Haul truck driver reached between the seat and counsel to get a part, when EE pulled EE's hand out it was cut between the middle finger and ring finger between the knuckles, causing EE to get stitches. Operator returned to work the next morning.

EE was assisting with carrying an electrical cabinet. While re-positioning after walking through a doorway, EE twisted right arm and felt a pop in right elbow. The injury was initially treated as first aid with no restrictions. On 8/16/2023, EE visited a medical clinic and work restrictions were given.

While pulling electrical wires, the employee noticed soreness in shoulder. The employee had surgery for a torn rotator cuff and was taken off work on 10/3/2022, EE had worked full duty and missed no time until that point.

US Silica has received a confirmation of a Reportable Hearing Loss in EE Right Ear.

EE suffered soft tissue damage, in the right shoulder, resulting from a fall to the same level, while walking on uneven ground in the pit. The injury was treated as a first aid. EE was prescribed Physical Therapy. EE began Therapy on 1/22/2021.

EE was removing a 1.5" aluminum fitting from a steel pipe collar. The fitting broke and separated from the threads inside the collar. The momentum cause the EE to strike an elbow on a 4" gate valve located behind EE. The injury was treated as a First Aid. EE was evaluated and had X-Rays on 12/17/19. The X-Rays revealed a chipped bone.

The employee was lifting a slurry pump trailing cable when they felt a pop in lower back.

Employee was cutting a pipe overhead using a grinder with a cut-off wheel when the grinder licked out and cut arm. The employee was taken to a medical facility where 11 stitches were used to close the laceration.

The employee was exiting and dismounting the machine. While going down the steps of the machine, the employee slipped on the last step and fell to the ground landing awkwardly and spraining his left knee in the process. Employee was taken to a medical clinic where he was placed on light duty.

US Silica received confirmation, dated 7/15/2015, that employee has a reportable hearing loss in his left ear.

A commercial truck driver for SDA trucking Had an apparent heart attack that resulted in his death on 11/10/2014. This incident has been investigated on MSHA Event # 6547649.

US Silica has received, on 7/9/14, a Medical Report that confirms that the empployee's hearing loss in his right ear is work related.

Employee was closing a bottom discharge gate on a rail car. The gate was difficult to move. The gate suddenly broke free and the employee caught his finger between the bar he was using and a railroad tie. The incident was treated as a first aid. The employee sought medical attention on 10/31/12 and was diagnosed with a fracture on the tip of the finger.

Truck bed on haul truck turned on its side on haul road. Excessive speed. Driver complained of back discomfort and was taken to the medical facility for a check up. Truck was checked out and placed back into service the next day.

Stepping off bulldozer, the individual twisted his ankle.

Employee was using a 60" bar while closing a bottom discharge slide/gate on a rail car. The slide was difficult to move. The bar slipped from the slide "knuckle", causing the employee to fall forward, mashing two fingers on his right hand between the bar and RR track rail.

Employee was welding and cutting on the wet process vacuum table. Fumes from the welding process and heated Nordbak wear compound irritated eyes.

Employee was driving a locking pin into a high pressure hose coupling. Employee struck the ring finger of his left hand with the handle of the hammer he was using, resulting in a laceration of the finger. The laceration required 2 stitches to close.

Employee was at the fuel station to fill the fuel tank on a D8 dozer. The employee was standing on the dozer blade push arm, when he jumped backwards to dismount. His ring caught on the latch for the batt box cover. The skin was pulled from the ring finger of his left hand. The finger was surgically removed later at the hospital.

Employee was opening a Rail Car discharge slide with a metal bar. The bar slipped from the slide roller knuckle causing him to lose his balance. He fractured his left ankle as he stumbled backwards.

Injured while attempting to move a large rock away from a power pole. Injured was pulling the rock away from the pole, when the rock rolled back toward the pole. The injured person jerked his hand from the rock and cut the second finger of his right hand on a sharp edge of the rock in the process. Injured was not wearing gloves.

Victim suffered a broken bone and laceration of the ring finger and a laceretion on the middle finger of the right hand. The injuries occured while the victim was dumping a scrap metal box in the maint shop and was struck by the metal roller frame that supported the scrap metal box.

Employee was closing the plant entrance gate to secure the plant for weekend shutdown when he stepped on the edge of the pavement twisting his ankle. Employee did not seek medical attention until 7/3/2006. He was examined and released with no apparent damage. Employee returned to the Dr. on 7/14/2006 and was started on therapy at that time to reduce swelling of his ankle.

The victim and a co-worker were attempting to load a steel beam, 8"x10"x16', onto a trailer with the aid of a backhoe. As they were postioning the bachoe to connect the lifting chains, the backhoe struck the steel beam causing it to roll onto the victims R. foot.

Greasing monitor, applied more pressure by shifting weight against it, felt back pop.

MECHANIC WAS REMOVING BOLTS FROM THE BELLYPAN ON THE DOZER. HE WAS USING HIS LEGS FOR ADDED LEVERAGE ON THE WRENCH USED TO REMOVE THE BOLTS. HE FELT PAIN IN HIS LEFT KNEE A FEW MINUTES LATER. THE END OF THE HOSE (W/VICTAULIC COUPLING ATTACHED) STRUCK THE OPERATOR ON T HE RT LEG KNOCKING HIM TO THE GROUND. THE HOSE FLEW AROUND AND HIT HIM SEVERAL MORE TIMES. (RT HAND & CHEST) KNOCKING HIS GLASSES AND RADIO AWAY FROM HIS.

EMPLOYEE WAS SETTING THE HANDBRAKE ON A COVERED RAILROAD HOPPER, WHEN THE HANDWHEEL TIGHTENED UPHE PULLED ON THE WHEEL WITH HIS RIGHT HAND AND IN TURN STRAINED HIS ARM.

WHILE WORKING ON TP-2 THE BALL OF THE PICKER WAS TIED OFF TO THE HOSE AND THE CHAIN SLIPPED CAUSING THE BALL OF THE PICKER TO TRAVEL ABOUT 8 TO 10 FEET AND HIT THE EE IN THE HEAD.

EE WAS WORKING ON THE DISCHARGE SECTION OF THE TORIT DUST COLLECTION SYSTEM AT THE NORTH PLANT IN TRYING TO FREE THE FROZEN ROTARY REEDER, EE LPLACED A PIPE WRENCH ON THE SHAFT OF THE ROTARY FEEDER TO BREAK IT LOOSE. WHEN THE FEEDER BROKE LOOSE, EE RECD THE INJURY

EE WAS DRAGGING A WATER HOSE TO WASH OUT BE2A. HE WALKED ACROSS A GRATING WHICH SLIPPED AND HE FELL BETWEEN GRATING AND DRAIN.

EMP SLIPPED AND FELL BETWEEN RAILCARS.

EMPLOYEE WAS SHOVELING SAND AND ALLEGEDLY STRAINED LOWER BACK. COMPANY DISPUTES THIS CLAIM.

EMPLOYEE WAS STEPPING ONTO A BOAT AND HIS FOOT SLIPPED, ALLEGEDLY CAUSING HIM TO FALL AND CUT HIS NOSE.

EE WAS TRYING TO SEPARATE 2 PIECES OF METAL. THEY CAME APART & THE BIG ONE CAME DOWN OF EE'S FINGER.

EMPLOYEE SSTTEED HE HAD TO CHECK BE2A BINDICATOR. THE WIND WAS BLOWING VERY STRONG THAT DASY. WHEN HE TOOK THE COVER OFF IT WAS FULL OF SAND AND THE SAND BLEW INTO HIS EYES.

FLUID BED DRYER WAS PREPARED FOR STARTING. AFTER 2 ATTEMPTS TO START BURNER TH EPILOT FLAME WAS ADJUSTED FOR PROPER AIR-GAS MIXTURE. AFTER THIRD ATTEMPT AT STARTING THE AUTO BURNER CONTROLLE R SHOWED FLAME WAS STARTED. GAS VALVE WAS OPENEYD AFTER BURNER CONTROLLER SHOWED LOW FIR ECONDITIONS(OK), AFTER 20 SECONDS THE BURNER PELNUM EXPLOSED.

FLARE UP OF DEGENERATIVE DISC SYNDRONE RELATED TO 1992 INJURY. REQUIRED 1 WEEK LIGHT DUTY. RELEASED TO FULL DUTY 11/01/93.

BUCKET CAME OFF ELEVATOR.PLUGGED UP THE CHUTE.HE TOOK A BROOM HANDLE AND PULLED UP TO THE SIDE DOOR AND PULLED IT OUT.

WORKING ON BE-25 TAKING BELT APART. HAD 5 BUCKETS OFF. TWO BUCKETS LEFT. GETTING READY TO ROLL HEAD PULLEY OVER TO GET THE OTHER TWO. THE BELT BROKE AND THE END HIT HIS LEFT LEG.

EMPLOYEE WAS RE7URNING TO THE SHOP AFTER COMPLETEING A JOB IN THE MICRON BUILDING, WHEN HE EXITED THE BUILDING HE SLIPPED ON STEPS COATED WITH ICE AND FELL, STRICKING LEFT ELBOW ON EDGE OF STE P. RESULTING S4ALL CUT REQUIRED SEVERAL STICHES AND ANTIBIOTICS WERE PRESCRIBED. EMPLOYEE RETURNED TO WORK AND COMPLETED SHIFT, BUT REPORTED OFF THE NEXT DAY DUE TO LOSS OF SLEEP.

EE WAS ATTEMPTI3G TO OPEN THE TOP HATCH OF A GRAIN TYPE COVERED HOPPER. WHEN THE LID WAS PARTLY OPENED IT CAUGHT & WHEN THE EE PULLED ON IT HE FELT PAIN IN BACK. EE WORKED NEXT SCHEDULED SHI FT AFTER ACCIDE3T THEN REPORTED OFF 3 DAYS BEFORE REQUESTING MEDICAL ATTENTION.

EE WAS REPAIRING STEEL CABLE ON A SMALL HOIST WHEN WIRE FROM CBALE PUNCTURED RT THUMB. HE WAS WEARING GLOVES AT THE TIME OF ACCIDENT & THOUGHT IT WAS TOO MINOR TO REPORT. 3 DAYS LATER SLIGHT INFECTION DEVELOPED & MEDICAL ATTENTION WAS REQUESTED. ANTI-BIOTIC WAS PRESCRIBED.

EE WAS USING STEEL BAR TO CLOSE A SLIDE GATE ON A R/R COVERED HOPPER. THE BAR SLIPPED CAUSING EE TO LOSE HIS BALANCE & FALL. HE PUT HIS RIGHT ARM OUT TO CATCH HIMSELF. IN SO DOING HE CAUSED S FULL WEIGHT TO BE PLACED ON THE RT ARM DISLOCATING THE SHOULDER.

EMPLOYEE WENT OUT TO CHECK GAS PRESSURE ON #2 DRYER. WHEN HE STARTED BACK TO CONTROL ROOM HE LOST HIS FOOTING ON PAVEMENT WHICH HAD BECOME SLIPPERY DUE TO SNOW FALLING AT THE TIME. HE FELL AND THE BACK OFHIS HEAD STRUCK THE PAVEMENT, CAUSING CONTUSION OF HEAD AND PULLED BNECK MUSCLES.

EMPLOYEE STATED HE HURT SHOULDER WHILE SWINGING SLEDGE HAMMER ACCIDENT WAS NOT REPORTED AND MEDICAL ATTENTION WAS NOT REQUESTED UNTIL FOLLOWING DAY
