMPLOYEE WAS CLEANING ROCKS OUT OF A CONVEYOR. AROCK SLIPPED AND HIT HIM ON THE ARM CUTTING IT. THE CONVEYOR WAS SHUT OFF AND LCKED OUT.

ATTEMPTING TO PRY A ROCK OUT OF THE JAW WITH A PRYBAR WHILE STANDING AND INJURED BACK. THE CRUSHING OPERATION WAS SHUT DOWN, AND CONTROL PANEL WAS LOCKED OUT.

EE WAS INSTALLING SIDE PLATES IN A UNIVERSAL IMPACTOR AND THEY SLID DOWN SMASHING HIS FINGER.

INJ LIFTED BEAM AND RUPTURED STOMACH MUSCLE WALL.

WHILE DRIVING OUT HAMMER LOCK PIIECE OF STEEL BROKE OFF & HIT EMPL IN RIGHT EYE.

BENT OVER TO PU7 TIRES IN LOADER BUCKET.STRAINED LOWER BACK.
