Employee was positioning a diesel heater into place after fueling. As EE released the handles and went to stand up, EE felt EE's back slip and a numbing sensation in EE's lower back and leg.
Third Party Truck Driver pulled up to outbound safety rack, truck was too far forward. CSP employee instructed Driver that the Driver had to back truck up to use the rack, Driver refused to move truck, ascended rack and continued to improperly use rack. CSP employee reiterated to driver to stop and reposition truck. Driver continued, tripped, and cut leg on frame.
Employee driving a haul truck descended down a ramp in the pit/mine. Driver did not notice cross ruts from the previous shift. Employee drove through the ruts and was slammed into the window, resulting in broken glass and lacerations to the face, ear and head.
Employee was assisting in installing a tail pulley guard after replacing the conveyor belt. While trying to line up the bolt holes, the guard shifted and pinched left pinky finger, requiring stitches.

Employee was topping off the auto greaser on an excavator. The employee was struggling to disconnect the fill line and jerked on it, causing EE to lose footing and fall to the ground. The fall resulted in a cracked rib.
Employee was changing a tail pulley bearing. The all thread part of the take up was ceased up. Once the all thread broke free, while using a break away bar, the employee's left middle finger was mashed between a lifting eye and break away tool. Resulted in a laceration and broken finger tip.
Two employees were replacing guarding on the side of a conveyor. The guard slipped and pinched the left index finger between the guard and frame work, resulting in need of stitches.

Employee was walking to the break room after clocking in. Lost footing and fell.
Employee lost footing after refilling the printer, causing a loss of balance and falling. The employee caught all their weight on their right arm which resulted in a fracture.

Employee ascended a fixed vertical ladder to inspect an agitation tank. On the way back down, feet slipped off the ladder rung and fell.
Employee was doing maintenance on Dryer-100 where employee smashed the tip of thumb between a steel plate and drive chain.
While walking on scale, employee lost balance and twisted knee.
The employee was cutting tape off of motor leads to the disconnect fan. EE then cut the right thumb to the back side of EE's hand. The cut required stitches.

Miner was assisting in the removal of a HDPE pipe from wet-processing equipment. As the pipe was being lifted, the load shifted, pinching the miner's right thumb between the load and the equipment's structure.
